Full endocrine work-up complete, clinic happy to start TRT + hCG. Does this sound reasonable £?

Hi all,

Thought I’d post an update since my one last week and get some opinions from those with more experience.

Background: 34M, 6’6”, train regularly, no medications. Main symptoms have been reduced libido, poorer recovery, lower energy and generally feeling like I’ve declined over the last couple of years.

My initial bloods were mostly normal:
Total T: 18.3 nmol/L
Free T: 0.405 nmol/L
LH/FSH: Normal
SHBG: 29
Estradiol & prolactin: Normal
The only abnormality was progesterone, which came back at around 3× the male upper limit.

Because of that, the endocrinologist delayed TRT and ordered further investigations:
ACTH – Normal
Cortisol – Normal
17-hydroxyprogesterone – Slightly elevated (4.70; ref <4.24)

She said she’s happy there’s nothing significant enough to stop treatment and is comfortable proceeding with TRT.

The proposed protocol is:
Testosterone cypionate 200 mg/mL
- 0.25 mL twice weekly (100 mg/week), although she said I could increase to 0.3 mL twice weekly (120 mg/week) if needed.
- hCG alongside it (waiting for confirmation of the exact dosing schedule).

The medication quote is:
Testosterone cypionate (10 mL): £385
3 × 5,000 IU hCG: £420
Injection kit: £35
Total: £840 (for 3 month supply)

A few questions:
Does this work-up and treatment plan seem sensible?
Would you start at 100 mg/week or go to 120 mg/week?
Do the medication prices seem reasonable for a UK private clinic, or am I paying a significant premium?
Are there any other UK clinic options I should consider before committing / could I transfer this so I don’t have to resit all the tests with another clinic?

Appreciate any thoughts, especially from anyone who’s gone through a similar optimisation-focused route rather than classical low-T treatment.

Very negative facebook adverts with lots of nasty TRT side effects?

hey all

I've had the idea of TRT in my head for a while now, or at the very least getting tested.

46, work two jobs, and have a 4 year old, so life is pretty busy even at the best of times, and I'm sure I could make some lifestyle changes that would help things. Have started that process.

But have been thinking about at the very least getting tested to make sure that low T isn't also a part of why I feel a bit rubbish these days. Even ordered the initial finger prick test from Optimale probably about a year ago now, though never actually got round to doing the test and sending it back. Would probably go with Origin now based on what I've saw over a few nights looking through this subredit.

However the all seeing eye of facebook has obviously noticed and has started giving me lots of adverts for supplements like Mars Men and Zapply T1. While whats in these supplements I'm sure might give me a touch of a boost, they're mostly nonsense in terms of raising T levels?

However the other thing that keeps popping up is sponsored posts like this one below which, to save you having to click on the link if you'd rather not, is a post that claims to be a doctor who's worked in the field for years, but "refuses to keep quiet any longer". How the problem isn't really low T but other issues that TRT will only mask for a while. How the side effects will be really bad, etc etc. and their supplement is the only thing that will help. I've had a bunch of slightly reworded ones, different doctors names, different supplements, but all basically the same.

I'm fairly certain it's all also nonsense, but what are people's thoughts. Have others also had these kinds of things pop up?

UK Male 41 looking for advice

Hi, I had my bloods done a couple of years ago and it came back with high SHBG. At the time I had a young child and getting very little sleep, so I put it down to that.

Fast forward to today and I’m getting much more sleep (although not perfect) and my fatigue hasn’t budged. I’ve also noticed a drop in libido.

Thought I‘d get another test and these are the results:

Total testosterone: 18.7 nmol/L

Free testosterone: 0.231 nmol/L

SHBG: 68.9 nmol/L

Free androgen index: 27.141

Turns out I’m also deficient in Vitamin D and I’ve started to supplement that.

I understand that I’m not in the range for TRT but I feel like my low Free T is probably a major factor in my issues. I’ve seen varying opinions on supplements like boron and tongkat ali.

Would love to know what you lot would do in my position. Have any of you been here and managed to rectify it?

Thanks to everyone who replied. A few people asked to see the results, so here they are. Quite a difference compared to the finger prick test a few days ago and my NHS test a few years back (14.8 ish).

I’ve made a lot of lifestyle changes since then, so I was surprised the result came back higher.

Everything else was normal apart from:
SHBG: 41
LH: 11.8
FSH: 13.5

I still have the symptoms, so I’ll be looking into the raised LH and FSH to see if that’s related, but Voy doesn’t seem concerned. It definitely shows there can be a big difference between a finger prick test and a venous blood test.

Thinking about ditching Voy/Manual/Optimale whatever it is called

Does anyone have suggestions for private clinics they’ve used? Getting fed up with Voy, the service is poor and it’s very expensive. I don’t feel like I’m getting value for money.

I require T Cypionate and HCG. Thank you.

Who has Left Voy/Manual and what were your reasons for this?

I’ve informed after nearly a year with manual/Voy that I am cancelling my subscription as I’ve experienced little to no improvement effects other than at the gym with Voy. Body recomposition has improved muscle built etc.. I can lift heavier and got more strength. But apart from that I still suffer same symptoms as having low testosterone. My latest results are these. And in April they were higher. So I’m going it alone and going to be using my local blood clinic who do full blood panel for £275 inc face to face consultation for trt dose etc.. who else has done the same as me ?

Question about my results :)

I’ve been getting the usual symptoms lately, but they seem worse than before, low energy, no drive, brain fog, irritated, no morning wood, that sort of thing.

A few years ago I tested at 14.8. Since then I’ve lost 50kg over about 3 years, don’t smoke or drink, eating at my 3k maintenance, I’m a healthy weight now and pretty active, so I honestly thought my levels would have gone up. Instead, I got my latest results back yesterday and they’re attached below.

I’m thinking it’s probably worth doing the enhanced blood test to check more markers and rule other things out, but I wanted to get some opinions on these results first. I know it’s symptoms as much as numbers, but do these levels seem low for a healthy, active 30 year old guy?

Clomid/ hCG only?

Hi all,
I’ve been looking into and considering starting some form of testosterone support (either Clomid or hCG) as I’ve been feeling extremely fatigued and generally down for the past 3 months.
As I’m based in the UK, I’m planning to go through NUMAN. At this stage, I’m not looking to start full TRT, mainly because I’d like to have a family in the future and, given my age, I’d prefer to avoid committing to TRT if possible.
Does anyone have experience with Clomid-only or hCG-only treatment, particularly through NUMAN? I’ve seen a lot of mixed opinions on Clomid—some people say it’s been amazing for them, while others say it’s awful.
Unfortunately, enclomiphene isn’t available through NUMAN, so unless anyone knows of a legitimate UK source, that’s probably off the table.
Any advice or experiences would be greatly appreciated. I just want to get back to feeling like my normal self again.

34M - Elevated progesterone (3x upper limit) discovered during TRT assessment. Anyone experienced this?

Hi all,

Looking to see if anyone has had something similar or can share what happened in their case.

I went to a private TRT clinic because over the last couple of years I’ve noticed a real decline in:
Libido
Gym recovery and muscle strength
Sleep quality
General energy
For context, I’m 34, 6’6”, train regularly, don’t take any medications or supplements, don’t drink alcohol at all, and had a testicular ultrasound recently which came back normal, after a ‘panging’ pain a few months ago unrelated to my TRT assessment.

My initial bloods showed:
Total Testosterone: 18.28 nmol/L
Free Testosterone: 0.405 nmol/L
LH: 5.0 IU/L
FSH: 5.7 IU/L
SHBG: 29 nmol/L
Estradiol: 89 pmol/L
Prolactin: 234 mIU/L
DHEA-S: 8.85 µmol/L

Everything looked normal except progesterone, which came back at 1.4 nmol/L (male reference range <0.474), so around 3x the upper limit.

The endocrinologist has paused any TRT discussions until they’ve investigated the cause. I’ve now had a repeat early morning blood test for:
Progesterone
17-hydroxyprogesterone
ACTH
Cortisol

I’ve been told they’re basically looking to rule out adrenal causes (such as non-classic CAH) before deciding whether TRT is appropriate. If those tests come back normal, the plan is to start 200 mg/week testosterone cypionate alongside hCG - this is optimisation, not to bring me into “normal”range, fully aware of that.

Has anyone here had:
An isolated high progesterone result?
Elevated 17-hydroxyprogesterone?
Additional endocrine investigations before starting TRT?

If so:
What turned out to be the cause?
Did everything come back normal?
Were you eventually started on TRT?

Did TRT help if your testosterone was technically “normal” to begin with?

Not looking for a diagnosis, just interested to hear if anyone has been through something similar and how it played out.

Am I cooked? What can I do about my SHBG?

So I did a couple of finger prick tests and it was showing high SHBG. I then got a fully hormone panel done with my latest medical and it confirmed the high SHBG reading. I am not suffering too badly from side affects apart for a bit of sluggishness and lack of motivation but that could be put down to other things.

For context I am 43 years old, 185cm, 82kg and work out 3-5 times a week with a combination of weights and cardio. I already take boron 3mg daily but it obviously hasn't had any affect. Is there any other recommended supplements or anything else I should consider?
