People who could not do passive tongue suction at first -- how did you manage to overcome your issues and learn to passively rest your tongue on the roof of your mouth?

Hi friends

I am trying to get my tongue passively stick to the roof of my mouth with suction hold but I am not managing it for some reason. I just can't achieve any vacuum hold. I try to follow the standard instructions but it is not working for me. Not sure why. I don't seem to have a tongue tie and the roof of my mouth looks fine.

Anyone here who struggled at first but managed to learn it somehow? What was your experience? What did you do to get your tongue sit passively on the roof of your mouth?

Puffy cheeks with nasal mask and tape -- can it cause micro-arousals?

Hi friends!

I am using BiPAP which improved my breathing a lot. I am using a nasal mask because it is more comfortable and it gives me less leaks. I do have to tape my mouth shut to make sure air does not escape my mouth but nonetheless sometimes while I am awake the air leaks into my mouth and gives me puffy cheeks (but it does not escape my mouth thanks to the tape).

I image the same thing might be happening while I am asleep. I was wondering, could it be a problem and disturb sleep / cause micro-arousals?

My current settings seem to take care of my breathing waveforms very nicely. In fact, my breathing has never looked better. However, I always get some instability which look like micro-arousals even though there are no signs of flow limitations preceding such episodes (see the screenshot). I don't think it is due to a transition to REM because the chaotic part lasts around 5 min or less and then gets back to normal. I am just trying to think about all different reasons for that and puffy cheeks is a thing that I thought about now cause I really don't see any other reasons why I would get chaotic breathing every now and then for no reason.

Would be curious to know wha you guys think <3

Does mask have impact on the effectiveness of BiPAP therapy?

Is mask just about personal preference / comfort or can it actually impact the effectives of BiPAP?

I am not talking about leaks and comfort and whatnot. I mean actual effectiveness of BiPAP -- hey way pressure is applied, the way the waveforms are impacted, etc etc...

Just to give you a specific example, I reached quite a quick pressure support (8) and now the problem is that the exhale is very sharp and quick -- e.g. on the flow rate chart the inhale is nice and smooth and goes up to 30, but the exhale is very sharp and quickly drops to -75. Could changing my mask help here?

[AirBreak] Is there a custom ResMed BiPAP SW which allows to have fixed rise time AND EasyBreathe on at the same time?

Hi friends!

I love Easy Breathe on ResMed BiPAP because it makes breathing feel very natural. However, the issue for people with UARS is that Easy Breathe 'learns' what rise time to apply from previous breaths. This is a problem because if previous breaths are abnormal (i.e. flow limited), EB learns from those abnormal breaths making the situation worse. The solution would be to use a fixed rise time.

But again, I love Easy Breathe because it applies pressure in a nice natural sinusoidal shape, helping to inhale and exhale very naturally. And fixed rise time, especially with high pressure support, feels just awful.

I thought it would be wonderful to have Easy Breathe AND fixed rise time together. I know there is a bunch of custom SW disused on this sub but I could not find anything that deals with my question specifically.

Would appreciate if you guys could point me in the right direction or let me know if such SW is possible at all.

SARPE vs MARPE -- why SARPE is a bad option?

I am considering doing MARPE to help with my chronic nasal congestion. From what I read, people are much more in favor of MARPE and say that SARPE is a waste of time when it comes to nasal congestion specifically. How come? Why MARPE is better?

Got CBCT but don't know what to look for -- can anyone take a look?

Hi friends,

Can you guys take a look at my CBCT? Don't know to look for really? But I would love to hear if you guys think there is any confirmation that my airways are narrow or any other useful info one could get from a CBCT.

Now, I got this CBCT as part of a consultation with an orthodontist. I am thinking of doing a palatal expansion with MARPE. So I would really appreciate if you guys could comment on it as well and share if you think MARPE is warranted given the scans I got -- like, if there is any palatal narrowing, nasal cavity volume reduction, or anything like that.

Also, I heard that SARPE is a waste of time. How come? I would think that SARPE would be even better cause they slice your face right where it should be sliced and then expand it and let it heal.

EDIT 1: considering MARPE because my nose is chronically congested and no treatment seems to work. ENT says everything looks fine in my nose but I always feel some sort of resistance. I am 27 yo. , and I know it is older than ideal for MARPE but at this point I am ready to try anything really.

Sooo... DV-2027... Is it still happening?

All processes, procedures, tech upgrades, rulings, etc -- everything seems to be in place. But it is late July already and nothing seems to be moving...

Anyone heard anything? What's going on here?

Also, I'm just very curious. DV is such a huge thing and there are some many people involved. How come none of them leaked some insider info about it? I mean, just tell us already, is it or is it not happening this year / ever, c'mon now.

Posterior Nasal Neurectomy (PNN) -- yay or nay?

Is Posterior Nasal Neurectomy (PNN) an effective procedure? Anyone tried it and found it helpful?

About me: chronic nasal congestion, most probably NAR. Tried everything from steroids to antihistamines to intermittent fasting, with no / negligible effect on the congestion. Afrin helps but it can't be used long term.

Could it be that you do not need to optimize your PAP till you eliminate 100% of your flow limitations?

You suffered from UARS for years -- you had lots of flow limitations and even apneas, and overall it looked terrible. Now you got your xPAP and you managed to get rid of 100% of gross obstructions and flow limitations and 50% more subtle ones. Could it be the case that you do not need to chase another 50% and instead focus on optimizing your sleep to re-learn how to sleep normally again? -- like sleep hygiene, sleep restriction therapy, general health, etc

Diamox for high loop gain ?

Has anyone used Diamox (Acetazolamide) to treat high loop gain / TECSAs? Perhaps as a temporary solution to adjust to the machine?

Additionally, some recent studies suggest that similar drugs might be effective at stabilizing sleep breathing overall and reducing AHI.

Anyone experimented? What are your thoughts?

Switched from ASV to BiPAP -- does my data look fine to you?

Hi Friends!

I recently switched from ASV to BiPAP and to me the data looks much better but I would be very grateful if you guys could take a look and share an opinion / advice.

Here is what makes me think BiPAP is producing better outcomes:

  • Breaths are much more textbook perfect and uniform overall (see the screenshot)
  • More uniformity overall, i.e. less variability in amplitude of the breaths throughout the night
  • Fewer and weaker messy periods
  • Fewer breaths that look flow limited

Currently I am still adjusting to BiPAP -- still have lots of central apneas vs ASV, but they don't seem to disrupt my sleep -- when they happen, I just resume breathing normally rather than gasping for air.

My current settings are:

  • EPAP 8
  • IPAP 13
  • Trigger med
  • Cycle very low
  • Rise time 500ms

I definitely don't feel worse after switching. But I am not quite sure if I am feeling better. I feel sort of sleepy through the day but at the same time at don't feel like I was run over by a truck, which would usually happen on bad nights on ASV.

I guess the key question I have now is if you think my current settings are adequate and if all my flow limitations are gone. To me, it seems to be the case and it looks like now I just need to wait till my centrals will be gone. I read it might take around a month or so.

Would be very curious to know what you guys think.
