Diagnoses

I have a dx of Pmdd and combined adhd but I don’t feel like it encompasses all of my issues.
I’ll list here what I struggle with most:

Poor emotional regulation and intermittent rage
Overall I’m “high masking” and blend into society well. I’m a healthcare provider and don’t have these issues at work bc of the masking.
Inability to “bounce back” after getting angry or upset. Sometimes it takes days to feel better
Need to isolate when feeling overwhelmed
Distrust of others and their intentions.
Paranoid I’m always doing something wrong and am going to “get caught” even though I’ve never been charged with a crime lol
Easily irritated and overstimulated and wear ear plugs a lot
Easily threatened around figures of authority or with interpersonal conflict leads to breakdown and need several days to recover
I don’t have conflict with people other than my husband
Avoidant in relationships and Extreme rejection sensitivity
Waves of passive SI
Many Sensory issues. Constantly needing to adjust my clothing or hair, reposition, can only wear cotton clothing, the feeling and sound of cardboard or microfiber makes me nauseous. Being outside is usually a bad experience bc of bugs and wind
I do not have routines. Every day needs to be different.
I feel bored and empty a lot and wish I could stick with a hobby or interest. My interests are typically very intense but only last a short period of time and then I go back to feeling empty again.
Spending money to feel better. Not reckless though

I was taking only Zoloft for about 5 years for depression/anxiety and didn’t feel like it helped that much.
Wellbutrin was added and made me very anxious
Lamotrigine was then added and made me feel much better for a few months
Adderall IR 5 mg was added and it helped my emotional regulation a lot and gave me more energy and thus less irritability.
Then we decided to switch Zoloft to Prozac. I had a very activating response and was in a state of panic after increasing to 20 mg.
Now I’m taking Lexapro 10 mg with the lamotrigine and still don’t feel good. Feeling very sensitive and drained and also don’t give a shit about anything I usually care about.
I feel so confused. Idk what is normal anymore and what to think. 2 therapists have mentioned quiet BPD but no diagnosis. I’ve also thought maybe autism but I’m not sold on either. Any thoughts?

Worried about SA

Hi. Idk if this is the right community but I’ll give it a shot. For much of my life I have been paranoid about SA. I do not remember being SA’d as a kid but had felt my privacy violated by my father several times. Ex: Wanting to watch me get in the shower as an older kid and giving shirtless back rubs.
I remember being ~5 years old and asking the boy next door around the same age show me his privates. The memory makes me feel awful.
I have a 4 y/o daughter now and am terrified of her being SA’d. I think about it a lot. I worry when I’m bathing her that she will think I’m being inappropriate even though it is strictly washing. I worry about her at daycare with the adults that watch her and other family members when I am not around. The paranoia gets worse as she gets older. It makes me wonder if it happened to me and I just don’t remember.

Prophys: subgingival scaling

My understanding was always that during a prophy you are getting everything off even if they have 4 mm pockets and mild inflammation. Or if they’re a stable perio patient that is coded as a prophy, I’m going to get to the base of all the pockets and scale everything. That’s how I was taught in school and for the boards that all deposits have to be removed. Obviously if there is more severe inflammation and perio they need SRP.

I’m now working at a private practice where the doc and other rdhs consistently mention prophys being above the gumline. I don’t understand how that works because if you only scale above their gums at their prophys, eventually you’re going to induce gum disease…and then they’ll need SRP.

I’ve already seen a few patients who had been going there on a routine basis for many years, and needed FMD and 4 quads SRP.

I’ve tried looking up the actual definition of a prophy but can’t find anything from a reputable source. But my understanding is that it’s for gingivitis and healthy patients and we’re going thoroughly under their gums to get all deposits off, to prevent disease.

Can anyone shed light on this?

Hygienist responsibilities

Is it common for offices to make the hygienists do their own scheduling and treatment planning? I recently started at a new office where I’m expected to schedule all my hygiene appts, put all hygiene and dental tx plans in and present it in a written list to the receptionist. I’m also expected to post the charges, clock how long patients are in the room and do all referral forms. This is on top of doing all my own sterilization and cleaning- no assistant help. Is this normal ? I come from an office where I never had to do any of those things.

Skeptical after starting at a new office today

I left my job of 5 years to work here, mainly because it is 1/3 of the commute but for other various reasons too. I just started today and am feeling completely frazzled. Of course there was expected stress of being in an unfamiliar office with new protocols etc, BUT the instruments are so bad. They’re all shared between 4 hygienists. Theyre all worn down way past their life and have no functional cutting edge. Most of their basic setups are a 6/7 scaler, a random gracey, 1/2 curette and a bent explorer and plastic probes. Needless to say they’re dull, cannot remove calc whatsoever. No posterior scalers or curettes, literally nothing to access for molars or perio patients. The only patients who had decent cleanings today were bc I used my own personal instruments. I just am baffled that the other hygienists see no issue with the state of their instruments and feel like they’re doing a good job that way. There’s just no way for the cavitron to get everything…

Anyway, I’ve already had to ask for a hands free suction system before I even started and I don’t want to be this greedy new hire. I don’t think there’s anyway to ethically practice with these instruments.. any tips in going about this without seeming like a complete a-hole?

4 y/o crossbite

My 4 year old daughter has a unilateral posterior crossbite. It originally wasn’t so bad but now it’s at the point where there is complete crossover up to the lateral incisor. She complains of biting her cheek a lot on the affected side and there is a lot of scar tissue there. I believe it’s also causing her to grind her teeth in her sleep. From what I’ve read most of the time the crossbite resolves itself during development into the adult dentition. Is there anything I should do right now though? I feel bad that she is always biting herself and that it seems to keep getting worse.

Resigned from my hygiene job today

I gave my dds notice and am now feeling a wave of guilt. For context It’s a very small private practice with 1 dentist and 2 other hygienists. I’ve been working there for 5 years. Overall it’s been a good experience but I am limited by the doctor. We aren’t allowed to do sealants, fluoride varnish, or whitening. We’re strictly prophys and srps. We aren’t allowed to educate about certain topics.
The office is adequately staffed and can definitely get by with 2 hygienists, but obviously it’s a burden for him to either find someone new or reschedule everyone.
There is a moderate level of micromanagement and moodiness from the dds. I love my coworkers but the OM is unbearable(in ways that would warrant a whole separate post).
When I told him I was leaving today he tried to negotiate and convince me to stay. He asked why I didn’t come to him before looking for a new job. He also definitely tried to make me feel guilty over some things, like it’s not fair to the patients, and that he had made xyz accommodations for me in the past, etc.
I do feel sad to leave, it’s not easy to change jobs, but he made it worse by not just accepting my resignation. As a dentist is this how you’d react? What do you think?

I gave my boss notice today and am now feeling a wave of guilt. It’s a private practice with 1 dentist and 2 other hygienists, I’ve been working there for 5 years.
