Need help regaining mobility and functionality

I’m a 21-year-old male and I had 5 compression fractures due to steroid-induced osteoporosis, immobilization, and malnutrition during a hospital stay.

After the fractures, I experienced severe muscle spasms, especially in the quadratus lumborum and latissimus dorsi. They have improved significantly, but I still have some pain with movement.

I have pain/stabbing sensations/a feeling of blockage in the lower back/sacral area when lifting my leg while sitting or lying down. I also have pain/stabbing sensations in the lower back/sacral area when bending my torso forward while standing. When performing a dip movement, I feel stretching/tightness in the area where the serratus posterior inferior is located, and when walking I feel mild pain/tightness in the same area.

In the morning, I experience pain/stabbing sensations in the lower back/sacral area, which improve throughout the day.

In the morning, I also feel like I cannot properly support my own body weight while sitting, but this feeling of instability improves by the end of the day.

I also have polineuropathy wich doesnt help with the mobility but first i need to adress my back problems.

I need advice to help me regain mobility, functionality, and strength because I am very deconditioned.

Please ask me anything you need to help you better understand my situation. Thanks

21 M Need help to gain mobility/functionality back

I’m a 21-year-old male and I had 5 compression fractures due to steroid-induced osteoporosis, immobilization, and malnutrition during a hospital stay.

I live in Portugal, and the healthcare system here is questionable, so I prefer to seek help here rather than make an appointment with a physiatrist.

Multiple lumbar spinal compression fractures (osteoporose) at 21 and trying to regain mobility

I’m a 21-year-old male diagnosed with GEPA vasculitis, which caused severe distal sensorimotor axonal polyneuropathy with foot drop in my left foot.

I was hospitalized for 2 months, during which I lost 20 kg because I couldn’t eat enough. After being discharged, I was prescribed 70 mg of Prednisolone per day, which quickly caused me to regain those 20 kg, mostly as fat. I stayed on 70 mg for 4 months and am currently on 45 mg.

During the third month on 70 mg, I went to a rehabilitation center. I left with back pain, and about a month later, I had to go to the emergency department due to much more severe back pain. I was discharged with a diagnosis of sequelae of fractures from T12 to L4, as well as osteoarthritis and disc degeneration (spondyloarthrosis). I was prescribed calcium, vitamin D, and a rigid TLSO brace, most likely due to corticosteroid-induced osteoporosis.

I believe the fractures may have occurred during my rehabilitation, even though I didn’t feel them at the time and continued doing very demanding mobility exercises. I ended up bedridden for one week out of fear of causing new fractures and to try to reduce the pain.
After that, I tried to move more by walking around the house, but the pain and lack of mobility have been getting worse. Today I had a musculoskeletal consultation to assess whether I was a candidate for a facet joint block, but the physiatrist examined my spine and said the pain seems more muscular, so facet inflammation is unlikely.

Other symptoms include a sharp, stabbing pain in the lower back when bending forward, and strong muscle spasms in the quadratus lumborum area in the morning with any movement or step. In the afternoon, the spasms become calmer and less frequent. In the morning, I feel unable to support my own weight without using the TLSO brace. I also cannot lift my leg while sitting — I feel a “block” in my lower back, making it impossible to lift my thigh.

The doctor recommended starting physiotherapy, as I am already deconditioned and compensations may be contributing to the pain. However, I experience pain and spasms with any movement, and I don’t know how to start moving without triggering pain or spasms.

I’m looking for help from someone who has unfortunately been in a similar situation, in order to regain functionality and mobility. And talk to about the recovery progress.

Vasculite GEPA, heart failure, corticoide, polyneuropathy, osteoporose, spinal fractures at 21

Hello im 21 years old male from Portugal, i recently 4 months ago, been diagnosed with vasculite GEPA, inflammation in small-to-medium blood vessels across multiple organ systems, that caused heart failure 35% and severe distal sensory-motor axonal polyneuropathy.

I was prescribed prednisolona, been taking it for 4 months at 70mg then reduced to 50 mg and now im on 45mg. A medication that probably saved my life but has alot of bad side effects . With that i think i got osteopenia or osteoporose since i recently discovered i have 5 spinal compression fractures ,d12, l1, l2, l3, l4, but still have not did the densitometry exam to confirm it. Waiting ,in bed scared to move so i dont get another fracture, to be called to do the densitometry exam and talk to a neurosurgeon. Thinking about going to a private hospital and ask for the exam myself and talk to an endocrinologist about starting medication for osteoporose since the public health system in Portugal is slow.

This post is very vague since im not really good at english and wrote it just to vent about my situation.
Any tips are welcome.
