Cat keeps pooping outside the litter box. No idea what to do.

My cat (12m) was diagnosed with colon cancer about 3 years ago. Takes his meds like a champ every day, he's had ups and downs, lost weight, gained weight. Currently he's on an upswing, gaining weight back and is generally his normal affectionate self.

**We see the vet every 3-6 months to manage medication and ultrasound , we have discussed this with him. The vet insists that this is a behavioral issue "if he urinates in the litter pan, he should be able to poop in the litter pan"**

For about the last 6 months or so my cat has taken to defecating outside the litter box. He urinates in the box just fine but poops outside of the pan at least once per day, often more. Lately his defecation has taken a more direct and sinister approach towards my son (M 2.5). The cat poos outside the door to his room, across the hallway from the bathroom where his first litter box is, he poops in my son's play area on the first floor directly on his trains, he has pooped directly on my son's stuffed animals which had to be thrown away, he poops on the basement steps 10 feet away from his second litter box.

My son loves the cat, gives him treats, loves to help clean the litter pan under the cats and my supervision, the cat allows him to pet him even.

We've added 2 additional litter pans. The cat has 3 litter pans in an 1800 sqft townhouse and still constantly chooses to poop on the floor. I am replacing the rug, I need to save up to replace the carpet on the steps as it's been defecated on more times than I can count. We have gotten the wet-vac down to a science.

I get up at 6:00 every morning and check the whole house for poop in his "usual places" ( children play area, steps, hallway, next to the front door) I'm so upset that this is now my life. I wake up, and immediately put on gloves and get to cleaning the poop. We moved to a nice new house 13 months ago and the place is already trashed by cat feces.

I've had enough, I love the little fellow, but he is turning my home and especially my child's play area into a biohazard. I've toyed with the idea that this is the end of the line, but I don't know if I'm just frustrated. This morning I cleaned up 3 poops in 90 minutes.

Is this just the end stage of colon cancer in cats? Does anyone have any creative strategies for helping him re-learn how to use the litter pan? Is there no coming back from this?

I feel terrible that I am thinking about euthanasia due to this issue. If he's too sick to control his bowels and this is only going to get worse I'm not sure that this is a dynamic that I can keep up. This is a health hazard for toddlers and I hate that even my toddler regularly finds poop and tells me and his teachers about it.

What to do???
