Low morning cortisol, low/normal acth, but normal 4pm cortisol and 24h urine cortisol?

Hi all. Was diagnosed with adrenal insufficiency this week after having an adrenal crisis and metabolic acidosis. Started hydrocortisone and feel like myself for first time in a year. But i just got a result in my chart showing normal afternoon cortisol the day i was admitted and normal 24h urine from the week before despite low cortisol (4ug/dl) and low borderline normal ACTH (2.1). What can this mean ? Is this still adrenal insuffiency. I'm so confused

Doctor and I are lost on next steps with my results..any input would be appreciated.

Hi all. I am very lucky i have an amazing GP, but where I live you cannot be referred to an endo unless you are within certain ranges for results, and every test we do my results are "low/normal" or "possibly suppressed and require further investigation" etc. There is no option to get a private endocrinologist where I live which is very frustrating.

So far we have done my morning cortisol which was 183 nmol/L (6.6 ug/dl), which was above the 130 threshold but below 300 which warranted further workup.

My ACTH was 2.1 PMOL/L, which was above the threshold of 1.6, but still says cannot rule out adrenal insufficiency as it's low normal and that combined with low cortisol may point to secondary adrenal insufficiency.

We are awaiting prolactin results but just not sure what to do. My doctor does not know much about low cortisol only high cortisol and we aren't sure whether to keep going on this journey or what. Has anyone experienced similar results? My symptoms point towards adrenal insufficiency which is why we explored this:

-extreme exhaustion to the point of lethargy

-nausea, loss of appetite, weight loss

- blurry vision, unexplained

- dizziness, lightheadedness

- no temperature regulation, either freezing or very hot

- muscle weakness to the point i can hardly pick up my 2 year old son when i was very active, walking my dogs feels like a marathon or climbing everest.

- headaches

- mood changes, severe depression and irritability

- 0 coping mechanisms for stress, its like i lost that ability and the smallest thing feels insurmountable

My electrolytes are normal, sodium, potassium, everything else is normal which is why we looked into this.

Id appreciate any insight at all. Thank you so much

Positive story for a change... after a year of couples therapy, I woke up to a spotless house, breakfast and was catered to all day for mothers day❤️

My husband and I were fighting daily to the point we seperated for 3 months last year. I was frustrated because we have a 2 year old and I was doing EVERYTHING. Cooking, cleaning, shopping, the mental load of remembering everything for our son, classic gender roles. I work a lot more hours than him and I honestly just broke and told him change or I'm out and ended up leaving. He suggested couples therapy and let me say hes put in the work. I feel valued, heard, and it has been consistent for a year. Our relationship has never been better and we are closer than ever.

Tl;dr If things are tough, don't give up right away, if someone loves you enough they will actually put in the work and your life might be completely different a year from now

I sleep with my dog and every morning I get a 65 ish sleep score and it tells me I was restful. My dog sleeps like spooning me and she twitches in her dreams. It doesnt wake me but I'm pretty sure the movement oura is picking up is her, not me lol. It is telling me I'm awake 2 hours a night which I'm definitely not. Its interesting because my samsung watch does not pick this movement up or interpret me as awake. Anyone else?

Following my kidnapping and years of abuse and being locked in rooms and or having siblings locked in rooms and trying to get them free, I experience severe panic when I close the bathroom or bedroom door. I dont even lock stalls in public. I just hold them closed for quick escape. My husband always tells me to close the door and I just can't explain to him why its so scary, he wouldn't understand. Even when he locks the bathroom door to go, if i try to open it (sometimes we just close it with no one in there if the dogs are trying to be destructive) and its locked, i panic cause it reminds me of when my sister was locked in her room and i couldnt get her free.

Anyone else experience this? How can i overcome this? I'm 30 years old and a mother and I just want to be normal....
