Excuse my language but why the F$&* do these bolts keep coming loose?

I replaced the entire flush mechanism about 3 months ago. Every 3 weeks or so these little shits come loose. My only tell is seeing a little puddle of water behind the toilet.

What's causing this? How do I stop this? I set up cameras to monitor and watched 3 weeks of footage to see if any gnomes or psychotic neighbors are coming in my bathroom to loosen these while I sleep just to fuck with my mental health.... saw nothing....

Please send help before I end up welding them on....

26 explorer ST line. Map worked for a few weeks but now does this.... never loads in android auto, works fine in the ford split screen.....

Already enable background unrestricted battery usage.... any other than ideas?

Split the cap off and rolled it.... salt and pepper on half and Dan 'Os other other. High heat stainless steel with some roasted potatoes and mushrooms.... thoughts?
