How to Apply for Medicare Part B (Medical Insurance) During Your Special Enrollment Period

My employment will be ending in June. I want to keep my group coverage through
12/31/2026. I asked my employer about filling out CMS-L564.. Medicare Request for Employment Information. They said they will not fill it out before my coverage terminates. I thought I read somewhere that there is a way to do this so there is no break in coverage. I called Medicare and they said ask them to terminate it in advance with the 12/31/2026 date. Still, the form asks, "Did the coverage end", not will it end?

How can I get my group coverage to term 12/31/2026 and Part B to start 01/01/2027?
