I had a rhinoseptoplasty in 2022 for fix a bulbous tip with slip cartilage, a deviated septum, and large nostrils. About one year later I had a cartilage graft done in office under local anesthesia to fix a small bump (this was successful and not an issue).

Overall, there is definitely a huge improvement from my previous nose. However, I’m still not super confident. I feel like the tip is still bulbous and the nostrils are long and pinched. A small bit of scar is uneven on my columella as well. From certain angles and if you feel my nose there are lumps and dips along the top and while I know nobody is 100% symmetrical, I think my nose is quite asymmetrical and it looks weird. My scars are also quite visible from the side.

That said, I still feel my nose is too big for my face. I’m only 5ft tall and I just want to feel pretty. One reason I got surgery in the first place was bc I realized I had so many more pictures of myself during COVID since I wore a mask and you couldn’t see my nose. Originally, I was adamant about a female surgeon but the one I went to recommended a male bc she said my nose was complex. I ended up using him and he’s super well respected in the southeast. I went to him a few months ago and he said he didn’t think I needed one but if I still wanted a revision in a year he would do it. Is it even possible to get what I want? Or am I making a mountain about of a molehill?

ETA: could’ve phrased it better but regarding the “goal nose” it’s more of certain features I’d prefer to see in my nose not the actual other persons nose if that makes sense. Like I’d want my tip to be roughly a certain shape or my nostrils one way.

I had a rhinoplasty done in 2022 and a second in office procedure in 2023 to Zyrtec a small slip in the cartilage with ear cartilage. For the most part, I am happy with my results but I still don’t love how my nose looks in pictures. I’m a small female (5ft) so I think it’s still a bit large for my face. There also seems to be asymmetry on the tip and nostril. Additionally, it’s not that visible in the pictures, but if you touch the bridge of my nose, there are lots of bumps and dips. Could a revision procedure resolve this? Or am I hyper fixating on a non issue?

I had a rhinoplasty done in 2022 and a second in office procedure in 2023 to fix a small slip in the bridge cartilage with ear cartilage. For the most part, I am happy with my results but I still don’t love how my nose looks in pictures. I’m a small female (5ft) so I think it’s still a bit large for my face. There also seems to be asymmetry on the tip and nostril. Additionally, it’s not that visible in the pictures, but if you touch the bridge of my nose, there are lots of bumps and dips. Could a revision procedure resolve this? Or am I hyper fixating on a non issue?

Revision?

I had a rhino/septoplasty in 2022. 11 months after I had an in office touch up where ear cartilage was placed in one section where there was a dip. I know that I’ve had a huge change from where I started and I can definitely breathe better but I am still not super confident with my nose. Definitely not like pre surgery but still I rarely like how I look in pictures.

Recently, I have felt that my nose still looks too large for my face. It also has developed small bumps and dips along the top. I feel that the sides are also noticeably asymmetrical. I also have very thin skin and my cartilage is not strong. I went back to my original surgeon to see about a revision but he said he didn’t think I needed one and that he thinks that what I’m noticing isn’t noticeable by others. Should I get a second opinion? I’m a really small person so I just don’t feel that my nose fits my face. Should I do a revision?

Hi everyone!
I’m a FNP student in my first clinical rotation. For one, I had to pay for this rotation thru NPHub, and I live in a major city so….it was not cheap.
I really liked my preceptor the first couple of shifts. However, the last 1-2 shifts there’s been a change in the energy a bit. It’s not just me, it seems like she has an attitude with all of her staff, so I don’t think it’s related to my work… I feel pretty confident with my training thus far, although…she’s not exactly teaching me much anymore? I’ve done 4 shifts so far. I’m basically already operating on my own, and then going to her for confirmation of what I think I should order. I don’t feel like I’m being taught much. She has her own practice, so it’s just her and sometimes another NP is there, but that’s it. She’s basically catching up on other work while I see the patients. So I’m essentially paying HER to act as her employee…great, right?!

She gets very irritated when I ask questions that are in this 20-page packet that she gave me. She doesn’t have any MAs or phlebotomists or anything at all, just front desk staff so I’m also doing all my own vitals and labs. I’ve never had to spin the blood tubes before, etc…but again, it’s in the packet I guess lol. But I’m sorry, the amount of time it’s going to take me to flip through these 20 pages to figure this out is just wasting time when you can give me a 2-second yes or no answer. The way she spoke to me last shift though was overall really off- putting. She treated me like I was an idiot when I wanted to confirm with her about what I needed to do with some blood tubes. Then I was letting her know that I wasn’t familiar with the EMR software she uses and that it’s a little hard to navigate sometimes (obviously with the point of, I’ll just need to get used to it), and she goes, “it’s actually not that hard.” Like ok, lol. There were other comments but that’s the general vibe.

So last shift she tells me that she’s not going to be there one of the days in the upcoming week because she has to testify in some court case (not healthcare related I don’t think, but still wtf lol), and she goes “you’re still gonna be here seeing my patients though”. I kind of laughed and said ok because I didn’t know how else to react in the moment.

I thought about it when I got home, and obviously there are multiple ethical issues here…for one, I’d basically be operating without a license. And also, I think it’s not fair that clearly she’s going to be charging these patients as if she’s the one seeing them, when she’s not even on site. Not only was she not going to be on site, but clearly unreachable since she’s gonna be in court…lol.

I ended up not going to clinical this past week because I was really sick, but I’ll be back this week. So this date has passed already thankfully since I was sick. I don’t want to jeopardize my hours, but I feel like I should make it clear that I’m not comfortable seeing patients when she isn’t there.

Am I crazy, is this normal?! Should I say something now, or wait to see if she tries to put me in that situation again? I also don’t want to come across as accusatory, I’m pretty sure she’s not going to react well at all to that, clearly! 😬 thanks for reading!

Is this okay to do? Trying to avoid a psych emergency.

21F. 98lbs. 5’5”. Taking Adderall, sertraline, Wellbutrin, bupropion, vit D, gabapentin PRN, breztri, albuterol. PMHX sexual abuse, depression, asthma, eczema. Non smoker. Social drinker.

I have a family member that I suspect has bipolar disorder. She has always had big emotions and a flair for the dramatic and she tends to see things in black and white. She also lies a lot over stupid stuff. Her tone/attitude/behavior can flip very suddenly, especially if she’s embarrassed or feels like she’s been wronged. For example, she got in a minor disagreement with another family member and blew up far out of proportion with tears, shouting, cursing, teeth gritting and kind of full body shaking/vibrating. Just way out of control. She also regularly threatens to kill herself, though I don’t think she actually would, she usually only does this when she’s trying to guilt trip someone with something she’s holding over their head from a while ago or when she is getting “in trouble”. Obviously she’s an adult so she doesn’t get in trouble like a kid it’s more like when she’s staying at her parents home during the summer and stuff and she gets asked to do something and she doesn’t and someone gets irritated with her.

Even before her SA, when she was a little kid and younger teenage she had a sort of victim mentality. Her father is also a narcissist who probably also has autism and he did a lot of triangulating and manipulating and verbal and emotional abuse when she was younger. So that definitely doesn’t help.

When bipolar was mentioned by a past therapist she got mad. Her aunt has bipolar disorder and it wasn’t well controlled for a long time so I think she’s got a stigma attached to it and I don’t think she realizes that if she gets treated she can live a normal life and that her aunt is more or less an anomaly who was propped up by someone who didn’t believe in medicine and would take her meds. She actually reminds me a lot of this aunt. Her grandma in this same side had untreated OCD, untreated depression, and probably a personality disorder. Grandfather on the same side is a sociopath and narcissist.

She does think she has autism, which I agree with. Idk how much that could be playing into this. Anyway, this family member sees a psychiatrist and a therapist. Since she’s an adult she goes to appointments herself but some of her behaviors recently have been concerning to me. I don’t think she’s a danger to herself or others (yet anyway) and I want to keep it that way. **Is it wrong for me to write a letter to her therapist and psychiatrist explaining my concerns?** She acts different in public than she does in private, her voice and mannerisms are all different. She also exaggerates a lot and stretches the truth, especially if she can make someone else look bad. I really want her to succeed because she is so smart and has so many ideas and goals. **Would reaching out to her psychiatrist help? I just don’t want her do deteriorate further and have a psychotic break or something**. Her behavior has been increasing in volatility and recklessness I guess. She was at a party and had a shot ton of alcohol but said she wasn’t drunk and didn’t feel drunk. Hasn’t done that in a bit though.

This is actually driving me insane. I redid my budget this year so I could pay for ESPN+ and just accepted that I would miss any regulation games on TNT/HBO. I bought HBO just for playoffs because I definitely don’t want to miss those games and plan on cancelling it after playoffs, but now there are also games only on ESPN 2. So I missed the Minnesota game yesterday and now I’ll miss the Montreal game tonight. All because everything needs a different subscription. It’s actually infuriating.
