The competition between us is killing our field

Every day i see hundreds of therapists on facebook and in listservs just dogpiling on every referral request, just out of breath spamming their awful chat GPT posters and saying IMMEDIATELY AVAILABLE! Yeah, we get it. We all offer SPECIALIZED COMPASSIONATE THERAPY!What other healthcare profession has tens of thousands of people doing that all day every day everywhere you look on every platform? Its really discouraging and honestly im feeling at a loss because im in that same boat despite spending thousands on advertising, SEO, and building relationships in the community.

What is with the paranoia about billing 90837

Seeing clinicians worrying about billing 90837 is new to me; ive only ever billed this for my thousands of sessions over several years at 4-5 different practices and have never once had an issue and its never come up in any professional discussion, until i joined a practice that makes all of their sessions 45min by default “to avoid problems”. Is this some kind of a brand new problem im not aware of?

Backdating claims

Hi. I saw a client a week ago out of network, but was informed today that i am now in network and can bill insurance. However, the effective date was a day or two before this appt. I havent charged the client for that visit. Would i actually submit a claim for that visit now since it was technically after the effective date even though they didnt inform me of that until after?

Are you also not showing up in your 2nd or 3rd zip codes on psychologytoday?

I know most of us hate psychology today including myself. After doing some digging around on the site, i realized my profile literally…i mean literally…does not appear at all whatsoever on any page for any zip code other than my auto-listed one where I live. Filter by my demographics and specialty and im at least somewhere for my home zip code, but not one ANY page for the other two.

I cant be the only one noticing this and i suspect this is why so many of us are getting no inquiries.

35m - ablation coming up

I am a fairly healthy 35 year old guy with no health problems other than paroxysmal afib and also i could lose about 40lbs. Between 2016 and 2026 ive had 4 day-long episodes, two required cardioversion, one stopped after a day when i took some “as needed” meds, and my most recent episode in january spontaneously stopped after about 12 hours and a little cardizem. The four episodes were 2016, 2019, 2021, and 2026.

When i have the afib i am so symptomatic i cant even exist. My heart rate is so high and its hard to breathe. Cant even think straight.

I have no underlying heart disease. I exercise regularly, although the episodes i have had have destroyed my confidence in myself and my body. Ive been terrified of dropping dead, having a stroke, having an episode while on a plane, out for a walk, etc, so ive been very mentally unwell over this.

My electrophysiologist is going to do a PFA on me in a couple weeks. Im really nervous, and im wondering, based on the info i shared, how successful this “should” be in preventing another episode for a really long time.

Simple practice insurance billing

Im going to be doing insurance claims and copay collection for by myself for the first time ever this week. Ive just been private pay until now and of course simple practice billing for self pay couldnt be any easier.

For this particular case, there is just a copay for the visit.

Im wondering if anyone could please tell me how i charge the clients card for the copay, and then submit the claim. I only understand how to charge clients who have their profile set up for self pay. Is there a way to charge their card directly just for the copay amount?

Also wondering, for clients who have a deductible that hasnt been met, do i charge the copay at time of visit like usual and then bill or charge again after I get the EOB and see the amount they owe?

Anthem/Carelon question

Hi there,

I am a provider who lives in a state where I have to go through Carelon (via Availity) in order to become in network with my local Anthem BCBS.

I am curious, is Carelon out of the picture once I am credentialed? For example, am I just submitting claims to Anthem and dealing with Anthem directly once im approved to see members? Or am i submitting claims through the Carelon payer space. I am so confused.

Im a psychologist in an affluent geographic area and the initial contract rates offered to me were not much more than medicaid was offering like ten years ago. Is this for real? Please tell me cigna sucks and im not imagining this lol

For those of you in your own practice 5-10 years out, how has it been continuing to be able to get referrals? Do you hit a peak, get lazy with networking, and see a decline? Once the ball starts rolling, does it just stay rolling? Does it take constant networking and hustling to get referrals all the time year after year?

W9 health provider question

I am a healthcare provider with a single member LLC (disregarded). I have to fill out a W9 for each insurance company i contract with cause they pay me.

Im contracting as a “group” to keep my own practice separate from me (from a legal perspective) as nearly every provider in my field has instructed me to do, because i want to bill and get paid under my LLCs tax ID, not my SSN. This also protects my ssn from circulating to countless patients and whoever else.

Because i want to bill and get paid under my practice as an entity, insurances will not allow me to designate my business as the payee without having the business EIN on the W9. Plain and simple.

Between my own CPA, CPAs ive talked to on here, read articles online, talked to chat gpt, i get all mixed info.

Theres no question the instructions say to NOT use the LLCs EIN, but some are saying its fine, or they do it thenselves, or “its not a big deal”.

I literally am pulling my hair out, and i just want to know whether or not im gonna run into trouble when i get 1099s with my disregarded LLC EIN while im supposed to be filing a 1040 under my SSN.

If this is “technically wrong based on instructions, but wont cause issues”, i just really need to understand why from an insider perspective.

Ive been told by virtually every person, every provider, every customer service rep at the insurance companies, to contract with them as a “group” as my business has a “group” NPI and a EIN which i want to bill under.

However, Insurance companies will not allow you to panel with them as a “group” with your business EIN unless your W9 has the business EIN.

Even though the instructions say to put my own SSN, my own CPA said just put the business EIN. Then i read online and see so much conflicting information. I went with my CPAs advice, and am still in the contracting phase so idk what will happen down the line.

Is the consensus that this is “fine”? Am i going to run into any big problems?

W-9 help

Hi,

Im a provider trying to credential with insurance. My practice is a single member llc with its own TIN and npi2. Im filling out applications enrolling as a new group as nearly every person has instructed me to do. On my w-9, per irs instructions, i am putting my ssn for the TIN because for disregarded entities like mine they instruct to put the owners ssn. On the applications i have my ssn where it asks for that, and i have my practice’s EIN where it asks for that.

I received an email from one insurance company saying theres a mismatch between the application and w9. I explained the w9 instructions and the person said ok thanks for clarifying. Now a second insurance company emailed me saying they need the w9 and application to be consistent.

Im not sure who is right here. I am pretty much positive i was supposed to enroll as a group, and also that i am supposed to have my own ssn for the tin on the w9.

Any insight?
