Form DE 2500A needed but nowhere to be found?

I am out on disability as I had a baby. My due date was July 17th, I had the baby on July 16th via csection.
I have been getting regular payments on the EDD card since 4 weeks before my due date with no issues.

Recently I had something come in the mail saying I needed to send in an appeal, which I couldn’t find anything in my portal about for more info at all, my claim still had active status and everything looked good, couldn’t get through to a rep when trying to call. I mailed in the appeal just in case, and sited that I hadn’t received any information why I need to send an appeal and that I’m still recovering from birth and all of my dates online are up to date and accurate.

This morning I logged in and instead of saying Claim Active, it says Certification Needed.
When I click on it it states:
Certification Needed: To continue receiving benefits, you must complete a certification. Visit Available Forms or your SDI Online inbox to complete the Cert for Continued Benefits (DE 2500A).

I have checked the Available Forms section, and my inbox to see where that form is to submit and it’s in neither of these places.
The Available Forms section only has an Appeal Attachment, or a 2587 which you fill out when you return to work.

My inbox has nothing.

My return to work date in my portal is set to 9/10/26, which was updated when I notified them via online that I had a csection.
I am supposed to receive another payment tomorrow for the previous 2 weeks, which I am relying on that payment as I now have a baby to take care of, but nothing says that I will or will not receive the payment..

Does anyone know where I can get the form they require so my payment won’t be delayed? I’m really stressing out about this cause this is my only source of income.

Got a letter that claim was canceled but nothing was sent on my portal

EDIT: DISQUALIFIED* NOT CANCELED

SECOND EDIT: I just logged in to my portal and it now changed back to “Claim Active” so I’m not sure what’s happening at all..

I had a baby and am out on disability. I got a letter in the mail today, the 13th, stating that my claim that I’ve had 3 payments for already was canceled on the 10th of August, and had the dates for 8/3/26-12/31/9999, it had boxes to check for the reason it was canceled but none of them were checked off. It has a separate section that listed a code 2706-3(c), which when I looked it up said failure to produce medical documentation.

I had received an email on the 10th in the morning about something in the portal that needed to be submitted (the extension for csection delivery.) I submitted it as asked, and it said everything was good. The return to work date updated in my portal shortly after to reflect the csection extension. I never received any type of notice anything was wrong or further info was needed. The portal never told me anything was canceled. All through the 10th it said the status was Active. I logged in today after receiving the letter and it said Active.

However I logged in later today by an hour and it said “We’re processing your claim” and now gives an error message about the system being unable to process my request at this time.
I am planning on calling them tomorrow morning asap to make sure everything is ok but just wondering if this has happened to anyone before? I am relying on each deposit I get for my baby, and the next one is supposed to be this coming Tuesday, but without any notice except this one mailed letter idk what I’m supposed to do if it really is canceled..

It gave me a mail back option to appeal it, and said I can do it online to but when I go to the appeal section there’s nothing for me to select to appeal..

Questions for flying with a newborn?

I am going to be traveling in September with my then 8week old baby. It is a domestic 4 hr flight, and hr layover, and then a very short less than an hour flight. I have read the tips to feed the baby on take off and landing if possible or have a binky for them to suck on. We are mostly planning on holding the baby or having them in their body carrier I just had some additional questions about the travel itself:

Do I have to buy a ticketed seat for my baby? We were planning on purchasing the seat in between us, so no one would be there, does that seat have to be in my baby’s name? Or can it also be in my name and I have 2 seats?

Can the baby’s car seat come onto the plane with me? Or does it have to be checked? If I purchase the seat, can the car seat be on that seat and strapped in? It’s small and rear facing, it’s a carrier and car seat in one that attaches to the stroller (I know the stroller part has to be checked)

If I cannot bring the car seat onto the plane and have to check it, can I bring the bassinet attachment to the stroller and have THAT in the extra seat I purchased? So the baby can lay down if needed?

If I have to add the baby on my ticket as a lap passenger does my baby need some sort of ID? Birth certificate? Anything to be identified with? (He only has a birth certificate and a SS card.)

Sleepwear for newborn

All of our swaddles are in the dryer, and I’m worried I might have dressed him too much. He is 5 days old.
The house is around 70-75°. I put him in a short sleeve onesie, and a bamboo footie pajama. He also has mittens on to keep him from scratching his face.
No blankets or anything in crib.
Is this too many layers for safe sleeping?? I have no one else to ask and I can’t find any answers on Google.
I will be replacing the outfit as soon as the swaddles are done drying but for the next hour I just have to know if it’s ok for him to fall asleep in.

EDIT: Thank you everyone, in my moment of panic I kinda just wrote this for help but everything is ok and I figured it out. He’s only 5 days old and I’m still getting the hang of things, I was just scared and needed an answer right away. I appreciate all of you!

Are the deli sandwich chips back anywhere?

I have been pregnant since November, and have wanted these for MONTHS, and now that it’s finally summer I thought they would bring them back but my local store in Northern California does not carry them. Were they discontinued? Has anyone found anything similar? … can someone mail me chips 😭
