How do I get out of the "Victim" mindset?

I'm writing this post because I'm going through experiencing another episode of facing problems because of the mistakes that I made all over again, like the usual. I am really looking for some advice here because I really am confused about what is going on with me...

I grew up in a household of two parents who always had a "victim-mindset". Where they constantly blamed their short comings on others and the circumstances, this got especially worse when my older brothed used to hit me and yet still they chose to not do anything about it and instead turned it around on me and told me that it's because my actions that made him do those things to me. They also added saying that I should just listen to what he says and not argue or ever say no to him. After a few years I realised how this affected my own relationships and friendships. This is just one example but they did the same thing with a lot of other things in my life, either blaming me or blaming things around me. I am now in a position in my life where I should be able to take responsibility and accountability of my actions but I physically feel like I can't and that I don't.

I tried finding a way to resolve this for so many years but only one thing ever worked (at least according to me), the one time I went on a solo trip to volunteer for a mental health campaign and I felt ALIVE after years, I stopped blaming, I stopped beating myself up and actually owned up to myself and became better. It went on for about a week until I came back to my parents house and everything started tumbling back down. I have now moved out and live with my partner, but the habits still haunt me and makes it feel like they will never go away. It feels like there's a voice in my head that tells me that I'll have to go back there to that unsafe place again, and no matter what I do their habits will inevitably follow me home.

I feel lost and am not sure how to get out of this loop because this doesn't feel like me at all. If anyone has any advice on this please let me know.
