Duostim- anyone tried this?

Hi! I'm considering a duostim (regular ivf cycle, then a few days after retrieval, start stimming again in the luteal phase for another retrieval 10-20 days later)

Please let me know what your experiences are with this. I have heard a lot of mixed things. I'd love to know:

Age, AMH, AFC, # retrieved each time, medication protocol, and how it went for you. Do you recommend it?

What priming protocol worked for you?

I will begin a luteal start priming protocol and I am considering T+omnitrope without estrace since I believe my ovaries were over suppressed last cycle from estrace, my clinic is encouraging I take estrace this time too.

For everyone with dor, what finally worked for you for priming?

Broker question- Offsetting debts in DTI calc to refi?

This question is specifically for mortgage brokers. PLEASE don’t jump in and say boarder income doesn’t qualify as income for a refi- I know this!

I’m hoping to qualify for a refi with a lower income than I had when I purchased this home.

A former broker friend said boarder income could be used to offset my high mortgage expense in my DTI calc so that I can qualify for a refi. E.g. debts minus boarder income = actual debts used for DTI calc, therefore my salary can be lower to qualify for a refi.

My credit score is 840 so no issue there.

I know boarder income isn’t used as qualifying income when calculating income for DTI, but he said it could be used to nearly zero out my mortgage expense.

Mortgage (with PITI) $6009, APR 7.5%, ~$730k principal, home will appraise for >$1.1M, maybe $1.25M.

I was earning 212k/yr w/no debt when I purchased it. Now I earn ~$65k salaried, plus $5200/mo in rent. I’m also in the process of building an ADU on the same property that will rent for $2500 later this year.

So instead of evaluating “can this person earning 65k afford a 6009/mo expense?”

I am hoping to find the correct terminology to get to a broker to evaluate “can this person earning 65k afford a 6009-5200 ($809) expense?”

I have impeccable records of long term renters with leases, receipts, and I pay 4.5% GET Hawaii tax on this rent too for more than 2 years. I feel like I should be able to figure this out especially since I’m paying the tax on it!

How much were your meds?

We are considering cash pay for another round. Willing to travel to other states or countries.

How much were your meds for a round, what meds did you have (and quantities please), where are you located, and what was your copay/coinsurance % if you don’t know the total cost?

Considering cny but I’m reading their affordable costs don’t include the meds.

Our first round, in Hawaii, with 20% copay for some meds, I estimate our total med cost to be ~$17k if insurance didn’t cover 80% of 3 of them.

Menopur 24: $1200.99 ($250/75 bottle)
Folistim 12: $1915.53 ($798/450 vial)
Ovidrel 2: $96.46 ($241/unit)
3212.98 total / 20% = 16064.9 total.

Ganarelix: 5 $174.93 not covered by insurance (~$35/unit)
Omnitrope 1 bottle $523.96 not covered

$16763.79 total.

Would you wait for the next cycle or start ivf now?

Should I be worried I haven’t primed enough? My clinic hasn’t offered MDL. Started estrace on cd 20 and my period started early on cd 22. Today is cd 4 and we start stims on cd 6.

Im 40. Amh 0.65 a few weeks ago, but 3 months ago while in hbc it tested 0.96. I think some supplements I was taking (ovasitol and nac) suppressed it and I have since stopped taking these about a month ago.

I have adenomyosis and we plan to do suppression after retrieval, but nothing has been discussed about mdl beforehand. Our insurance only covers one cycle, so this is the one chance we have.

After learning about mdl from this group, I wonder if it would benefit me.

I’m worried since estrace triggered my cycle to come early, I’m not adequately prepared for this stim round.

AFC count 6 days ago was 8 total.

Wait a month for AMH to increase or start now?

Would you wait a month in the hopes your amh goes up to join a ivf trial w/ 2 free rounds or start ivf with insurance now that only covers one round, at age 40 with medical conditions that nearly guarantee one round won’t be enough?

Everyone says at this age, it’s important to just start and don’t wait even one more month.

TLDR my amh may go up in a month and make me eligible for the grace ivf study which includes 1-2 FETs and 3 transfers free. The study trial is for an existing bioidentical ivf medication that a competitor is in final stages of fda approval for. There is an 80% chance I’ll get the medicine the first time around, and 100% chance for the 2nd cycle regardless of the first one.

However, in 40, nearly 41, with adenomyosis and DOR. Our insurance only covers one FET and one transfer. The longer I’m off hbc, the worse the adeno will become and the harder it will be to do ivf.

My AMH may go up because it was 0.96 in March when I was taking slynd birth control continuously. At that time, I was told this was suppressed on hbc and would increase 30-55% after stopping. I stopped taking hbc 9 weeks ago, and just got results back today at 0.62. I need to be at least 1.2 to be eligible for the free ivf study.

All data is pointing to the supplements I was taking recently (inositol, choline, NAC- these are good for ttc unless you have DOR I just learned 💩) and the continuous birth control and spironolactone I was on for 2 years continuing to suppress my amh production right now, and that I need a true 90 day washout for the ovaries to fully wake up and have amh return to normal levels.

This will be our first round. We are approved to start the one and only round that our insurance covers. We do plan to do pgt-a, icsi, zymot, 3 months of lupron to shrink my uterus, etc, regardless of whether we are in the trial or not.

We can’t do the trial later since the trial requires no history of failed transfers.

What would you do if you were in our situation?

Tips to rejoin tech after 2 years of burnout?

I was a senior eng manager with a terrible boss who fired me one month after I got a raise, after I told her about my severe gynecological health issue that needed surgery. She sucked. It’s okay, I don’t need condolences.

I haven’t worked since. I don’t have kids. I just needed a lot of time to recover from the health issue and burnout. I want to join other industries, but nothing pays as well, especially in my location (Hawaii)

I was good at my job and I did enjoy it when things were going well.

Tips on how to get hired after 2 years of no tech jobs? I do have technical things to put on my resume to fill in the gap so I’m not worried about that, but tech has changed so much since I was last in it, not sure where to even begin. I know I don’t want to be an IC again. I’m better as an Eng/technical/product manager.

We need atisoban now! So mad at our medical system

I just learned what atisoban is. It is a highly effective oxytocin blocker that stops uterine cramping.

TIL TLDR: endo and adeno cause the uterus to have too many oxytocin receptors. These overreact (think just like an overactive immune system) causing cramps and pain, as well as MC and preterm labor. Did you know this? I didn’t!

The treatment for this is effective, but only permitted for use during ivf transfer to ensure implantation, reduce risk of miscarriage in the first trimester, and to prevent early labor in the third trimester.

It is not yet available in pill form (trials are ongoing) and only permitted to be used for the above reasons.

Personally, I’ve experienced such severe cramping that I absolutely would go to a hospital to get an IV of this stuff if I knew it would eliminate cramps. Wouldn’t you?

Our medical system is fucking infuriating. I am enraged that we don’t adequately fund women’s research. UGGHHHHHHHHH JUST IMAGINE ALL THE PAIN WE COULD ELIMINATE IF WE FOCUSED MORE RESEARCH ON THINGS LIKE THIS.
