Apparently my NPI and full name has been found on dark web-what to do

I use Norton for security on my work laptop. Today I received a notification stating my NPI, full name, city/state (which is the specific city where the group practice I am a 1099 at) has been found on the dark web from an Unknown Source in mid August 2026. Since I’m 1099 and the group practice does all billing/credentialing/that admin side of things, I don’t believe I am able to check to see if any unauthorized billing has occurred under my NPI (and I wouldn’t even know how to go about or where to look to find this information). Has anyone experienced this and/or what do I do?

Suspected periorbital hyperpigmentation and/or melasma-no clear answer from dermatologist-looking for possible correct diagnosis/differential diagnosis and potential treatment options

So I’ve always had some darkening under and around my eyes (up to eyebrows) since adolescent/teen (regardless whether I get plenty or less sleep, which I think may be genetic or hereditary since a few extended family members have it which I’ve just come to accept after years of this and figure no solution for).

However, as I’ve gotten older (I’m in my mid 30s) and particularly during and after being pregnant (I’m 15 months post partum/since March 2025), the hyperpigmentation/brown outlines around my eyes/cheekbones started and worsened, but I also started to develop what I suspect/think this hyperpigmentation/discoloration also involving the ‘mask of melasma’ during pregnancy and has continued post-partum (it’s a little better post birth, assuming the hormones).
However, I still am dealing with these awful ‘outlines’ that are light brown and almost look freckle like, but they are same shape and start around cheek bones to near temple and they are impossible to cover up with make up and I’m at my wits end.

I first started using as transexamic acid mid/late-summer 2025ish and seemed to lighten a bit (compared to pregnancy). I’ve been using a different product now for a few months with a higher percentage of transexamic acid, and contains niacinamide, and arbutin as well and doesn’t seem to make any difference.

Would love/welcome some feedback—any other differential diagnoses/thoughts, product recommendations, etc.

I’ve also attached a few photos for reference, I hope the lighting is okay and I know it’s a little challenging with cropping to get full extent, but I’m happy to add some others if needed. Some days the brown outlines on cheek bones to end of eyebrows are worse than other days, not sure why.

I also wear sunscreen and am not in sun frequently. I also realize I have broken blood vessels on side of my nose and assume laser treatment is my only way to remove….

Treatment plan writing-advice needed

For those who use SimplePractice —do you use the Wiley treatment planner option or a blank box to copy/paste everything? I’ve been using the Wiley treatment planner option bc it links the goals and objectives to the actual progress note where you can click whichever was addressed in the session and you can rate progress with a description…but this Wiley format does not allow for sections to be added like medical necessity, conditions for discharge, prognosis, etc….

Of course I can do this in the simple box treatment plan option, but I find it harder to track progress for each goal
And objectives without going thru each individual progress note especially for regular treatment plan reviews (I accept commercial insurance and state Medicaid). I also tend to work with complex presentations with multiple diagnoses (anxiety, depression) and heavily trauma issues/diagnoses so these are not simple cases and I am in private practice.

Any thoughts/recommendations/feedback much recommended!

Supplements to stack for adhd and anxiety symptoms

I am prescribed Ritalin for ADHD symptoms (switched from long-term Adderall) and also Xanax very low dose prn as needed. I have noticed that my ADHD medications tend to illicit somewhat OCD type tendencies/thinking, particularly surrounding perfectionistic thinking patterns (which then I often am in time blind mode and takes me double to triple the amount of time to
Get things done compared to average person) and feeling overwhelmed with everything I need to
Get done and also having difficulties getting started due to where to prioritize the work/paperwork I have to do in my job (which is substantially less severe) since switching to Ritalin from adderall.

I have tried l-tyrosine and l-theanine without any effects.

Within the past six months, I was taking ALCAR and think some positive efforts on energy (who knows though with various other factors) and have since run out. I started NAC on days taking Ritalin which seemed to has lessened the perfectionist/ocd tendencies in thinking, but still not efficient in time management for work tasks/paperwork. I was also taking omega 3s and plan to reorder soon.
With that being said, any recommendations on another supplement I may benefit from adding? I am also a vegetarian so I curious about b-12 supplement (I also take multivitamin)….i also see posts about alpha gpc, among others and didn’t know if any others had recommendations.

I’ve tried several different types of l-theanine powder with caffeine for a more smooth focus and manage anxiety when doing paperwork. No effects. Any other supplements recommended?
