HRT?

Hi! I had er+/pr+ stage 1 idc almost 10 years ago at age 29. I’m getting close to my bilateral oophrectomy for my brca1 status and curious if anyone else with a history of hormonal breast cancer was able to convince their doc to let them use HRT? I’m terrified of how i will feel, not to mention the known risk to heart and bones (already have osteoporosis!) and mood and… anyway. Looking forward to hearing from you all and happy to answer any questions!

Gonna cross post in r/brca too!

IVF/FMLA

I am currently using intermittent FMLA for IVF, which I hope to wrap up by the winter with a transfer maybe in sept/oct and then I’m calling it quits.

I also am thinking about getting a new job, but am feeling stuck because of the FMLA, which as I understand, won’t be available until being with a new job for 12 months.

What do others do in this position? Especially knowing that I could (hopefully) end up pregnant and would need additional FMLA coverage for a pregnancy, too, but a delivery would be over a year away if my Sept/Oct transfer works.

I work in a large health system as a nurse if that’s relevant. I’m sure I’m not the only person who has been in this position lol feeling super stuck!!!

E2 drop at the end of stims?

Hi everyone!!

I’m on my 6th and final IVF egg retrieval and was hoping to sanity-check an estradiol trend before trigger.

My cycle has been a fairly high responder with a large cohort of growing follicles, and I was originally expecting to trigger last night, but my clinic had me do one more low-dose day.

My last several days of labs:
5/27 → 3780
5/28 → 5327
5/29 → 6623
5/30 → 6603
5/31 → 4116

P4 has been slowly rising but still relatively low (0.9 → 1.5 range → 1.12 today).

Med adjustments over the last few days:
Gonal-F tapered (200 to 150 to 25 for a few days.. then 450 IU “boost” today when they called)
Menopur reduced and then stopped/coasting
Antagonist daily

Ultrasound today:
Left ovary: 13 follicles (11, 16x2, 18x3, 20x4, 24x2)
Right ovary: 10 follicles (11, 14x2, 15, 16, 17, 19x3, 22)
Total: ~23 follicles, with a strong cohort in the 18 to 24 mm range.

One important detail: my usual blood draw is ~9am (about 12 hours after meds), but today it was ~12:30pm (about 15 hours after meds), so timing was different.

Clinic is having me trigger tonight, and the nurse said they see E2 drops like this sometimes and it is not concerning.
I’m mostly trying to understand:

  1. how common it is to see a significant E2 drop right before trigger
  2. whether lab timing differences or coasting can explain it
    and
  3. whether this is clinically meaningful when ultrasound still looks stable

Appreciate any insight or similar experiences. Would like to have my expectations appropriate for the retrieval on Tues.

Who has had a lap just to see even if no symptoms (except I presume infertility)? Did they find endo? My MRI is suspicious for adeno, which i will use suppression to deal with, but I’m not sure if a lap is worth doing just in case?
