My cat only uses the litter box 50% of the time and I’ve ran out of solutions.

My (M25) cat, Frankfurt (M11, Nuetered) has occasionally had accidents. Usually in the shower or bathroom. But as of the last 3-4 months he’s been only using the litter box 50% of the time.

He urinates in the litter box 100% of the time. He often defecates far from the box, in other rooms completely, often on carpet. I’ve brought him to the vet twice, he’s had multiple stool samples tested, as well as blood work. Everything checks out good and there’s no obvious problems. Sometimes his stool has mucus or bright blood. The vet thinks this is pancreatitis. The two solutions the vet has had me try is Proviable (probiotic + prebiotic) as well as a hydrolyzed protein diet. Neither of these has seemed to help, and his stool is still very loose.

I have 2 cats, they’re brothers. My other cat, Benny has no issues with the litter box. I have 3 litter boxes total, 2 are near each other, and 1 is across the house. These litterboxes are cleaned twice daily.

I feel like I’ve tried everything and no avail, I’m considering rehoming them due to this issue. Does anyone have any suggestions or anything to try?

TLDR: cat defecates in random places across the house, 3 clean litter boxes, no obvious health issues as per my vet.

Edit: Thank you all for the help, I want to add the the boxes are large, uncovered, and plenty full (2”-4”). I’ve tried multiple different styles of litter, mostly “world’s best” and regular clumping litter from arm and hammer.
