Mirtazapine, elvanse and serotonin syndrome

Hi all,
So I’m on 60mg elvanse and have started 15mg mirtazapine around 3 weeks ago. I’m waiting for CMHT assessment so for now just have the gp.
The gp suggested increasing the dose of mirtazapine to 30mg as it isn’t doing anything and my self harm suicidal thoughts etc are increasing.
However she seemed to be quite worried about the risk of serotonin syndrome.
Now I am overthinking it and I’m really stressed because obviously it can be life threatening but also if I get it I will have to stop both medications. The 2 meds combined are the only reason I am still here so having to come off them both would actually be the nail in the coffin.
Gp is chasing CMHT to try and bring the appt forwards because it isn’t until 21st sept but does anyone else take this combo of meds?
Has anyone ever had serotonin syndrome what if I have it but I don’t know I do? What happens in terms of medication does it just mean all medication affecting serotonin in the future is off the cards?
Anyone’s experiences are so appreciated because I am spiralling about it all

Mirtazapine experiences after a week

So I started mirtazapine a week ago and I have a review with the dr today. I kind of feel like it is already working a bit on the anxiety side of things and sleep obz but it isn’t helping my depression symptoms. I have also sh a few times since the last appt last week.
I do think it is working so don’t want the gp to take me off it but I’m worried they will say well how is it helping if you have increased sh.
The thing is last week I was with my long distance bf for 5 out of 7 days and we were away camping so I didn’t sh because I was with him, distracted, away from my main triggers etc. this week I have been fully on my own and circumstances have been different.
For context I was clean from sh for years relapsed around 6 weeks ago all of which gp is aware so it’s not like I only started after I started the mirtazapine.
Can anyone offer any kind of reassurance because I’m so so worried they will make me stop something which I feel has the potential to be very helpful and after a week it isn’t really long enough to tell.

CMHT?

Have as assessment coming up with the community mental health team. They have accepted my referral. For context I’m a 25 year old female, done cbt before however am just not coping at present. A lot of childhood trauma has resurfaced I have never addressed or admitted before, I have recently ish being diagnosed with adhd and autism which has affected me and I’ve started self harming and thinking about suicide again.
I have a telephone assessment on 21st sept and I am so worried and confused about how it works what happens during the assessment how much detail do they go into, how does it work having a named person in charge of your care? Do you see all members of the team or just one?
Please can someone give me some more advice about what they actually do, how long you tend to be under them and how it is different to cbt

Are 111 any good? TW: SH

So recently relapsed and I’m really trying to not give in to urges. My GP told me to ring 111 when I have the urges if I can’t distract myself but I’ve heard of a lot of bad experiences with them.
Are they actually helpful? Do they take hours to answer? They won’t ring an ambulance will they?

How to know if it needs closure?

So after a couple weeks it’s starting to heal but the edges still aren’t together yet. I didn’t think it was deep when I did it. How do I know if it needs stitches or closure in some way or if it will be ok?

Talking to therapist

So impulsively did something yesterday. This week my therapist asked me to do homework where I record what’s happened each day and how much I achieved and enjoyed it etc and send it to him before the next session in a day. we’ve had around 4 sessions now over the phone and he always asks about risk if I’ve had any thoughts etc and I’ve said no.

2 issues really. 1. Idk if I should record what’s happened in the diary for homework. I really struggle to talk about it so part of me thinks if I don’t write it down I will just deny it when I do want him to know. But I also don’t want him to be worried all day while we wait for the appointment. 2. I feel bad for lying to him previously. And I think he will probably be a bit annoyed/disappointed because I guess if I’d told him maybe he could have given me some stuff to help and it wouldn’t have happened.

Already ashamed, first time somethings happened in a few years and I just don’t know the best or easiest way to bring it up. Any advice is appreciated

Support with self harm thoughts

25 F, mental health has been massively declining in the last 6 months or so mainly due to work stress, cutting off my abusive family and a recent autism diagnosis. Because of how I grew up, I’ve been conditioned to be terrified of asking for any kind of help or engaging with services. I recently reached out to work and my GP about how I was feeling but I can’t be fully honest with them. My GP was very unhelpful, made me feel daft for even going and dismissive. I saw another GP who was helpful and has referred me to CMHT but obz they reject loads of referrals so not feeling to hopeful.

When I was 18-21 ish I had a poor period of mental health couple of attempts and self harmed regularly. Recently the self harm thoughts have returned and they are getting more intense and harder to ignore.

My area has a mental health advice line which is 24 hours, and I’ve rung it a couple times but when someone answers I hang up. I can’t bring myself to get the words out over the phone or in person. When the first GP asked I said no, when the second GP asked if I had any thoughts I said not really kind of hoping he would probe that but he didn’t.

I know it’s my fault I can’t reach out and it’s stupid and they can’t help if they don’t know but I don’t know how to move past that

What do the community mental health team do?

Hey, so I have adhd and recent autism diagnosis, long standing anxiety and depression etc. my GP has referred me to the CMHT. I have heard lots of bad things about them and overall I just don’t get who they are or what they do. What is the process, what can they help with, who is the team made up of etc. any advice or experiences are very welcome

Getting over therapy sessions

So whoever I have a therapy session (I do mine over the phone) I feel bad mentally. It’s hard to explain I guess it’s not bad it’s just emotionally dysregulated and ruminating on stuff I didn’t say but wanted to or over shared or on past things we’ve discussed. I honestly feel like I need a therapy session straight after to get over the therapy session!
Is it normal ish to feel like this and how do people get over it?

ADHD, Autism, burnt out need help

So I’m a 25 year old F in the UK. All my life I’ve had issues with motivation, emotional regulation, impulsivity, major social anxiety etc. I lived in an abusive household with my mum who had MH issues and addictions and my little bro.
When I got out that house my life fell apart. I was naive and thought when I’m out everything will be fine and I’ll live a normal life but that stuff follows you childhood trauma. First year of uni I just smashed drugs and partied all the time, because I wanted to die but was too scared to do it directly.
I met a boy in first year just before the covid lockdown started. We got together and he actually brought up to me he thinks I could be autistic. I’d already thought I had ADHD and had been to the GP about my symptoms but it was misdiagnosed as depression and anxiety.
Fast forward around 3 years and I get diagnosed with ADHD and start meds. The ADHD assessor said he thought I was autistic and that I might notice more ASD symptoms when I start the ADHD meds which is what Happened.
I’m still with the boy (man now lol) that I got with all those years ago and he has massively helped my MH. He has saved my life before and genuinely just made me happier and I am so grateful for him.
I recently (a month ago ish) got diagnosed with ASD. I got ADHD and autism diagnoses through right to choose. They heavily recommended counselling but it’s to expensive.
I’ve been burnt out at work for months. I work in a stressful role and have been doing the work of 3 people for months due to long term sickness within the team. I’m extremely bad at asking for help irl because of my childhood so if I do it’s bad.
I asked my manager for a 1:1 because of my MH which was postponed twice. I then messaged him and said I need to talk to you in so burnt out and my MH is bad. He organised a meeting with him and his manager.
His manager postponed it again and when it finally happened he rocked up 15 mins late, told me autism is a superpower and than I should take the fact I get more work than everyone else a compliment because it means he thinks I’m doing my job well. Which was a tone deaf response in my opinion.
I went to the GP who just said what do you want me to do how can I help, then he told me off for being emotional. I’m emotional because I’m downing and I’m trying to do my bit and reach out for help and no one cares or is helping. He hadn’t read my notes, didn’t know about my previous MH treatment, didn’t signpost me or give me crisis resources.
Idk why I’m writing this or what to do I just know I can’t keep living like this. Majorly welcome any advice, or anyone who’s been where I am because it feels so isolating.
