Nothing works

Just had an alternate diagnosis of FND rather than primary oromandibular dystonia. Either way my whole head feels like it's clenching all day every day. I either wear a mouthguard and my jaw never stops displacing laterally, or I go without and the tension across my nose, temples and tongue is unbearable. Botox has done nothing, procyclidine has done nothing. I'm desperate for just a tiny bit of relief but I don't think I'll ever get it. If anybody's been in my position and found a way to make life a little more tolerable I could really do with hearing it.

Ears crunching when I swallow/stiff valsalva maneuver

As the title says- two of the more unpleasant symptoms that have accompanied my oromandibular dystonia. I can stop the ear crunching for a few swallows by doing the valsalva maneuver, but it doesn't last long. The valsalva maneuver itself feels stiff and unresponsive.

Both things I'm assuming are because my masseters/other facial miscles get stiffer and tenser as the day goes on, but would be curious if anybody's experienced similar symptoms from dystonia specifically.

Thoughts on The Origin of Capitalism: A Longer View by Ellen Meiksins Wood

I'm aware this has been around for a while, and I'm woefully deficient on a lot of theory, but I found her thesis pretty convincing- i.e. that capitalism arose in rural England specifically because of the unique relationship between landowners, tenants, and wage workers, which made everyone subject to market forces at all times, contra earlier modes of production.

I also thought her general point that capitalism isn't an inevitability (i.e. there wasn't a capitalist society just waiting to emerge in every society that had merchants/trading/feudal expropriation) an interesting one, and think it's a useful counterpoint to some of the more teleological strains of Marxist thought.

I'd be very interested in hearing some counter arguments or people's thoughts on it generally- I know in particular that her assertion that the Dutch Republic wasn't really capitalist is a controversial one.

Communist Germany focuses

I managed to get Thalmann as head of the government democratically but contra the latest patch notes "Towards a Party State" requires him not to be in power. There also seem to be other focuses with impossible to achieve prerequisites. Has anybody else explored this tree? Is it just a bug at my end?

Finding it very, very hard to go on

I've made a similar post recently, apologies for the repetition, but I had some responses that I found very helpful. Recent diagnosis of oromandibular and cervical dystonia.

As well, a general tremor, muscle aches, and a fluctuating feeling that I'm going to have a seizure, which might be polypharmacy related, might not- I've had various drugs thrown at me by different doctors while we were guessing what I had, before it became clear it was dystonia. Procyclidine seems to be doing nothing.

The dystonia had a very odd initial presentation- an ear infection that triggered the initial muscle contraction adjacent to the infected ear, then about 8 months of initially sporadic muscle tensing in various sites around my head, but no involuntary movements until relatively recently, hence why I've only been diagnosed recently.

Basically I wake up every day knowing what's coming and it's just going to get worse. I can feel the muscle spasms ramp up, my jaw will gradually start displacing until it's constantly doing so, so I get about half an hour of relative sanity knowing what's coming. I can feel my the all the muscles in my head tensing up until my whole head is guaranteed to be hurting/feeling like it's being crushed by the end of the day. Something in my ears clicks every time I swallow, which becomes increasingly painful as the day goes on. My tinnitus becomes more and more exacerbated, changing in pitch and volume constantly with the muscle contractions.

I know I'm only a very short way into the therapeutic pipeline but it's been a year of pretty much unremitting pain now, and my life feels basically destroyed in terms of general quality and doing what I liked to do before. Returning to work seems impossible. The timescales of getting anything done- months, years- make me utterly despair. I feel like my consciousness is in an inescapable agonising prison.

I know people cope with far worse for far longer but I really can't see much reason to carry on- three rounds of 100 unit masseter/temporalis botox (admittedly not EMG guided and before it was diagnosed as dystonia) did nothing, and now the medication just seems to be making me feel worse. That's about it really, this is pretty much just to vent to people I've found very sympathetic.

Oromandibular dystonia- presentation changes when I wear a bite guard

Just something I've noticed and am finding weird as a newly diagnosed sufferer of oromandibular dystonia; without a mouthguard it presents as a pulsing rearward tugging of my tongue which I feel across the roof of my mouth and my nose, and around the temples.

Wearing a lower teeth bite guard takes some of the tension away from my tongue/nose but changes the movement into lateral jaw displacement instead. Has anybody else noticed something similar?

Jaw dystonia driving me insane

It never stops. I wake up and have a few minutes of sanity before it kicks in, the constant wrenching of my jaw from side to side. My head hurts all the time; wearing a bite shield is the only thing that eases off the constant tension across my nose and wrenching of my tongue. My jaw is still constantly getting wrenched from side to side.

Three rounds of botox have been ineffective. How do you cope with something like this? It's absolutely infantilised me into a lachrymose wreck and destroyed my enjoyment of pretty much anything.

This is more venting than anything else but if anybody has had similar experiences (especially non-response to botox) I'd really appreciate hearing from you.

Only relief I get is a hot water bottle wrapped around my head

Chronic pain started last July; an ear infection triggered some kind of muscle contraction that was sporadic at first, then became more and more constant, spread to the rest of my head, until now where my masseters are constantly clenching and getting tighter and tighter as the day goes on.

Went through ENTs, neurologists, and maxfax before it was narrowed down to a muscular issue, which I just didn't have the words for while going through it. Now the headcrusing pressure and tension across my nose and jaw starts as soon as I get out of bed.

By evening my jaw and tongue feel like they're being wrenched around. Three rounds of botox have been ineffective; I'm almost certain it's some kind of dystonia. Which likely means more medication on top of the cocktail I'm already on. It will never go away.

The only means of relief I have are a lower bite guard, which alleviates some of the tension across my nose for some reason, and wrapping a long hot water bottle around my head. The minute I remove either, the pain and tension come back.

My attempt to go back to work smashed on contact with it. It's been 10 months now. Getting through every day is a trial. I'm in such mourning for the great life I had before. How do people cope like this?

Oromandibular dystonia, recent diagnosis, botox ineffective

Hi all, an ear infection last year triggered muscle clenching around the infected ear, which spread bilaterally.

Presentation was sporadic at first but now is a constant; things worsen throughout the day until I get intense temple pain/pressure and wrenching of my tongue and lower mandible. TMD excluded via MRI.

Three rounds of botox have had no effect; last one was 9 days ago directly into the muscle round the TMJ and intraorally into thr buccinators, still to no effect.

*I am not asking for medical advice*, I just want to hear from anybody who has had a similar experience.

Likely masseter dystonia- botox not working

Hi all. Last year I got an ear infection that precipitated muscle clenching adjacent to the infected ear. This spread to the other side of my head. It presented sporadically at first, but now has a constant and consistent pattern. TMD has been excluded by MRI.

My symptoms manifest generally by feeling an intense tugging sensation on my upper teeth when I wake up, with feelings of head crushing around my temples and across my nose, increasing throughout the day, and increasing wrenching of my tongue and lower mandible. Valsalva maneuver also feels increasingly stiff as the day goes on.

I had my third round of oromandibular/masseter botox 9 days ago, this one directly targeting the muscles around the TMJs and intraorally into the buccinators. It again feels completely ineffective so far.

Has anybody else had experience with similar symptoms, and could you describe what your medical journey has been? Any help greatly appreciated.

Can't design self-propelled super-heavy howitzers

Probably a completely noob question I realise, but I've got the option for self-propelled heaby-howitzer support companies after researching the relevant special project, but don't seem to have any way of actually designing the relevant equipment, any suggestions for obvious things I'm missing?

Chronic masseter tension, only ever gets worse as the day goes on

I had an ear infection last year that left me with chronic masseter tension for whatever reason. Didn't have the words to describe the sensation so got fucked around by innumerable GPs, ENTs and Neurologists before I got an actual diagnosis.

It's been 8 months of hell (which I know is nothing compared to what a lot of people here have gone through) and there doesn't seem to be an end in sight.

Every morning I wake up, feel my ears click when I swallow, and the tension and tightening starting across my nose, knowing that it's just going to get worse and I'm guaranteed to feel crushing pressure round my jaw, nose and temples from 3-5PM onwards.

Had masseter/temporalis botox 22 days ago and it's done nothing but make it worse so far. Don't know how I can go on like this, it's destroyed my life. Just a rant, that's it really.

Trying to find a specific soundtrack remix

Hi all, I'm trying to find a specific remix of one of DS1's tracks on YouTube that I liked a lot, but for the life of me can't find now. I think it might have been the UNATCO theme, and sampled the "crooked bureaucrats"/"big words for a rookie" exchange between JC and Manderley. Any ideas?
