Facelift, Transconjunctival Lower Eyelid Blepharoplasty & Fat Grafting — Seattle, WA

This patient came to me bothered by the aging appearance of her lower eyelids, face, and jowls, and she also felt she had lost definition along her neckline and in her chin area. Her goal was to look refreshed and natural rather than dramatically different. To address her concerns, we performed a facelift, a transconjunctival lower eyelid blepharoplasty (an approach through the inside of the lower lid that avoids any visible external scarring), fat grafting to the jawline and perioral areas, and a chin implant. She now shows a softer, more oval facial contour with a meaningful reduction in the lines around her eyes, mouth, and jawline, and her neck and chin definition have come back noticeably. Feel free to drop any questions below!

Uneveness at 7wpo breast lift

Hello,

I come here to know if any of you experienced this, and if it got better?...

I've been like this since the 4th week or before, i dont remember now because at that moment I had more serious issues like seroma and under breast folds because of it.

If it doesn't get better I will need a revision and I feel devastated by the thought of undergoing surgery again since I had a lot of complications and struggled a lot with my mental health too (I have borderline personality disorder) :/

The swelling on the left (your right) subsided pretty quickly, however the right (your left) gets worse and then better and then worse again... I dont know what to do anymore, I'm seriously at my limit right now. Im right-handed so perhaps that plays a role in the swelling? Well, im trying to use more my left now...

Any tips will be pretty much appreciated

Thank you for reading

2-Month Update: Verteporfin Trial on Leg Scar Excision with Dr. Bloxham (New York) Baby hairs and melanin returning

Here is my 2-month update on the Verteporfin trial for a body scar. The procedure was a scar excision (left completely unsutured) performed by Dr. Bloxham in New York, using a 3 mg/ml dose of Verteporfin.

What happened so far:

Delayed Healing: We noticed a significant slowdown in the healing process caused by the Verteporfin, which was expected.

Suboptimal Aftercare: Unfortunately, the wound care wasn't perfect. A thick scab was allowed to form, and the area was accidentally exposed to direct heat sources, which might have compromised the final result.

Current Observations (2 Months In):

Baby Hairs: We are seeing brand new baby hair growing right in the middle of the wound. This is huge, as hair follicle regeneration is impossible in normal, fibrous scar tissue.

Repigmentation: The tissue seems to be regaining melanin. It used to be completely white and featureless, but now the color is blending back in.

Our Take & Expectations:

We likely won't get a 100% "scarless" result due to the lack of sutures and the aftercare mishaps. Instead, it looks like it will be a hybrid of fibrous tissue and healthy skin. However, the hair growth is an incredibly positive and encouraging sign of regeneration.

Next Steps:

We plan to run more trials in the future using the same dose (3 mg/ml) or slightly lower on other body scars. Crucially, we will use sutures next time, as we noticed the outer edges of this wound—where the tension was lower—are healing almost completely scarless.

IF YOU ARE FROM NY AND WANT TO CONTRIBUTE DR BLOXHAM IS SEEKING NEW PATIENTS TO TREAT THEIR BODY SCARS.

The Perpetual Dissatisfaction Look

Perpetual dissatisfaction loop—a condition of modern life, deeply embedded in how our minds have been conditioned by society, capitalism, and even survival instincts. Here are more examples of how this plays out:

  1. Body Image: You lose 30 pounds, but now you’re insecure about loose skin. You get surgery, then wish your waist was smaller. You get implants, then feel your nose isn’t right. It never ends. There’s always something to “fix.”
  2. Love and Relationships: You finally find someone who loves you deeply, but then you start to notice they don’t dress well enough, or they don’t make as much money as you hoped. You once prayed for love, but now you crave admiration or status instead.
  3. Social Media Validation: A post gets 200 likes, but your last one got 400—now you feel ignored. You gain followers, but you compare yourself to someone with 10x more. You always need a bigger audience to feel seen.
  4. Career Climbing: You get promoted and feel excited—for a week. Then you find out someone else your age is a VP. Now you feel behind again. Success is short-lived when you measure it by someone else’s ruler.
  5. Home Ownership: You buy a house and feel proud, but then your neighbor renovates theirs. Now yours looks outdated. You were once thrilled to have a backyard, but now you want a pool, a home theater, and a three-car garage.
  6. Vacations: You go on a dream trip, but see influencers doing luxury getaways in the Maldives with private chefs. Suddenly, your beachfront Airbnb feels cheap. You can’t even relax because your trip doesn’t “look” elite enough.
  7. Friendships: You have loyal, loving friends—but you envy people with huge friend groups who throw lavish birthday trips. You begin to question whether your circle is “enough.”
  8. Parenting: You raise a healthy, kind child—but feel like a failure because your friend's kid speaks three languages or got into an elite school. You forget how rare a happy child actually is.
  9. Spirituality and Wellness: You start meditating, eating clean, and journaling, but then feel inferior to someone doing 4 a.m. cold plunges and silent retreats in Bali. Even peace becomes a competition.
  10. Wealth: You become a millionaire, but then feel poor compared to a billionaire. Once money was freedom—now it’s just another rat race at a higher level.

This is the curse of always living in "not quite enough." Even joy becomes muted when everything is a status comparison.
The darkest part?
Many people spend their entire lives chasing upgrades they never needed… only to feel empty when they get them.

Rhinoplasty - 7 months post-op (Livingston, NJ)

This patient is 7 months post-op following a complex functional and cosmetic rhinoplasty to address an overprojected and pointy nasal tip, deviated septum, tension lip deformity, and dorsal hump. External nasal valve weakness and turbinate enlargement were also corrected, had the valve weakness gone unrecognized preoperatively, breathing would have actually worsened after reduction. Lateral crural strut grafts were used to support the airway, the septum was corrected, and turbinates were reduced. She's already showing a more natural, less distracting nasal appearance with significantly improved airflow, and we're all thrilled with how beautifully she's healing. You may also notice her lips appear subtly fuller, the result of microdroplet Silikon-1000 for permanent lip augmentation. Feel free to drop any questions below!

4 weeks post deep plane face and neck lift

I wanted to share some before and after photos following a deep plane face and neck lift. The after is 4 weeks post surgery.

I am almost 42F, which sounds young for this procedure (I thought the same before I had it) but I had the start of a visible turkey neck / platysmal banding, which you can see in the before photo. I had spent a small fortune on minimally invasive procedures to try to fix the problem, but decided in the end to go for what I knew would work.

Happy to share any details about the procedure or recovery for those interested!

EDIT to answer some of the common questions I'm being asked.

The doctor was Dr. Prem Tripathi in the San Francisco Bay area. I am very happy with the whole experience and the result. One thing I really appreciated is that the doctor did not try to push procedures on me. I asked him about other possible complementary procedures, and he straight up said I did not need them.

I paid $30k for the dp face and neck lift. If I had just done an extended contour neck lift, it would have been 20k. Adding a browlift to the face and neck lift would have made it 37k. Disclaimer: I think the prices are not universal, they depend on the individual. If you call the office, they will tell you ballparks, though. The prices include a full face laser, usually 2k. They also normally include fat transfer to the face, but I did not do this (don't want to male my face more puffy, plus doctor said I don't have enough fat).

Recovery was pretty easy. I basically had some jaw pain for maybe 1.5 weeks, akin to tmj pain. It was not bad at all and mostly just at night. I would say I was presentable to the point people would not think I had surgery by day 8. There was some tightness around the chin for maybe 1.5 weeks were it felt like I had a chin strap on even when I didn't, a really weird sensation, but that went away pretty quickly. I still have some numbness around the ears, but it is not uncomfortable and should resolve with time. Otherwise back to normal 100%.

The incisions are around and behind the ears and under the chin. I am using silicon scar strips which should help them fade. The doctor will laser them to help diminish them free of charge, if necessary.

Short lasting facelifts

i’m seeing a growing trend within the plastic surgery community of surgeons for SMAS or deep and doing old-school “facelifts” that don’t last more than a year. Let me know if this has happened to you.

Facelift, Brow Lift & Upper Blepharoplasty — 10 months post-op (SF Bay Area)

This patient is 10 months post-op from a facelift, brow lift, and upper blepharoplasty. Her goal was to look like herself, just more rested and youthful. Her recovery was smooth and seamless, and she was able to return to her routine within a few weeks while her results continued to refine. As with all my surgical patients, her scars were treated at 6 weeks with an erbium ablative laser. We also used an Nd:YAG laser to fade a venous lake on her lower lip. Drop any questions below!

Hi, I'm hoping someone kind here can give me some advice or peace of mind because I'm currently spiraling. I got a septorhino 6 days ago (early on I know) with slight bump removal, tip refinement and turbinate reduction. I specifically told my surgeon that I don't want a piggy nose and the simulation we agreed on shows that clearly, I still have a columella from the side. Fast-forward 6 days and my nose is literally pin straight from the side, I look like I have a snout. I'm overwhelmed and confused as it's not what was agreed on. Is it potentially just because of extreme swelling that the columella is tight and cannot descend? Or did he really screw me over... Does anyone have experience with this? I'm willing to share photos. Would so appreciate feedback 😭🙏
