Watery output causing leaks and “chemical burn” skin irritation around colostomy stoma

Hello, my mom is 7 weeks post-op with a colostomy. Our main problem is severe pain from the stoma output irritating the skin, which looks like a chemical burn. The skin around the stoma is very red in a circular pattern where the output contacts the skin, and it is extremely painful during cleaning and bag changes, making care very difficult.

Her output is very watery with some small soft formed bits, which seems to be causing frequent leakage issues. Sometimes after movement or changes in position, the wafer develops gaps and stops sticking properly, leading to leakage onto the skin again. At most, the wafer only lasts around 1 to 1.5 days before failing.

The stoma is located on the upper abdomen, so we are not fully sure but it may be from the ascending colon area.

We’ve already tried different ostomy bag brands including Partner’s, MC, and Convatec, but we’re still struggling with both leakage and skin irritation.

For care, we are currently using colostomy powder and spray with the crusting technique. We do not use a barrier ring yet. We were advised to use Leukoplast around the wafer for sealing, but removing it is very painful for her and seems to further irritate the skin. We’ve also tried an ostomy adhesive remover, but she says it stings when applied.

We also have calamine lotion with zinc oxide, but we have not tried using it around the stoma since we previously only used it for bedsores and are unsure if it is safe or appropriate for peristomal skin.

She is also experiencing body pain (lower back, hips, buttocks, and around the stoma area), which seems related to limited movement and discomfort from leakage and skin pain, since movement often worsens leakage and makes the skin pain worse.

We are from the Philippines, so some ostomy supplies or accessories may be limited or hard to access locally.

Has anyone experienced severe peristomal skin pain and irritation from watery output and frequent leaks? Any advice on protecting the skin, improving the seal, or reducing the burning pain would really help. TYIA!
