I'm a beginner no experience with any coding language

I had i friend who asked me for help and i said i can ask reddit smart people there

So we are trying to figure out how to make a code that ask you to input you info, pause i know this sounds so sketchy but its for a school project they want to help our small town Civil Registry so the handled us this paper and they asked him to make a code that run as an exe and make you fill the spaces and then print it or get the pdf of it

For some reason my hdmi wont work on my xbox until i plug this hdmi extension, i tride to use the caple on another screen and it just worked no problem but on my xbox it dose not like it i tried another caple on my setup and it works just fine so i believe the problem is from the xbox but i dont know what is it, also when i plugged another hdmi cable and went back for the old one it worked fine until i touched the cable by mistake

Any ideas?

كنت شاغل بالي بأمر مستقبلي اطلبه من الله وكنت مركز على ان حالتي الان ما تسمح لي وظروفي صعبه، اليوم في صلاة الجمعة شفت هذا الشاب لابس هذا القميص وشدني مره الكتابه الى عليه فستشعرت شي فقلبي وكأن كل شي فراسي من شكوك وتسائلات راحت من بالي وصرت بس مركز على مستقبلي اما الحاضر فا اعمل فيه بل اسباب والباقي على الله
