This Trump to Bernie converter always brings me tears

If anyone hasn’t watched this episode from A More Perfect Union on YouTube, I highly recommend it. It was basically Bernie going to a Trump town and sat down with a few of lovcals and just talked to them person to person about Corporate greed they’re facing and what he’s trying to do about it. After, one of the women ended up going to all his rallies and bought a T-shirt.

I’m not usually somebody that cries over movies and things like that but this really did make me cry. As someone from the New York tri-state area I moved to Texas for a few years and got to really understand how different we are based on such wildly different environments which then shapes who we are as a person. This lesson later became very essential as I began my career as a mental health therapist. To see this woman change her mind just made me think of how many more people are out there who were brainwashed by Fox News and the like because they didn’t grow up with a basic education that taught them to question things or were around environments of group think. It was just really beautiful to see somebody change in such a different direction. It really just touched my heart. it gave me a different view of Trump supporters who are probably not as well informed not because of their own doing, but what kind of media and social environment dominates them

Anyway just wanted to share some feelings lol

Supervisor called me quiet at our meeting (thanks socially restricting APs) Any advice on social skill development appreciated<3

This is literallyyy why I have resentment and yet my mom says "the past is in the past." NO. This shit literally impacts me TODAY as an ADULT. I have a social PHOBIA at the highest severity levels. (I'm a therapist myself so I know that I'm on a severe level.)

If anyone else learned how to develop normal social skills would appreciate any advice or resources you have<3

Care everywhere on Epic - do we need an authorization?

I had different opinions from my supervisors about this so I’m so confused. The official FAQ for mental health professionals from HIPAA says no authorization to disclosure PHI between providers for treatment. Online the forms for authorization from other hospitals (not ours) says the same. They only have to opt out but automatically the records are available. One supervisor agreed with this and another said to get authorizarion.

Just wanted to ask if anyone here knew the answer

Is it reasonable to ask for ongoing consultation in a new setting?

This is shameful to admit so I hope the people who respond are kind.

I'm fully licensed and am working in a CMH for adults for the first time (work hx includes kids and private practice). And its been quite overwhelming as I'm so new to all the severe & persistent disorders and resistance and documentation so I spend a lot of time outside of work doing readings and training to catch up. Its been helpful but nothing that would replace supervision to really process whats going on. 3 months into the job, my coworker who started with me said our direct supervisor told her he was supposed to meet with us weekly for an hour for 6 months. But at this 3 month mark, he only did 30 minutes weekly with me for the last 3 months which hasn't been enough.

I think I could use some quick consultation times set aside with another coworker (supervision with our actual supervisors make it so hard to be honest) whose been here longer. I struggle with Social Pragmatic Disorder so I just wanted to ask everyone here if its socially acceptable / reasonable to ask my new supervisor for structured consultations with a coworker?

Women: DAE have a dad who acts like she's his girlfriend?

FML sorry the title should've been who acts like your his girlfriend. Promise I'm not a fob lmao!

I'm seeing some weird dynamics with my uncle and cousin that me and my sister both independently labeled as him acting like she's his girlfriend. Even my cousin's younger sister commented that he treats her like his gf. He goes crazy when she doesn't speak to him (rightly so after his many incidents of domestic violence), loses his mind that when she gets married to her current fiance that she won't have time for him, jealousy when she speaks to other boys. Sorry its a slow morning for me and I can't recall other incidents at the moment to further elaborate. My sister, her sister and I have all independently labeled his behavior as a bf-gf relationship (in his head, but not my cousins). And she's giving half her nursing salary to him and worked since she was 18 to help with the house so idk if that fear of losing their paycheck plays a role either. Regardless I haven't seen this dynamic before among other cousins or heard about it from other people in the community so I was just curious how prevalent this is and if this is indicating something darker. I did ask if there was any sexual abuse through text but was left unread. Just curious to know if others have heard or experienced this and if it ended up that there was something nefarious going on.

Assessing for decision making capacity?

LCSW here. I trust PhDs so I wanted to ask here. My local APS apparently only goes out if someone is delirious level but I'm uncertain about a ct who is not that level but I just want to rule it out. Any recommendations for assessments regarding the ability for decision making capacity?

Distraught we can’t trust our supervisors for clinical guidance

I cannot believe the disregard for what’s clinically appropriate. They’re unwilling to discharge and tell us to keep people on our caseload and yet they continue to give each of us 2-3 new clients a week. When complicated situations come up, it leaves us stuck on what to do because they don’t follow the ethics and instead are guided by what will feed our for profit hospital. I skip the fucking hospital policy and go by what I’ve learned from malpractice trainings and my code of ethics and case studies on situations.. I’m glad at least we have each other. but it still just adds a sense of stress doing this on our own without being able to trust our supervisors

"I know what not to say so I don't get hospitalized"

I live in a state where a client's hospitalization is determined by a screener. Two clients thus far said to me that they know what to say and what not to say for hospitalization. I know I can call the screener ahead of time and let them know that and who knows if they'll take that into consideration but if anyone has any advice let me know.

Brown spot on my scarlet bee balm?

Would appreciate any insight into what this is! It’s in its second year of life and the first time I’m ever seeing this spot. I’ve been inconsistently using the BONIDE Jack copper fungicide spray. It gets about maybe 4 to 6 hours of sunlight and is water daily. it’s grown in a mix of miracle grow potting soil, and the local natural soil.

pseudopsychosis secondary to borderline personality disorder ?

Is anyone familiar with what this means? I can’t tell if they’re trying to insinuate something about a client w psychotic disorder or if this is the referencing the transient paranoia symptom. Because if it’s the symptom of bod why not just say that?

Tips for CMH clients that get mad at suicide / substance abuse / etc assessment?

Despite me explaining the reasons (hospital mandate, insurance mandate, safety concern) they are still upset with me and go off ranting about how they're upset and not cooperate. How do I get them to buy in especially those who've been hospitalized before?

Suicide malpractice case study from HSPO: Therapist expected to get consent to coordinate with psychiatry even if client initially refused, so follow-up/referral support could have been possible if client later changed her mind.

I'm so confused about what these defense experts are saying what is expected here from this suicide malpractice lawsuit case study from HPSO (bless the fuck out of them for doing this). Can someone explain:

"Although the client stated that she did not want to see a psychiatrist, defense experts opined that the LPCC should have obtained the client’s consent to collaborate with the psychiatrist so that he could have followed-up if the client changed her mind regarding the referral. The experts noted that the LPCC may have been able to assist the client in obtaining an appointment had he obtained the consent."

(Its in the 4th paragraph down from this case study: https://www.hpso.com/Resources/Legal-and-Ethical-Issues/Counselor-Case-Study-Failure-to-perform-a-suicide-risk-assessment)

Uncessary info: I've been deep diving the past two weekends about suicide malpractice and everywhere cites "standard of care" yet they don't cite a single guideline that is standard of care. So this is all wishy washy abstract and decided by these random experts? so where did THEY get their so called standard of care? This is so vague it puts us in such a treacherous terrority if there's no cite-able standard of care.

Ongoing sexual abuse with adult client that makes a cycle of abuse -> stress -> si?

cluster b adult client with on going sexual abuse, unwilling to report due to living circumstances. problem is that it worsens SI and theres multiple si hospitalizations.

tried finding some guidance on ongoing SA but surprisingly i'm not finding anything other than past hx.

would appreciate any advice 😞

Edit for context because they have had allllll the supports before multiple times but won’t budge: previous therapist is a coworker because we’re in CMH. My coworker also affirmed when I consulted that there’s something off about the veracity of this. Because it just feels so off how casually and vaguely they talk about it (journals they write me write about it it like a book) they have housing set up from DV shelter but won’t stay there. First it was fear of crime because the family puts it into their head (which is huge they’re easily influenced by others) and then recently it’s about not being self efficient. Perp is family member who doesn’t live in the home but visits here and there. Ct won’t report because they’re concerned due to perp threatening to harm if reported and family she lives with will get in trouble due to allowing it to happen.
