Tirzepatide panic attacks and other side effects

I’ve been on tirzepatide since mid February 2026, the last 3 weeks I have experienced bad side effects, worse than when I started, panic attacks, extreme fatigue, nausea, etc. I’ve done micro doses from the beginning. has anyone started on tirzepatide and switched to semaglutide and done better with it?

Hey all need advice

Hey, this is probably going to be very silly but.. my mother started tirz today. She bought 2.5mg vial and her friend told her to add 4ml (400units) bac water to the 2.5mg vial. And said after reconstitution it will give her 4 doses. She started on 1mg dose.

This doesn’t seem right to me. I’m on Reta
Any advice pls- does this seem right?

10mg day 1

Well, that is like a day and night kindof feeling from 7.5 to 10!

The 'burps were there, but nothing different or worth noting.

Then, I woke up feeling too full on my stomach still (hello slow gastric emptying). Went to the loo to pee, and I ended up vomiting before peeing for the 1st time since I started on GLP1 medication.

Again nothing serious, there was no food in the stomach obviously, and I ate in deficit all day yesterday as usual.

It is a note-worthy difference in side effects though.

Feeling good overall (well, after I cleaned up self and surrounding areas, and changed clothes lol). No other side effects.

I will take it easier today, more water, for sure.

Any suggestions on how to eat today to ease into it, welcome. Maybe withhold coffee? I don't know. It doesn't feel like I need to do anything at all. I will nurse my water for the next few hours and take it from there.

Is my vial still good?

While filtering my tirzeptide the ENTIRE rubber stopped popped off

(earlier the entire metal part of the vial came off along with the cap when i popped it off but since i’m filtering it into a new sterile vial i didn’t think too much of it)

I had about 99% of the liquid already in the syringe regardless before the rubber stopped popped off and the rest of the vial got exposed to the outside world but i’m not worried because i filtered it through a NES filter into a new sterile vial so i think it’s fine but i guess im wondering should i be worried? i wouldn’t mind doing it all over again since i have 10 vials and would rather waste a vial then cause permanent harm to myself

Transfer kwick pen to a sterilized vial?

My dr ordered me 15mg tirz quick pen. But I want to stay at 10 and slowly go up. Like I’ve been doing with the vial and syringe. do the quick releases have dosing to go up slowly? Or Has anyone released a Kwick release pen into a sterile vial?

Dosing

I’ve been on Zepbound for 12 injections total. I was on 2.5 mg for about six weeks before moving up to 5 mg for about five weeks.

I took my 5 mg a day early because I was going away. I had very strong appetite suppression, almost to the point where I didn’t want to eat at all. I also noticed what felt like slower digestion, like food was sitting in my stomach longer than usual, I felt I could feel the food somewhat backing up into my esophagus if that makes sense.

Because of that, I stretched my dosing out to about 8 days and then took a 2.5 mg dose instead of 5, Friday night to kind of reset things.

Now it’s been about 48 hours and I feel like I’m starving. My appetite came back very suddenly and strongly.

I was originally thinking about trying 2.5 mg every 4 days to keep things more steady, but now I’m wondering if I should just go back to 5 mg instead for more consistency. What’s confusing is that I actually lost more weight overall on 2.5 mg, but I didn’t have this kind of rebound hunger until I stretched the dose this week.

Has anyone else experienced big swings in hunger when adjusting timing, stronger GI side effects like slower digestion or reflux, feeling more stable on a higher vs lower dose, or intense hunger after spacing doses out?

My doctor has told me I can be flexible specifically with the 2.5 mg dose and take it about every 3.5 days, 4 days, or 5 days. If I go back to five, I’ll have to wait until Friday, which is going to be so hard. I’m so hungry.

I’m just looking for feedback if anyone has gone through something similar.

Transition from t!rz to re+a?

I am currently in the first week of transitioning from 15mg tirz to 2 mg of reta. I don’t feel that much appetite suppression at all and have no side effects so far from it (on 4th day since starting) but I’m curious … people who did this transition what was it like for you? Should I up my dose to 4mg this upcoming week or see what happens? Is it worth it in the long run to stay on reta instead of tirz? I have cag too but didn’t do that with it yet to see how my body reacted to a different GLP first. I actively workout and strength train 4-5x a week and also on ghk-cu 2mg daily. Any insights would be helpful :)

Reconstituting/Vial Question

I have my 30mg of powdered Tirz from Ion Peptide, curious because it says “actual amount: 32.16mg” at the bottom. Does this affect reconstituting? TIA
