Should I repair the hernia or have my hysterectomy first?

Hi all,

I was recently diagnosed with a hiatal hernia. During the winter months (I have cold induced asthma) I lose about half of what I eat. In the warmer months, I throw up about 1/2 cup to a cup of phlegm. I can’t drink too much liquid too fast either (used to be able to) and my food has a hard time settling in my stomach. I’m also realizing that the burning sensation I get is where the hernia is.

I also need a hysterectomy by the end of the year/beginning of next year due to PMDD. I’m on medication that induces menopause so I don’t have actively have any symptoms that the PMDD causes.

My hesitation with repairing the hernia first is honestly the menopause meds themself. They need to be taken every day at the same time just like birth control. I’m concerned between the hospital and recovery, I’d have a hard time with that and then PMDD would hit my full force.

Any tips or opinions, even just on the hiatal repair itself, are welcome.

Getting my ovaries removed, what to do with the uterus?

I have severe PMDD and am currently going through chemical menopause, but the end goal is to remove the ovaries. My doctor said we can discuss what to do with the uterus, but I was hoping to get some feedback from y’all about it.

When I try googling, I just get results for what to do with the ovaries when removing the uterus.

I do know that if I kept it, I’d need to take progesterone to prevent cancer. Are there any other pros/cons I’m unaware of to keeping/getting rid of?

Update to: Has anybody gotten Myfembree approved by insurance for PMDD?

My insurance rejected the prior authorization and reconsideration from my doctor’s office. My denial letter said I could send in an appeal with why I disagreed with the decision and could include medical records.

Sooooo I got 2.5 years of medical records from 7 providers, had letters from 2 therapists explaining why/how my pmdd is detrimental to my quality of life and included 2 pages of quotes (with curations) from peer reviewed, scholarly articles on why GnRH treatments were effective for PMDD. The on label usage for Myfembree is endo and fibroids, neither of which I have.

My appeal ended up being about 150+ pages. I mailed it certified, originally, and they lost it. I spent about 4-5 hours on the phone with supervisors last week So I had my doctor’s office fax it Friday morning and I had an answer this morning. IT WAS APPROVED.

I also filed a complaint with my states insurance commissioner for them losing my sensitive, medical information. A big fuck you to them for even making me work this hard for life saving medication!

I think I might need to declare bankruptcy

I’ve dug myself into a big hole of debt. I’m working with a non-profit for debt consolidation but switched jobs shortly after and between the paycut and my part time job not bringing in enough income, I’m short every month. All year I’ve gotten lucky with things like a tax return or hidden money found, but that well has dried up.

My only hope is someone co-signing for a 50k personal loan so I can pay down enough debt to make my budget work every month. I don’t qualify for anything above a 3-5k loan with a 30% interest rate with my current credit. I don’t have late payments, my cards are just maxed out.

I’ve addressed, and continue to address, the issues that got me to this point, but I don’t know how to get out. I can’t bank (no pun intended) on someone crazy enough to cosign a loan that large, and I can’t get it on my own. My debts are credit cards and my car, which has about 8k left owed on it.

I used to be really good with my money and my credit, I had an 800+ credit score in my mid 20s that I was proud of. I just don’t know how to get out of the bed I so nicely made for myself.

Has anybody gotten Myfembree approved by insurance for PMDD?

My doctor and I agree that medical menopause is the next step for me, but I’m having a hard time getting insurance to cover it.

I don’t want to do Lupron because I’ve read too many horror stories about it wearing off early. I like the idea of a pill and compared to Orlilissa, Myfembree has the add back hormones with it. The on label use for it uterine fibroids or endometriosis.

My insurance denied it twice so I’m writing my appeal right now. I’m going to try to get some medical records just showing how severe my pmdd is, as well.

I WAS APPROVED FOR MENOPAUSE!!!

I’m the happiest I can remember in months!!

I was very nervous to start because when I told the nurse I wanted to talk about it (she thought I was there for irregular periods), she had never heard of it and was very confused. It gave me very low hopes.

I’ve always said that I couldn’t wait for menopause because (most) people stop experiencing their PMDD symptoms. Well I don’t have to wait anymore!!!

I went in with the following, writing it down really helped because (first, I was crying) I didn’t have to think/try to remember:

4 failed anti depressants

2 failed birth controls

4 current anti depressants/anti psychotics

Therapy 1-2x a week (CBT&EMDR) for over a year

Seeing a psychiatrist for over a year

I also had letters from all 3 providers just in case just stating how long I’ve been seeing them, so if there was any question on how long I’ve been trying, I had something to back it up.

Even thought I’ve only been seeing the psychiatrist for over a year, I’ve been playing with meds for 2.5 years. I told the gyno that I found what I thought was the perfect combo in October, but then I had some personal things happen and I’d been spiraling ever since.

I think it also helps that I need to have a lowered dose of a current medication because of a tremor and elevated heart rate (like 120bmp) but things got very… dark when I tried.

She suggested Lupron but I’ve read a lot of stories of it wearing off before it was time for the next shot and symptoms coming back full force. She was receptive to this and welcome to the idea of taking a pill for it. We landed on Relugolix because it also has the add back of estrogen and progesterone.

She said I can be on it for 1 year if it works then we need to discuss surgery. She did ask about fertility/children but the way she asked was like making sure she asked so she could check the box she did. It was completely non-judgmental and felt like it came from a caring place.

God, I’m just so excited!!
