People with N24 experience daily shifts in the circadian rhythm such as peak time of alertness, body temperature minimum, metabolism and hormone secretion. These shifts do not align with the natural light–dark cycle. N24 sleep–wake disorder causes a person's sleep–wake cycle to move around the clock every day, to a degree dependent on the length of the cycle. This is known as free-running sleep.
From Frank Zappa's autobiography The Real Frank Zappa Book:
"My schedule sort of twirls around the clock. I can't stay on nights all the time because every night I work an extra hour or so, editing, or recording, or on the Synclavier, or, presently, on this fucking book -- pushing it a little later each night -- but then, once I go to sleep, I want to grab eight or ten hours, and so my "day" keeps changing around."
So he probably had a 25 hours sleep-wake cycle. The man lived with N24 and didn't even know it because the diagnosis didn't officially exist yet.

Meaning?
CBT-I isn't merely ineffective. It is dangerous, because they get people killed by advising them to perform tasks like driving while sleep-deprived.
Unlike for instance zolpidem, CBT-I has never been tested in a randomized controlled trial.
