Loaded beans

I just made something called loaded baked beans. I like the idea, but would like a different flavor.

This was what I did had in it...

Ground meat browned, put aside

Precooked sausage browned

Onion and bell pepper added.

Added can of baked beans drained.

Bbq sauce

Mustard

Garlic powder

Worcestershire

Brown sugar.

Salt

Pepper

Maybe missed something

Added cheese later

Too much of a beanie and weanie taste, but I liked the concept.

Thought what could I do with black, pinto, or other beans that could be put together this way.

Something with green chillies maybe..

So:

Ground meat

Precooked sausage

Onion, pepper...

Some type of beans

Skip the bbq sauce etc.. do something different

Can I use these mushrooms?

First, I am cooking more regularly and feel more comfortable doing it. Nothing complicated, but things are decent for when my daughter visits. 3 entirely different dishes for us last time.

This site has been a big help.

Tonight I decided to braise some chicken thighs in some broth and wine w onions, potatoes, and carrots.

I just noticed a half carton of sliced mushrooms that I used maybe a week ago. A bit dry, seem decent enough to be thrown in here.

When are these mushrooms to be tossed?

How to store these in the future?

Baking chicken in a metal cake pan

I have a large toaster oven that I like to use since it preheats very quickly. Better than heating up my kitchen with my oven.

Baking some chicken. My glass casserole dish barely fits.

But I have a metal pan (i guess for a cake) that fits more easily. Also, I have a cookie sheet that fits on the top nicely.

Instead of putting foil on my glass casserole dish, and chance spilling, is there any reason not to use my metal pan?

RTs are not RPSGTs and are not doing full studies when titrating PAP

(ADD FOR CLARIFICATION: Expecting RTs, without the advantages of the monitoring that an RPSGT in a sleep lab has, to have the ability to titrate a complicated patient, is not a good idea.)

Do the best that you can, but sleep is its own animal.

In REM sleep muscles used for respiration can change

When doing PSGs sometimes people who do not have good diaphragm function (COPD, obese, had one guy with a paralyzed hemidiaphram) are using their accessory muscles for breathing.

Hit REM and lose them.

Person w a paralyzed hemidiaphram was doing well while I titrated his CPAP to get rid of obstructive events... until he hit REM and the bottom fell out...

I got him into REM, he stopped the normal arousing himself to snort and half wake up, and instead just went apneic.

So, possible that there's OSA and the hypoventilation for that reason.

That's why I really don't like RTs to have to play RPSGTs when setting someone up in the hospital."

Communion wo kneeling

I attend a church that usually administers communion at the rail.

Yesterday they did the walk by method. I am inquiring why, just so I know. I sent a message yesterday.

​

I have been in churches that do that regularly. I often felt rushed. I would usually carry it back to my seat.

​

Anyhow, if we are going to do this more often, ill let them know what I will probably do and why.

Recommendation of a Lutheran perspective on the Psalms

I am reading a bit through the Psalms.

Looking for some supplemental reading.

​

I started at Ps 38. Very powerful and penitent.

​

Looking at the previous ones, 35, 36 and 37 its David speaking about God's protection of the righteous over the evil people. How great He is and calling people to keep Hus ways.

​

Then 38 is like: "Oh, Im wicked..."

​

39 more introspective, seeing the need to know he is nothing so he won't sin.

​

40 seeing the deliverance God gave him, and learning a lesson that it's not about the sacrifices, but about the heart (and also is used in the NT concerning the Christ).

Love God

​

41 more about the heart, but toward people (the poor). Love my neighbor.

​

Then that book of the Psalms ends.

​

Also, looking at 36. Quite a Psalm considering the circumstances it relates to.

Does through the Psalms with Luther seem like what I'm looking for?

Setting up an AirCurve 11 to auto cpap

Hospital setting. Pt uses an auto cpap at home and uses a range of 10 to 18.

We have some AirCurve 11s. Im not greatly familiar with this.

​

If I want to set it up as an auto CPAP with a range of 10 to 18 (instead of Bipap), do i just go to VAuto and set the max ipap to 18, minimum epap to 10 and the PS to 0?

Browning chicken. Any seasoning no-no's or would be good to do's?

For instance, if i am browning chicken that is going to be in a curry dish, are there some of the seasonings in typical curry that should not be subjected to the high heat of browing the chicken?

(Curried cabbage and chicken thighs)

In this instance, I am not using curry powder, but I am using turmeric, cumin, coriander etc.

​

I could leave out something put on the chicken if needed.

​

Or should I just use salt and pepper and put in my home curry ingredients just in the cabbage?

And with other dishes that I might want to brown chicken in (Italian, Mexican etc) are there things to not put on the chicken when browning?

Constant flow/square vs decelerating/ramp waveforms

I remember when the only way I could get a decelerating waveform was by putting our Severo C and B on Pressure Control. That seems to be why pressure control became popular in ARDS.

Now ACVC can do something similar with a button.

I almost akways only use the decelerating option. Exception is in a rare case of needing to shorten my I time to get a long E time for high rates or severe obstruction.

However, Ive been watching some videos by.people that seem to always use the constant flow. Not occasionally, but almost all the time.

Questions Ive heard:

Why are so may people using decelerating waveform? (I say it allows more of the volume in sooner and thus more time for gas exchange. That was the rationale for using ACPC years ago.), and it's more comfortable for the patient.

On the other side someone asked, "can't you just increase the peak flow and keep the decelerating flow on to get longer expiratory time?" My thought is that I can use a lower flow, have more laminar (sp?) flow by changing to a constant flow.

I understand that Servo has an Adaptive Flow mode which allows some latitude for using the square waveform, but that's a different topic.

Curious about other thoughts and experiences.

What vegetable would be good to put in this rice

Working on varying chicken pieces and rice.

Browned salt and peppered chicken and removed. Sauted onions, garlic, salt. Toasted the rice throwing in the tumeric and cumin (should I have put that in then?

Then added broth to simmer the rice with the chicken it is.

So,

Turmeric, cumin and, at the end fresh dill is my flavor.

(I tried defrosted frozen broccoli, but should have salted it first. Maybe something different instead)

What vegetable would fit in with this (instead of broccoli)

Any other tips?

Basic chicken and rice/pasta recipe

Thinking about some general way to cook regularly.

Brown chicken thighs in a skillet, take out.

Put in onions, garlic, etc. and then deglaze.

Put in rice or pasta with broth.

Place chicken back in.

Assuming the above is okay to expect a decent result, how to finish cooking it and how to observe when its done?

Stovetop vs in the oven and lid on or off?

Maybe put some vegetables in sometime.

Head injured pt breathing

MVC with head injury.

Had a pt with frequent periods of hiccup like ventilation.

Put on ACPC with 5 or 10 of pressure.

Inspiratory flows up to 200 lpm Vts of > 800.

RR in mid 20s.

Sometimes he would flow w the I time, but then the I time was just causing a breath hold.

Cardinals have a nest in this.

I've seen cardinals around before, but know nothing about them

There were 2 chicks in the nest yesterday, but now there's only one. What could have happened? A squirrel?

What do the parents feed them?

Would having a feeder nearby help?

Which Creed.

Today the Athanasian Creed was used, as expected.

But the church I go to now tends to use the Nicene Creed vs the Apostle's Creed usually.

My previous church used the Apostle's Creed most of the time.

I'm guessing that Isa congregational preference.

If so, why would one be preferred over the other?

I am curious how the order of service is put together.

The Gloria in Excelsis seemed to be used for services a lot and then it was not, and then it's back.

Does it depend upon the time of the church calendar? Is it a congregational preference?

And sometime and pasta and broccoli?

My video did not turn out well, so I drew this.

Pt with severe COPD on a PEEP of 5.

A loss of volume return was noticed. This would indicate either a leak or air trapping.

Watching the loop it was observed that there was a repeating pattern. Volume loss, less volume loss, and then excessive volume returned

This was thought to be caused by auto PEEP and an increasing FRC on the breaths until the elastic forces were so great that the air trapping was relieved. Then the cycle repeated.

Usually the instruction is to measure the total PEEP and adjust the PEEP to just under the total PEEP. 

However, often it is hard to get a measurement of the total PEEP.

We increased the PEEP and titrated it until this pattern resolved to a consistent emptying on each breath (12 cm).

I wonder how many times I've missed this.

HHNC on nasal fracture

A patient self extubated.

Had a therapist refuse to use a HHNC on him because he had a nasal fracture. The Dr said there was no communication through sinuses that would cause problems. So, we don't see a problem using a HHNC on the patient.

Thought I would throw this out here.
