[Ukrainian - English] Birthday message text

One of my coworkers birthdays is soon and we're making a card for her. She's one of my favorite people on Earth and I always wanted to write something to her in Ukrainian considering she teaches me words, and I in English. I asked another coworker from Ukraine to translate "Happy Birthday to my favorite person from [company]" and I don't doubt for a second what she wrote is incorrect, I'm just having difficulty understanding the alphabet and what the letters look like in default text because I want to rewrite it in my handwriting on the card.

Any help with this would be greatly appreciated.
