What is more attainable for visa requiring non-us IMGs with less research opportunities, FM or IM?

Just started medical school. They're teaching the BRS books (brs physiology etc). I intend to buy UWorld and start doing questions but not sure what the appropriate time to do that is because I don't have any of the base knowledge yet. I have USMLE Step 1 first aid book.

Yesterday: "I just noticed the pinkish spot on the skin around my upper vulva today. Pinkish spot with barely there white outline (not very visible in the pictures). Not circular. Not unbearably itchy but itchy. Also I was itching near my vagina and I itched it a lot with toilet paper and then I saw a bit of blood on the toilet paper. My period just about to end after like around a week and period related bleeding is almost non existent now. Why is my vagina area so reddish? Last sexual encounter was middle of June. Penetration didn't happen. Condom always on when we were trying to get it inside. Gave him head without condom though. In early march we tried with condom but also tried getting it inside once without condom but it was unsuccessful. That time I gave him head without condom too. When he has sex with strangers he wears a condom but they give him head without condom. He has a high body count. His penis looked clean and he is hygienic. He fingered me also. Only person I had any sexual encounter with. I always use a bidet and then dry it. I also use wipes down there but mostly I use bidet"
Today: The spot is like barely visible down there now (I have a lot of hair there though) and everything feels totally pain free and comfortable. Yesterday, bidet felt like hell. Today it feels completely fine and I also showered and washed the area comfortably. Not itching. Bled a drop of blackish blood though but I think that must be remnants from my period
I live in a hot and humid tropical country and sweat a lot. Wear jeans and stuff. My period usually lasts a long time. Sometimes I keep the same pad on for a long time and also overnight too. the pink spot is at the area where thigh meets crotch area. First pic is yesterday, second pic is today

My medical school does not have a lot of research projects going on, especially for medical research. Hoping to do IM residency in the US

I have relatives who own and run a multi-branch clinic in the US. How exactly will it be possible for me to get USCE there during my clinical years if the clinic and my medical school are willing to allow everything necessary. There's a guy who I've been in a situationship with for like 7 months now and now he wants to actually commit and be my boyfriend, which I have implied that I want like many times. But well. I'm lower middle class and I'm working somewhere right now before starting university (which is free here) and well he lives in poverty basically. And he wants me to give him some money (pretty low amount (like 50-100 usd) but like 15-25% of my monthly income in a very expensive developing country) monthly for two months before he gets a job. He needs money for some official stuff in order to get a job. He also needs a haircut. And it makes me extremely sad to think that he can't even afford that and his family can't afford that to such an extent that he is asking me, which definitely feels like a last resort. I'm pretty much financially supported by my parents and my job is just so I get out of the house and spend my time doing something productive. But I'm saving all of the money I'm making so that I can have some independence and pay for housing and stuff so I can be a little self sufficient cause well my parents are really strict and also no one in my family ever lived at home after high school. So idk. I don't know what the ethical thing to do is. He wants to be my boyfriend as long as I agree to the money thing or he wants to move on and find someone else who would agree to it (which, honestly, does not sound probable). I don't want him to have zero money and be unemployed. I also feel like giving money to your boyfriend as a teenager is not a great idea idk.

Is it actually possible to give the USMLE and actually get a decent score if I'm not going to med school in the US? When does one give the USMLE step 1, step 2, etc? How should I prepare for that? I heard that some residencies require "research items"? Would a medical school like mine have research opportunities that those residencies care about? I know that my medical school is accredited. Would I really get to decide which residency I go into or is it largely based on what I can get? Like, say if I want to become a radiologist and only a radiologist, is it possible to get stuck in another field because I had no option and just took what I could get?
