Sexual health as a bottom in Tanzania

Hi, so I'm a sexually active 18-year-old bottom in Dar es Salaam, and I have some health concerns. The first time that I bottomed, it was fun and went relatively well. However, I think I developed something afterwards. I decided to get checked, but lie about the cause. I was told I had an anal fissure (even though my doctor didn't even check to verify, he just concluded based on the symptoms I listed), and he prescribed me laxatives and painkillers to facilitate smooth stool for the tear to heal. However, after the prescribed amount of time, I stopped taking my medicine, and noticed the pain reduced, but wasn't completely gone. Thus, I decided to hop back on Grindr and hooked up with this guy I'm currently seeing. Anyway, mid-sex, as I tried to fit it in, I felt an intense, sharp pain, and we immediately stopped and pivoted to other forms of intimacy. Now, while at the time, I didn't think it was that deep, I've been noticing that my anus feels worse than before, and there is, in fact, blood when I wipe now. I asked Gemini, and it said that bottoming with an anal fissure seriously exacerbates the situation and can cause a chronic issue. I'm panicking and need some reassurance. Any progressive medical experts on here who can help me?? Does anyone know a doctor I can safely consult for help? Anything would be of great help!

am a graphics designer and am working on this industry and i get paid decent amount, so i always thinking about starting a youtube channel but my problem when i put effort into something and didnt get instant results - i tend to give up
