My 10+ year yeast infection

So I have a yeast infection thats been with me for 10+ years. 

Got it in middle school, and brought up my green discharge to my doctor and he said that it was normal. So for years I just thought nothing was wrong until I went to a OBGYN 4 ish years ago. 

I have "CANDIDA SPECIES GROUP" according to my test results. My OB prescribed me fluconazole, which at first made a change in my discharge, but only for the first few days, then my discharge went back to the normal green yellowish color. 

My OB isn’t sure why the fluconazole is not working, but she has suggested other methods like trying anything with lactobacillus (greek yogurt, and probiotic), prescribing me more fluconazole (I’m trying the maximum dosage right now and I still have a yeast infection), and vaginal creams to insert inside. 

I really want to try the vaginal creams, but every time I do I just freeze, I tighten up, and I’m scared of the pain. How can I overcome this? I tried using tampons and masturbation but it’s the same thing. 

Also, for the yeast infection the only symptoms I have is an odor, greenish yellow discharge, and clumsiness.

Other things I do is sleep with no under wear, wear 100% cotton white underwear, panty liners 100% cotton, eat a balanced diet, greek yogurt, probiotics, water, and clean it with just warm water
