How do you handle blurred vision?

I was recently diagnosed with a migraine with aura. Besides my attacks which could last 1 week, now they happen rarely, on my normal days I have really bad vision in my eyes (I think it is not connected to my migraines), sometimes it affects only right eye, I can feel kind of small visual vibrations especially when I look at my floor which is white with a lot of black small figures. Sometimes when I look them they are kind of “moving”, I have kind of short gray small sticks falling, and gray spots. I found out it is called floaters. They are daily. But blurred vision sometimes is very bad, sometimes not too bad, sometimes I can see ok, but now it’s almost daily very bad. My oftalmologist told everything is okay, it is not an eye problem but the exam was made when the symptoms were not so strong.
Sometimes my vision get back to normal but still with those gray sticks. And I have very visible red vain on my right eye which never disappears even though I used drops for antiinflamations, dry eyes.

And most of day I have those symptoms I see really bad. I feel myself like in a vacuum. It’s difficult to go outside because faces of people are blurred.
My vision was perfect and now I cannot see what is written on the screen 2 meters in from of me. And this sensation sometimes is coming together with nasal pressure (I have chronic rinitis but it is not active now, probably I feen nervous pressure in my sinuses). Sometimes I can suddenly see super bad in my right eye after feeling heart pain. Cardiograma is ok, I tried holter 24 h but for sure on that day of holder I felt fine.
Sometimes my right eye is pulsating. It started to happen 1 years ago but episodically. Now it is worsen and daily. Also I cannot be on the light a lot.

I’m really tired of all these doctors and tests. I’ve spent all my money and still don’t have a clear diagnosis. I feel so bad, like all my energy has been drained out of me. I’m trying not to give up and stay positive, but I really need the opinion of someone who has gone through something similar.

Briefly, my story:

I’m 27, woman. My weight is normal but I can gain weight easy genetically, I keep myself fit with intermittent fasting 16:8 for 2 years (before skipped breakfast, since last weeks I do 14:10). I have polycystic ovary syndrome (PCOS). Two years ago, I took oral contraceptive pills for about one year and smoked. Now I don’t take them and don’t smoke.

In 2018–2019, twice I had episodes in public transport when suddenly everything went black, my ears felt blocked, I felt nauseous, and I almost lost consciousness.

In 2023, a few times I suddenly felt dizziness, cold sweat, and noticed enlarged veins on my hands when I was walking in a rush.

In 2024–2025 (for about 5 months), I started feeling this more often. I also had pressure in the nasal area, blurred vision, sometimes heart pain, headaches, difficulty breathing, and constant watery nasal discharge. Doctors told me it was chronic rhinitis that was active at that time. It was treated with steroid nasal spray and then steroid pills. I also have a deviated nasal septum and enlarged turbinates.

But even after that, for about a year the dizziness, blurred vision, and weakness did not completely disappear. They were not daily though. No headaches.

I always thought it was my rhinitis again, even though the sinus scan and ENT tests were okay. Vestibular testing was also normal. I still used steroid nasal spray because my ENT recommended it.

Separately, I sometimes had suddenly enlarged veins on my arms and heart pain, but ECGs were always normal. Maybe it would be better to do a Holter monitor test.

Sometimes I also had a light tic in public when someone moved suddenly in front of me.

I was very concerned about my vision. Sometimes it became blurry and I couldn’t focus properly, like being in a vacuum, for about a week, and then it got better again for a few days.

The ophthalmologist said everything looked normal and that I should look for a neurological cause.

At the end of 2025, one night I had a sensation like an earthquake in my head, and the next morning I forgot some words, couldn’t type normally, and had difficulty connecting words while speaking. It lasted about 20 minutes. I guess it was a migraine. Then it passed.

I still had blurred vision almost daily, sometimes together with pressure in the nasal area, blocked ears, and prolonged ringing in my ears.

Sometimes it also became more difficult for me to speak and I would pause in the middle of sentences.

Two months ago, I had an attack that lasted 6 days. Everything started with enlarged veins on my arms and heart pain, then numbness (I could still move) on the right side of my face, right arm, and right leg. My blurred vision became much worse, especially in the right eye, which also felt pulsating. My right ear felt blocked. I had some difficulty speaking, weakness, and sensitivity to light.

The same thing happened again one month ago and lasted for 5 days. I thought it was a stroke, but CT, MRI, and blood tests for thrombosis were all normal.

I visited a neurologist with all these test results. He told me it was migraine with aura, that no treatment was needed, and that I should go to therapy. He didn’t care and he was young so he didn’t have enough experience.

Then I found a neurologist specialized in migraines. I paid a lot of money, but he didn’t even look at my MRI (I told it was described as okay) he only read the history of symptoms I had written down. He told me it is possible that this is migraine with aura and prescribed Amitriptyline 10 mg.

I asked him if this could be IIH. He said it was possible and told me to see how I feel after Amitriptyline.

Right now I have daily blurred vision and difficulty focusing visually. I have headaches a few times per day (before, despite all these symptoms, I rarely had headaches). My ears feel blocked and I have prolonged ringing in them. I have pressure in my nasal area. I feel weak and low on energy.

I worry a lot about my vision. My MRI is normal and my sinuses are clear. The ophthalmologist said my retina is normal. I do not have epilepsy. I have low ferritin and low blood pressure when I feel really bad. I don’t want to start taking Amitriptyline before I am sure this is not IIH.

Please, if anyone has experienced something similar, let me know what you think and what you would recommend.

Should I visit a neuro-ophthalmologist?

And should I ask for a lumbar puncture even if the neuro-ophthalmologist does not see anything abnormal in my optic nerves?

Do you think I have migraines and IIH?

Is this right not to take Amitriptyline now?

Just any opinion and advice would be greatly appreciated. Thank you very much.
