Need advice: Private school withholding Form 137 over auto-generated Grade 12 balance (Never enrolled or attended)

Hi everyone, especially to our DepEd teachers and school administrators here. First of all, I want to apologize for posting this here, but I am currently in a very difficult situation and I really need some advice regarding my Senior High School records.
I completed Grade 11 at a private school under the DepEd Voucher Program. For Grade 12, I decided to transfer to a public high school. I did not sign any Basic Education Enrollment Form (BEEF), did not sign any contract, paid zero fees, and never attended a single day of class at the private school for Grade 12. In fact, I was never encoded in the LIS under them for Grade 12, and my official Certificate of Registration (COR) shows a payment status of 0.00 with TBA class schedules. I am currently actively enrolled and attending my Grade 12 classes in my new public school.
However, when my public school issued an official Request Letter for my Form 137 / SF10, the private school registrar refused to process or transmit it. They claim I have an active portal balance of around ₱8,700+ because their system automatically rolled my account over for Grade 12. The registrar told me that since I didn’t formally "withdraw" or notify them, I am liable for the auto-generated fee, and even claimed that voucher students are required to finish Grade 12 in the same school (which I know isn't true under DepEd guidelines).
I know that under Civil Code Articles 1318 and 1157, no contract exists without consent, so an auto-generated fee cannot be a legal basis to hold records. Also, DepEd Order No. 54, s. 2016 mandates school-to-school record transmittals. Since the registrar is refusing to cooperate at the counter window, I am trying to figure out my best options.

Should I file a simple Letter Request for Assistance first, or go straight to a formal complaint?
In the meantime, can I execute an Affidavit of Undertaking at my current public school so my Grade 12 standing/graduation won't be delayed while this transmittal issue is being resolved?
