Intermountain Health had a medical record conversion around 08/2025. This caused the MEGAHIT records request received by ssa/dds to not list dates of diagnosis and only lists 08/2025 as the "date noted". (Which in my CDR was used against me) It also only listed a date, location, and my primary care physicians name for any appointments I had with them before 08/2025. So 2/5's of the visits my claim was to be judged on had no information. And the visits weren't even noted in my DDE, what was noted was a lack of continuity of care. So please check your records.

Question about hierarchy and wiseness of possibly doing something CDR appeal

Is a DHO a higher level than the person who originally did my CDR. And would it be beneficial to point out multiple points of my cdr from neglect to not following protocol to using things out of scope etc?

One i got was SUGAR-CHIPPER-MACHINE OPERATOR (grain-feed mills) alternate titles: sugar grinder.

Went for case file request and they gave me a dvd with 10 medical records on it, didn't even have anything about my cessation on it

What are the magic words to actually get my case file? The record form SSA-3288-OP1 I submitted was in April.

Increasing APE grade and property value?

Cant seem to find anything that talks about how much property value increases by going from a low grade (F) to higher (A1-4)

If anyone has an article or something they can point me to that would be great. (Doesn't need to be in English)

Finally figured out how to describe my feelings about the new ui

  1. Multiple times I've thought "God I wish (insert streaming service) had this ui" with classic ui.
  2. If a service switched to the new ui, I would cancel my subscription. (Looking at you Netflix)
  3. and the cherry on top is changing my most commonly used remote button press (up during playback to bring up the menu) to show me a playlist type thing?

What can I do...

Cdr cessation letter in April, 2 days later filed appeal and continuation of benefits (both) using web portal. Only got a letter for continuation of survivors, nothing for ssdi. Have called ssa 5 times they all say I'm getting both payments next month. BUT when I called Medicare they said my coverage is ending at the end of the month, this is why the state is cancelling assistance on the 24th. The soonest appointment I could get into local ssa is the 22nd.

I can barely manage to deal with this because I'm taking meds. Suddenly stopping them will leave me unable to handle any of this.

The 1800 ssa number seems useless for anything beyond checking case status, seemingly from a lack of access to systems or a lack of training.

Not sure when this was implemented

But chrome password manager had a toggled on "automatically create a passkey" which my understanding is that law enforcement can force you to use biometrics to unlock anything. Probably just being worried for no reason. But Google hasn't given me a reason to trust anything they silently implement for years now.

Had reviews before, always saw a mandatory psychologist for an eval, not this time. This time they listed my mental health diagnoses, then said this:

"We considered the medical and other information as well as work experience in determining how your condition affects your ability to work. We do not have sufficient vocational information to determine whether you can perform any of your past relevant work. However, based on the evidence in file, we have determined that you can adjust to other work."

I already filed the continue benefits form online as well as a request for reconsideration (appeal). But I can't find any mention of something happening like this before here with the "can adjust to work" wording. There's no explanation given just 'gfy'.

What else can I do?

The only medical records it shows them accessing are my PCP, (clarification) The only appointments I've had have been with my PCP (of ~10+ years) for med management, as my psychologist got dropped by insurance in 2022, and also a psych ward stay in 2024.
