7th Letrozole cycle — when do you consider IVF with PCOS?

Hi everyone! Looking for perspectives from others with PCOS who have been through multiple ovulation-induction cycles.
A little background: i’m 33f and my partner is 36m.
I have fairly classic PCOS, mainly high AMH and mild facial hair, without other major symptoms. I’ve also lost weight and feel much healthier now. I’ve done 7 Letrozole cycles; the 2nd resulted in a chemical pregnancy. After my 6th unsuccessful cycle, my doctor gave me a 2-month break with medication to calm my ovaries, and my AMH came down to 6.1.
This 7th cycle was after the break, with Letrozole + two Humog injections. I developed two mature follicles, ovulated without trigger, and had a 7.4 mm trilaminar lining. It was planned as an IUI cycle, but I believe I ovulated before the planned IUI, so we decided to try naturally this cycle and consider IUI next time. There is also a mild male-factor component (DFI ~24%). My HSG showed open tubes.
I’m currently in the two-week wait and, as expected, overthinking everything because I don’t have any symptoms! 😅
My main questions are:
Does lowering AMH/“calming” the ovaries actually improve pregnancy chances in PCOS, or is it mainly about improving the response to stimulation?
After 7 Letrozole cycles, would you start seriously considering IVF, or would you try a few IUI cycles first?
I’m not questioning my doctor’s protocol — she’s monitoring everything closely. I’m just trying to understand what others’ experiences have been and when they decided to move from Letrozole → IUI → IVF.

Obgyn vs RE

I am based out of India and my pcos treatment etc is being handled by ny gynaecologist.
However I see a lot of posts in different sub reddits where people often move from an OB to RE for letrozole cycles etc.

What I know is that if letrozole doesnt work my OB would refer me to a IVF specialist.
Is this different from consulting an RE? Am I missing anything?

Since my AMH is high, my doctor has started me on Diane 35 (kind of a hormonal pill) for two month to ‘calm’ the ovaries and control the AMH and ovarian volume. She’s then continue with IUI for 3 cycles max and then would want me to move onto IVF.

My husband has mild MFI so my doctor thinks that would also help for IUI.

What I want to understand is if using hormonal pill for two months is the right approach? Or would it really help bringing the AMH better and meanwhile i continue with supplements for overall health? Am I just wasting two months by going on the pill?
Please please help 🙏

First IUI - need advise

I am 33 and partner is 36, we have been trying since past 8 months now medicated monitored cycles with letrozole and past three with HMG injections too. I have had one chemical last November but nothing since then sadly.
Recently got my partner’s semen report and it has 24%DNA Frag with 1% morphology and hence doctor naturally suggested IUI.

I guess I need some advise on how many days prior to IUI were you asked to abstain? Since for DNA Frag normally ejaculating every other day or daily helps but at the same time I want to maintain the protocol of abstaining as well.

Anyone in a similar situation?

Husband (36) and me (33) have been trying medicated cycles for 7 months now with no luck - had one chemical pregnancy.
The semen test shows the below data -

Sperm Concentration- 51mill/ml
Total count - 188.7Million
Total Motility - 65%

a. Progressive 40%

6-7/HPF (elevated)

I have been on medicated cycles since October with no luck yet. My question is on AMH value.. I have always had high AMH and back in October it was 11 ng/ml and today I I got back my amh result and it was 9 ng/ml.

Is the drop something I should be concerned about? Is this common because of medicated cycles letrozole + menopur injections?

I have had PCOS since forever so I always knew it was going to be difficult to conceive.
This would be my fourth cycle with HMG injections and letroz together. Past two cycles have been textbook perfect with two good follicles and good lining but not sure whats going wrong. I have had a chemical on my second letroz cycle back in November’2025.
I am already taking coq, folic acid, estrabet, progesterone post ovulation, glycomet/metformin etc.
HSG done last cycle and confirmed no blockage (just longer fallopian tube which isnt a problem as per my doctor). Sperm analysis done and no issues.

For reference I am 33 and my husband is 36.

Is there anything am missing or more that I should ask my doctor for? Any help or anything is truly appreciated 🙏
