29/M NEED URGENT Help !! Hairfall has gone worse even after using finasteride, Minoxidil & Ketoconazole shampoo.

Hair loss worsening rapidly after 1 year on Fin, Min, and Keto (was working, now losing ground). Need advice.

Hey everyone, looking for some insight or advice on what might be going on.

Background & Protocol:

I’ve been on Finasteride, Minoxidil, and Ketoconazole shampoo for about a year now. Initially, it worked amazingly well—after a few months, my hair fall dropped to practically zero.

Back then, I was unemployed, eating a solid gym diet, and working out regularly.

Recent Changes (Last 3–4 Months):

A few months ago, I started a new job. Here’s what changed in my daily routine:

Commute: I ride a bike ~30 mins each way (1 hour total daily) wearing a helmet. I always wear a balaclava underneath and keep everything washed and clean.

Workplace: I work in an fully air-conditioned office. However, I keep a high-quality cap on almost all day. I don't feel like my head is sweating much, and I wash my caps frequently to keep things hygienic.

Routine: My overall diet and daily routine have naturally shifted a bit since starting the job.

The Problem:

Over the last 2 months, my hair fall has increased rapidly. It’s reaching an alarming rate—I'm now seeing my scalp clearly, which used to only happen when my hair was cropped super short or directly under harsh lighting.

I’ve visited a couple of doctors locally, but they weren't very thorough and just handed me generic prescriptions without looking into the root cause. Any guidance on what steps or blood tests I should ask for next would be greatly appreciated.
