Patient came in with a large abscess on the lingual gingiva of #30. I drained the abscess, rinsed it with saline and placed the patient on abx. How possible is it that this virgin tooth is cracked or what’s the source of infection on a seemingly perfect tooth. Lingual pd is 11mm traced with gp the distal of 30

More patient stories: Patient stole my bone graft

Patient came in for limited exam and said he could not get in with any other dentist for 3 weeks and had pain on ur. #2 was decayed non restorable and indicated for extraction. Front desk collected payment for extraction. I get done extracting the tooth and patient asks if I’ll do the bone graft today as he plans to get an implant. I tell him it’ll cost the same as he paid for the extraction if he’s fine with paying another 413 I’ll do it no problem. He’s an older guy so I figure he’s good for it. I go into the next procedure and tell the front desk to settle up with him before he leaves. He gets to the front desk and says he thought the graft was free and they let him walk. I get to the front and tell them I told him about the fees and he accepted and to call him back and get payment. So now tbd.

Weird patients

I started a new job and it’s filled with weird patients. This is just a story from the past 2 weeks. Patient comes in and says she has lupus and wants me to take a look because “she’s noticed some bumps on her gums at the top” upon examining it’s hard like buccal exostosis. I ask her which type of lupus she says idk. She also has a long span pfm bridge, the ceramic has fractured off as she said she was intubated and she has like 5 implants under the gingiva with cover screws. She also has splinted crowns on 5 & 7. I told her orally it looks fine, as far as the bridge and splinted crowns it’s all been done and to undo it she’d probably have to go see prosth and she needs to see a rheumatologist and she needs hygiene treatment as she mentioned she’s not able to clean under the bridge. I also ask about the implants and she kinda shrugs it off and says the dentist is too far. 3 days later she comes in and the splinted crown is broken 5 is in the crown 7 is re cementable so I tell her 5 has to be ext and 7 I’ll cut the other crown off and recement. She then goes on to tell me she thinks she has internal bleeding so I tell her it’s probably best to go to the er because I’m not trained to manage that. She walks in today and says I’m cold and I should treat people with autoimmune disease better and have some compassion bc no dentist has ever acted like me. In the back of my mind I’m thinking what about the dentist who did all this work that’s failing and the dentist you ran off on implant crown completion.

But to me I treat all patients the same. I manage treatments I can and refer what I cannot manage. Am I wrong?

More patient stories

Patient was set to do a bridge prep on 18 and 20 for a 3 unit bridge. I do #20 first and start 18 do the occlusal reduction buccal lingual and start axial reduction and he says he wants to stop and be sedated due to his gag reflex. The place where I work doesn’t offer sedation for restorative and honestly I don’t know anyone who does. I let the patient know and they say they’ll find someone I just temporize and say okay. How exactly should this work?

Honestly situations like this make me want to pick another profession.

2 bathrooms in a 600 sqft 2br apt or 1 bathroom

I bought a duplex in 2019 and re did it to this floor plan with 2br/2ba. I’m buying a similar duplex but I was leaning toward 2br/1ba. The current tenants like that they have their own bathrooms. But what’s the consensus?

Patient came 1pm on a half day and wouldn’t leave until I extracted this. Even started crying. Almost questioning dentistry after patients like this. Share some good stories. I can use some laughs to get me through.

Board complaint

So today a board complaint was sent to the office I work at. The patient had an emergency appointment and wanted a crown recemented. The da took an x ray without the crown on and one with the crown on. Patient sat in the op for about 20 mins I came in and he says he had to wait 20 mins. I explain to him that the schedule was overbooked and the office was understaffed. It’s unfortunate but that’s just how things were that day. He’s still going on and on about waiting 20 mins. At this point the crown is on his tooth. I look at the x ray and there’s a large lucency as the tooth has a large post and I try to get the crown off gently with a spoon but it’s not coming off. I tell him please try to remove the crown because I don’t want to push too hard and break the tooth getting it off, as these things sometimes happen. He gets it off and I explain to him that tooth is in bad shape he insists he still wants to recement the crown. I tell him ok, it’ll be $132. He says “fuck no I already paid $77, give me my fucking crown”. I tell him $77 is the exam fee $132 is the recement fee. He’s still cursing so I give him the crown and tell him to leave. I let the front desk know that’s he’s no longer allowed to be a patient here and they can transfer his records anywhere he wants. He keeps cursing and yelling so I ask him to leave again until he finally does. The complaint says I physically threatened him. What to do now?

Also I found out he went to an office an hour away and paid 400 to get the tooth extracted.

How often does this happen to you?

Appt started off well. Removed decay pre endo build up, RD, access, instrument and cone shot. Everything looking good. Obturate and build up and palatal is well short of apex. Had to apply heat dig it out and re obturate. How often does that happen to you all? Glad I re did it but hate that this happened. Massive waste of time.

What to do

Graduated in 2023. Long story short I wasn’t sold on dentistry at that point for different reasons. Anyway I completed gpr in 2024 and gained alot of confidence there and I’ve been working 6 days ever since. Decided I wanted to apply endo took the adat and got letters from the dean and head of my gpr but the 2 endo faculty from my school won’t write me letters, one replied and said I didn’t work with them much in clinic the other didn’t reply at all. So what to do?

I 29M broke up with my girlfriend 29F over wasting my time

I 29 M have been dating 29 F for about 7 months, since November 2025. Sometimes she goes out with her friends and I don’t mind dropping them off especially if I have something else I have to do in the city. Anyway yesterday she said they’d be ready at 810pm, so I got there at 810pm and long story short they weren’t actually ready to leave until 945pm. She knows I’m taking an exam next month to try to get back into school and my time is tight these days. During this time her friend keeps commenting about “I’m no fun”. I don’t really like confrontation esp with women so I just waited and dropped them off. Called her this morning and ended it.

UPDATE: this is probably the third time.
First time was on her birthday. I planned on her being late so I made multiple dinner reservations

Second time. Her and her friend had a birthday dinner they were maybe 45 mins late

TLDR: broke up with my girlfriend for wasting my time

I feel good about my decision, but just wanted to see what others think?

Feeling a lot of guilt

I’m 29M Been dating a girl 29F for about 7 months now and I’ve recently realized she has so much dysfunction partially caused by her upbringing and partially caused by herself. For example she told me her parents had to move often due to finances/instability as a result she’s a hoarder, I go to her place and things are in disarray, clothes stacked in the bathroom half drank bottles and cans everywhere. She stays at my place sometimes and I’ll leave for work and ask her to make the bed if she gets out of it after me and she says she never was taught to make a bed. I ask about 401k she says she was never taught to save money. I ask about dieting she says her parents got her fast food every meal and it made her sister obese (she ran track so she’s fit). She talks loud which to me is yelling but to her she says that’s how her family talks. I don’t want to be an ass and drop her because we weren’t raised the same but idk how much I can take. Advice please?

Build up

What do you all use for build up? Office just got fluorocore blue. I like it bc I can clearly see what’s build up and what’s margin. But I’m not sure yet of bonding strength yet. Ik #18 has a failing resto pt wants to have it fixed later.

Why do people do this shit?

I work Saturdays at a Medicaid facility. Patient was last here 11/29/24 and went somewhere else and comes back with splinted crowns on 4 & 5 with root decay on 5. What’s the rationale on doing something like this?

Obturation long or right on?

Recently I’ve had a lot of patients come in with pain and I always do crown down to prevent post op sensitivity and make sure to follow my apex locator. I see the outline of the palatal root but I just want a second opinion

Weird filling under crown

80yo female patient said she was worried about a spot under her crown but wanted to save the tooth. Half way in I tell her root canal filling has been exposed to saliva for a long time and it may need to be redone or tooth extracted. What else to do?

Sensitivity

Patient had a primary tooth that had to be extracted. She chose to do bridge instead of implant. When I took the temp bridge off she immediately had sensitivity. She described it like biting into a cold ice cream cone but only after I took off the temp bridge. When I took it off she had some inflammation and food stuck under the bridge. Cemented the final bridge with temp bond just to see how it goes for her after 1 week. I told her sensitivity is probably caused because the enamel removed or the gingival inflammation Reason for sensitivity?
