What do I need to do to file passport card reissue?

I never received my passport card. I called and this was the emailed steps I got:

To request a reissue at no cost, submit the following by mail within 120 days from the date the passport was issued:

  1. Form DS-86, which can be found and completed online at https://travel.state.gov/content/travel/en/passports/how-apply/forms.html or at an Acceptance Facility.

  2. Photocopy of the front and back of the photo identification.

I just want to clarify what photo identification are they asking for? The photo I sent with my original application is with them. All I got back from them is my old passport, new passport, and supporting documents for name change. Should I photo copy my new passport? I want to make sure they don’t disable my new passport, I just want to recieve the passport card I paid for. I am US based in California.

Passport renewal timeline question

So I applied for passport renewal and passport card (never had passport card before). I did this by mail (had to fill out a name change/correction form)

They stated they received the application April 7, I ended up getting my new passport maybe 1-2 weeks ago, can’t remember exact date. today I got my old passport and supporting document back.

But I still haven’t received my new passport card. Is this normal? I had a call with them today and they confirmed to me that the new passport card was shipped out the same day as my new passport (but separately) on April 23.

They emailed me step by step process to retry the passport card process. But they said if I do that, then the original passport card that was sent out would be invalid. Should I wait it out longer to see if it turns up? Or do you think it’s just got lost in the mail?
