How does an online psychiatric consultation work?

I am in Kolkata. I want to get assessed online by a psychiatrist who is up-to-date with adult ADHD.

  1. How much time do I get to explain my problems?

  2. How much is the average online fee?

  3. How many sessions until a diagnosis is done?

  4. How do I share my documents (previous prescriptions and other reports) with them?

  5. Can I get assessed by an English-speaking psychiatrist from another state?

  6. Are they cool with a patient who reads their problems from a document?

  7. How will I get the prescription (through gmail, whatsapp, etc.)?

  8. Will they prescribe medicines online?

  9. Do those prescriptions work if I take them to a local pharmacy?

  10. Will they tell me when to get a next appointment with them?

  11. Can you suggest the best psychiatrists for online consultation, if possible Bengali ones?

Can discontinuing 200 mg Sertraline cause a seizure?

I basically have a seizure disorder. The first and the last time I had a seizure was the day after my Fluvoxamine 200 mg + Clomipramine 150 mg was changed to Fluvoxamine 300 mg + Clomipramine 150 mg. My neurologist told me that the antipsychotic I was taking with these medicines was not the reason for my seizure.

ADHD x Misophonia?

During COVID lockdown, for 2 years non-stop for every 5 seconds, I had to hear my mom sitting near me, pulling her nose. Since then, I have developed some kind of trauma towards sounds like chewing, nose-pulling, my mom clapping her hand on her thigh while on call with somebody, etc. basically frequently occuring sounds that have no guarantee of stopping anytime soon. Maybe its my hatred towards my mom and dad that makes it even worse. Or, maybe its the frequency of the sounds they make, or the loudness of them that makes it worse.

The worst part of it all is that I possibly have ADHD, which gives me severe forgetfulness. This is a forgetfulness of a level where I can't remember the previous phrase of a sentence while reading a given phrase of that sentence. Whenever the sound occurs, my attention gets diverted, and I can't remember what I was specifically on. To remember it, I have to give additional mental effort. Then boom, the sound occurs again. Basically, it keeps occuring so frequently that I can't end up remembering what I was on right before this second, and trying to remember it everytime, so frequently for the whole duration ( maybe a few hours, maybe a day, maybe a few days ), costs me so much mental resources that it gives me a headache.

Anger or frustration is bound to multiply under these circumstances.

For my friends located in INDIA, if you seek an ADHD diagnosis, get an ADHD assessment done by an RCI-registered clinical psychologist

Call up a clinic, ask for an ADHD assessment. It will take 3k-4k. Be prepared with your main symptoms, research about the common symptoms of ADHD, note down symptoms from other kind of problems such as anxiety, OCD, depression, etc. and then go. They will hear everything, and everything needs to be heard to be judged correctly. In the first interview, they will hear, in the second, they will assess you. There will be questions that rule out other causes of your symptoms. They will ask for an additional interview with your parents or with someone who knows you since childhood, be prepared for that. Remember, to get the correct diagnosis, you must give them every bit of information you have. After the results come, your mind will have clarity that you don't have ADHD, or you will have a permanent diagnostic report to carry to any psychiatrist for medicines or to any therapist for therapy/advice.

Personally, I am from Kolkata, so I went to Ishita Das from Carring Minds International but I got an incomplete diagnosis because I didn't research well enough about my symptoms and didn't give them enough information. I am seeking another assessment soon. Good luck.

If you suspect you have ADHD, get an ADHD assessment from an RCI-registered Clinical Psychologist

People, if you are wondering whether you might have ADHD, get an ADHD assessment done from a clinic of RCI-registered Clinical Psychologists. It will take 3k-4k if I remember correctly.

Once you have the result, you either will have clarity that you don't have ADHD, or you will have a permanent ADHD diagnosis with which you can go to a proper modern-minded psychiatrist.

Warning: Make some notes of your symptoms and do some research about ADHD and its characteristic symptoms before going to the assessment, because there might be symptoms you face yet are unfamiliar with them, that is, you have masked your symptoms so well that even you are unaware of them. Don't be prepared with just your ADHD symptoms. If you have symptoms such as those of anxiety, OCD, depression, etc., note them down and take them to the interview, because they will affect the end-result of your assessment. Back to ADHD, questions about many kind of symptoms will be asked in the assessment, so be prepared. And, take your parents or someone who knows you since childhood with you to the collateral assessment, because they will ask questions about your childhood to those people. Don't worry, the psychologist will make it clear to you when to bring those people. In the first interview, you will tell them your main symptoms, they will ask about other kinds of problems you face, as well. In the next interview, the assessment will be done, both yours and the collateral assessment if it doesn't need one more interview. Don't worry, there will be questions which will help the psychologist clear out other causes of your ADHD symptoms.

There are people here who are debating whether going from psychiatrist to psychiatrist just to get diagnosed is right or wrong. Let me put it this way, going from a psychiatrist who still has the old "hyperactive child" impression of ADHD and who dismisses all your ADHD concerns to someone with a modern impression of the disorder and someone who listens to your concerns is the right thing. However, if multiple modern-minded psychiatrists have dismissed your concerns of ADHD even after listening to your symptoms, then instead of going to another one, just call up a clinic and ask for an ADHD assessment. After getting the results of the assessment, there will either be clarity that you don't have ADHD or there will be a permanent diagnosis of your ADHD that you can carry to any modern-minded psychiatrist, to get medicines, or to a therapist to get therapy/advice. Don't listen to people who are dismissive of your symptoms based on their experience or who tell you to trust the average psychiatrist regarding ADHD. Get a proper assessment done, that's the best thing you can do.

Personally, I am from Kolkata, so I went to Carring Minds International last year, to Ishita Das, to get an ADHD assessment done. However, I was very very unaware of my symptoms back then, so the result only came out to be "inattention". I will get the test redone soon, with all my symptoms including those of other disorders. So, there will be a proper diagnosis this time, if not of ADHD, then of something that I really have which is causing my ADHD-type symptoms. If anyone is from Kolkata, they can also call up Carring Minds International and seek for the same clinical psychologist I mentioned. She is really nice.

"If you had ADHD, you wouldn't have been able to get into an engineering college" - THE WHOLE STORY

You guys might have seen my few posts here. This is the same person who talked about the doctor who said "If you had ADHD, you wouldn't have been able to get into an engineering college" and "If you had ADHD, you would have had trouble understanding stuff from 12-14 years of age".

So, basically, around 1 month ago, I went to this popular doctor here in India who has years of experience. I went with my parents, and with my history of psychiatric medications -> 1 year and a few months of the strongest and combined doses of SSRIs, and low dose anti-psychotics and anxiety-medications, which treat OCD, anxiety and depression. The treatment had mainly been for OCD because the previous doctor didn't believe I had ADHD. He had a reason though. I went through an IQ and ADHD test (by a clinical psychologist) on his suggestion, 1 year ago, and they found out I had incredible working memory, a low processing speed, anxiety (which they got from the first interview) and the ADHD test results showed that I only had inattention, because I was absolutely unaware of my own ADHD-type symptoms back then. I only started studying and recognising my ADHD-type symptoms since a friend mentioned that extreme forgetfulness is also a symptom of ADHD, which was a few months back.

During the appointment with this new doctor, while I was speaking out 30 different symptoms, including OCD and anxiety symptoms, the doctor was observing my clinical summary from 1 year ago. He saw "inattention" written, and so he wrote down inattention on the prescription. He somehow concluded I didn't have ADHD, I don't know how. He wrote down OCD and anxiety because he heard the symptoms of those conditions from me while I was speaking. Mind you, he also saw the history of medications of my treatment-resistant OCD. He asked my parents certain things like "Does he shave?", "How does he get angry?", and some more things, and started writing down again. Meanwhile, my dad mentioned that I study in an engineering college. He finished writing and handed me the prescription.

I asked him to explain, and he said its probably not ADHD because I study in an engineering college and I don't have trouble understanding stuff from 12-14 years of age. He just said "You have inattention". What I saw next was laughable. Bro increased the dosage of my OCD medication (to Sertraline 150 mg). He gave me a long-term anxiety medication (Etifoxine 100), and he gave me some medication called Amisulpride 100. I still don't know what that last one's for. I asked him, "Will this improve my working memory?", he said "Thats the mission" and then, he said that I should try doing yoga and pranayam, and then when I am all better, he will reduce the medication doses. Bro thinks depression's causing my symptoms even after I have been on SSRIs continuously for the last 12 months :)

Moreover, I have heard that SSRIs worsen forgetfulness, which I even mentioned to him as my main problem.

Oh yeah, I also asked him "What about hyperactivity? I have a few symptoms of that." and he replied "Few symptoms are not enough. People used to climb trees in the past, does that mean they were hyperactive?"

Do Clinical Psychologists read your notes during an ADHD diagnosis?

I am soon going to get an ADHD diagnosis done by a clinical psychologist. I have a number of notes that I want to take with me, which have my entire symptom history and not just of ADHD symptoms. Additionally, I have questionnaires too. And, mind you, they are not small notes, they are multi-page notes, all of them combining to be of 20+ pages. They are digital as well, if required.

The reason for this is my severe forgetfulness, which was worsened over the years. I will tell her about it, of course. Like, I cannot say anything on the spot because I can't remember anything. The problem has got to the point that I cannot remember the previous phrase of a sentence when reading a given phrase of that sentence. So, you can understand how bad the forgetfulness is. I also can't shorten down the notes because of my forgetfulness. I don't know whether she will read the notes or take them home to read them.

What is my best strategy here?

Does ERP/CBT work for Misophonia?

Hi. The misophoniasupport subreddit seemed inactive so I considered posting here. I have a reaction to CHEWING and NOSE-PULLING sounds, especially loud ones, even if I myself pull my nose a lot. It seems not many people around the world have misophonia, which makes me upset. I have heard that there is no medicine for this disorder. And, I do worry that noises like such at work might cause me trouble as much as they do in my personal life at home. So, just out of hopelessness, does going to a psychologist who specializes in ERP/CBT work even a little for this disorder?

My Psychiatrist told me I don't have ADHD because I had somehow got myself into an Engineering College

Like...wtf? 22 M. I have been waking up tired of my life for the last 2 months after I became all aware of my ADHD symptoms. I had finally found out the reason for my lifelong problems. I researched all about it for a month, and prepared documents and short notes to tell the psychiatrist. First of all, I didn't have much time to say anything in detail. Secondly, he stopped listening after hearing the first few words. He looked into my ADHD and IQ reports from one-year back when I had zero clue about my symptoms and saw this written -> "...features of inattention and anxiety...". He then started asking my parents about certain things and concluded that I do have inattention, anxiety and OCD (which I certainly do, diagnosed by an earlier psychiatrist) only and nothing else. My dad had mentioned that I was in an engineering college, and the psychiatrist told me that if I had ADHD, I would have had trouble understanding words and stuff from 12 years of age. The funny part is, I got myself into that engineering college because that college was taking in anyone, I needed no competitive exam to get into that. This psychiatrist has years of experience and some top-level degrees btw. And yeah, I am from India, if you hadn't guessed it already. I am now preparing to get myself tested again for ADHD by a clinical psychologist. This time, I am all prepared.

ChatGPT not remembering previous details...

So, basically, I have a long chat in ChatGPT about my million health symptoms but it doesn't quite seem to remember previous details. So, I can't ask it to make me a summarized version of that whole chat, because it will miss out important stuff. I can't copy my entire chat and paste it somewhere else because ChatGPT doesn't have a feature to do that. I can't feed my messages from that chat to another chat and ask it to summarize the entire thing because it would still be the same problem. I can't do the same thing with Gemini because it has the same issue. Moreover, both of their limits on chats, chats with attachments, file uploads and the size of file uploads also pose a problem. Is there a workaround?
