Update on the whole “sleep study showed 327 arousals” thing. Doctor went “huh, neat” and told me to try cbti again

Got it in front of the sleep doctor. 50 spontaneous arousals an hour, mild sleep apnea. Taking mirtazapine/seroquel/temazepam/melatonin just to fall asleep.

N3 core sleep showed nearly zero and the histogram showed me flickering into and out of sleep.

I showed all this to her and she was like “neat. we don’t do cpap for mild apnea. you slept for 6 hours you’re fine try cbti again lololololol”

I feel like ripping my hair out.

Is anyone else at the end of medical science for insomnia?

I have taken every medication in every dosage for every time frame. I have had every test, seen multiple psychiatrists, tried exercise, cbti, had a sleep study, and done TMS. The only thing left for me is ECT which I can't do because I need to work. Either that or ketamine or psilocybin assisted therapy or something.

Currently taking 15mg temazepam 200mg seroquel 15mg mirtazapine 5mg melatonin. Reducing any of those by a single milligram takes me from 8 hours of sleep to absolutely zero. There is no coming back from this dosage.

What do I do? I've just been going in circles for 10 years I need something that actually works and handles the root of this forever.

I'm in love with a cis man

And that man is my boyfriend! I just have to write about him because I'm not getting my himb time cause he's out with his homies for a birthday party.

We've been together for a few years now, and he's just the best. He's sooooooooo handsome. He has long blond hair and blue eyes and a beard, he looks like a dashing paladin come to rescue me just like out of a fantasy book!

Okay then take that paladin image you have in your head and make him into a marketable plushie and that's what he's actually like. He's so squeezable and huggable and cuddleable. He has a soft smooth voice and he's shy in groups of people.

I love how he holds me. He has this subtle strength and I feel so safe when he wraps me up in his gorgeous arms. His left arm is sleeved all the way to the shoulder in a mashup of all of his favorite intellectual properties and his right arm has 3 or 4 different pieces on it.

He's so good to me too! He fawns over me and calls me pretty and buys me flowers. He is a golden retriever boyfriend for sure. He doesn't see me as any different from any other girl. I'm into women as well and I point out pretty ladies to him and he says I'm prettier than all of them. He's so supportive of my transition.

He even made a whole Vessel cosplay for me even though he absolutely detestes Sleep Token (it's a hot shirtless dude singing about beautiful women it's understandable, it's not for him)

We go downhill skateboarding together too and he can do some really impressive slides. We're working on getting a snowboard for him so we can go together and I can finally be better than him at something.

He's so optimistic and whimsical and relaxing and safe and I just love him so so much

Hit with the "It's impossible to forget how to sleep" again today (Seroquel 400mg, Mirtazapine 15mg, Temazepam 15mg)

I'm working with a new psychiatrist because my usual one schedules 3 months out and has stopped responding as quickly to my messages and calls for the nurse. I'm taking the meds in the title at this point and waking up after about 3 hours. I'm getting no REM or deep sleep at all.

He's big on being "holistic", and toxins, and 5g, yada yada. He's like "okay this is not working I want to start from scratch"

And I'm like "bet, but I've actually thought of that before. I would love to do that but I need to do it from a place of sleeping. If we just take all my meds away I will be awake for multiple weeks, as I have already tried this and my ability to sleep naturally never comes back"

And he's all like "you have to restart your circadian rhythm" and I'm like "I don't have one. It literally doesn't exist. I haven't in over 10 years felt sleepy without taking some sort of medication to initiate".

Which then gets me the whole "It's impossible to forget how to sleep" and he brings up sleep deprivation experiments from the war and how the people in the concentration camps would eventually fall asleep while walking and I'm like "well that process is broken for me and I don't know why".

He won't rx more Temazepam and I have tried to sleep without it and I literally can't. I need all 3 medications at once or sleep never initiates. So I'm going to be in big trouble when I run out of Temazepam. I asked him for a bridge rx that is a tablet instead of a capsule so I can taper rather than try to go from 15mg to zero all at once because I will literally never initiate sleep ever again if I don't. He refuses to rx any benzos

So, awesome. I have all these appointments and I'm no closer at all to figuring out why I need 3 medications to sleep.

Sleep apnea is negative. Estradiol, Testosterone, TSH, Ferritin, CBC, Iron Panel, WBC differential, Metabolic Panel, Lipid Panel, and MRI are all normal.

We just keep going in circles raising my seroquel and I don’t know what to do

I am trying to find the real route cause of my insomnia and I just keep getting hit with the “it is impossible for your brain to forget how to sleep” line, and then my psychiatrist starts talking about toxins and 5g and vitamin b.

Last night I took 200mg seroquel, 15mg mirtazapine, 15mg temazepam and things have deteriorated so much that this combo only gets me 3 hours. My sleeping heart rate is in the 70s, and after 3 hours, it spikes to 110 and I wake up. My watch thinks I am doing 3am cardio.

I am getting a quarter of the REM sleep I am supposed to be getting, usually under 30 minutes a night.

To get a normal amount of sleep I have to redose another 200mg of seroquel just to initiate sleep again. I have never in my adult life slept for even a single minute without having taken seroquel to do it. No naps, no dozing off, no passing out, nothing.

So now I’m back up to 400mg of seroquel (I am not schizophrenic or bipolar) just to make it through the night. Temazepam doesn’t work. I’ve tried every sleeping pill ever made. Only seroquel works and I don’t have much longer before I max it out. It’s barely working.

I’ve already tried TMS as well. No effect.

I can’t keep doing this. Sleep medicine has no idea what is happening to me. My sleep is verified fragmented and low quality. Going without meds makes it worse.

All labs are normal, thyroid normal, levels nornal, mri normal.

Please help me

So I’m supposed to get out of bed until I feel sleepy again, but I never feel sleepy without medication, so what am I supposed to do?

Currently awake on the couch after I took mirtazspine, temazepam, clonidine, propranolol, and seroquel and it only got me 4 hours.

As soon as I wake up it’s all over until my next dose. According to the “gold standard” (lol) I am evidently supposed to just get out of bed until “sleepiness returns” (lol) but that doesn’t happen unless I take more meds.

So what am I supposed to do? I don’t even feel drowsy when I initially go to sleep, I just go to sleep when I am too sedated to do anything else. I haven’t felt ” sleepy “ in over 10 years.

My doctor says I’m already on the strongest available medication and there is nothing more she can do for me. She is utterly useless. What do I do?

What's some autistic stuff I can do to help me sleep

I have the worst case of severe primary insomnia I have ever seen anyone ever have. I am taking 3 high dose sedatives that would mess up a normal person for at least 24 hours, and they're getting me under 4 hours a night. I have to take another dose that I'm not supposed to be taking just to get some sleep.

I can't sleep because my head sounds like if you play 5 youtube videos all at once, two of them music and three of them talking, on your car stereo and then turn it all the way up, and I have no way to turn the volume down.

I don't know if I am actually autistic but it would make sense and I have never found a way to quiet my head. It feels like someone has injected me with caffeine and adrenaline all the time.

I have reached the end of modern medicine and there are no more medical treatments for my condition. I have tried every medication ever made for sleep. When you see someone freaking out in a mental hospital in a movie and they give them an injection that knocks them out, that's what I'm on, and it barely even makes me tired. I am continuing to work it up because I think I am actually sick sick, but the scans haven't found anything yet. I am trying to approach it from every angle so I have to consider that I might be neurodiverse and need to do some weird neurodiverse thing for sleep.

I know yall are gonna hit me with the shower orange recommendations but that's already part of my bedtime routine.

Treating severe primary insomnia, discovered I have one copy 677CT and normal 1298AA. What dose of L methylfolate should I use?

Hi, I posted here a while ago about my severe unexplained primary insomnia.

I have the worst case of primary insomnia I have ever seen anyone have in my entire life. I was taking 200mg seroquel, 15mg mirtazipine, 0.2 mg clonidine, 5mg melatonin, and sometimes some benadryl and still only getting 3 hours.

I also have bad reactions to most psych meds, and the ones I tolerate well usually don't do anything. SSRIs mess me up and I can't take them. I tolerate mirtazipine great and it's been my staple. Lately I've been depressed for really good reasons outside of my medication so I don't think I am psychiatrically depressed anymore my life just regular sucks.

My doctors kept telling me try to take it easy, go to therapy, work on your anxiety, do sleep hygiene, blah blah blah.

And I keep telling them "I am an otherwise healthy 20 something who needs 4 sedatives to even initiate sleep at all, then I am waking up after 3 hours. It's not my sleep hygiene because I am initiating and then waking up. Something is wrong with me."

A sleep study was 2200$ so I decided to just go rogue with the whole thing, and went and got an MTHFR test.

It came back as having "one copy of C677T, and normal unmutated 1298A" which shouldn't explain the level of messed up I am, but I've tried every sleeping pill ever made and none of them work so I might as well start throwing random things at it.

I exchanged the seroquel for 30 mg temazepam (after years and years of begging. They would rather give me seroquel than temazepam because temazepam is controlled. So you give me the known scary and ultra dirty drug, seroquel instead? seriously???????)

and have been taking 15mg l methylfolate and sleeping like a log. I started both only a week or two ago so I don't know if the l methylfolate is doing anything but sleep wise I feel great. Big honkshooing.

Outside factors in my life are still absolute garbage but hopefully once I'm out of this awful situation I'm in I'll be big chillin. Maybe one day.

Anyway, what dosage of l methylfolate should I be taking? 15mg seems like a lot but I don't notice any side effects or anything. It seems like a lot for someone with just one copy of C677T though. I ordered a 1mg too so I might try that. What do you think?

Most meds do literally nothing. I have tried every medication ever made for insomnia. The stuff that the doctor and other insomniacs say "Oh yeah this will knock you into next week" doesn't even make me sleepy.

I have the strongest stack I have ever seen anyone taking for insomnia ever, and it's still not working. I'm begging for workups and MTHFR is on the list.

I have a really long history of psych meds either doing nothing at all or doing nothing at all and also messing me up. I've tried 35 meds at this point. The ones I'm on I'm not even on because I'm like "oh this one is great I feel normal again", it's a "well at least I take it and don't actively feel worse" situation.

Does this sound like this could be MTHFR?

It isn’t my sleep hygiene because I am successfully initiating sleep and then waking up. Once I wake up it’s over until I take more meds.

I have a sleep doctor and a psychiatrist and they say they can’t help me anymore. I’m begging for work ups because this doesn’t make sense but they just keep giving me metabolic panels that are coming back normal. I’m begging to see a gastro and a neurologist but they never put in the referral.

I am not otherwise mentally ill I’m fine when I sleep.

I’m fine with inpatient but went for a consult and got the vibe that they can’t help me

It’s been 10 years since I got sick. When I sleep I am very active and healthy. I’m not overweight

Sleep hygiene is a hoax

All the good vibes and ” keep a cool quiet room 😁😁🌸🌹” cannot fix that I have a fucking chemical imbalance.

It cannot fix that I am taking my meds and successfully initiating sleep, then just waking up after 3 hours anyway.

I have been tracking it too. I’ve been getting under 4 hours of sleep for two months.

I currently take the maximum dose of mirtazipine, seroquel, temazepam, clonidine, and melatonin.

*4. fucking. hours.*

getting a sleep study is 2200$ btw. I’M A TEACHER. I HAVE NEVER EVEN HAD 2200$ IN MY BANK ACCOUNT. i am begging for work ups but they just keep sending me to get a metabolic panel that comes back normal.

It snowed here today for potentially the last time of the season. I have had some stuff going on and haven't been vibing, and my therapist has been encouraging me to keep myself distracted from the things that are going on and engage in some whimsy when I can.

So I decide to smoke a bowl and go on a little midnight adventure, since I'm not going to get to vibe in the snow until next season.

There is a greenway path that passes right behind my backyard. I recently moved and have been thoroughly enjoying the path, but haven't been out there this late yet, and not yet in the snow.

The path follows a stream that gives life to an unusually thick cover of trees for this usually desolate place. In tandem they wander through adorable parks with open meadows, now fully green for the season, and pass under bridges to cut through busy thoroughfares, completely silent in this moment. Silent except for the sound of the swelling stream and the snow, which was melting from the trees and falling down to the path as rain. The overcast sky blended with the snow coverage to paint the world almost completely black and white.

I am, obviously, absolutely fucking vibing. I spent the entire evening listening to my favorite soundtrack to do vibey shit to: minimalist classical music (I had on music for 18 musicians), walking around taking pictures of anything that stood out to me. I was fascinated with standing under the bridges as the water rolled off the sides, and taking pictures of the dilapidated sheds in backyards on the other side of the stream. It was fucking delightful.

Eventually, I need to go home, and my clothes are now damp from the snow. I turned around, paused my music, and spent the walk home enjoying being surrounded by the soundscape of one of those nature sound videos you put on to sleep.

I was then presented with a random encounter. The sidewalk home takes me through a small clearing, where I found, because it is so late into the year, that the sprinklers were on. There are three or four of them. They were the type with only one linear water stream that rotates around in a circle, and their streams were all rotating through different places and crossing over one another.

I stop to ponder my next move. I don't want to get any wetter than I already am, but this is the only sidewalk through this clearing. I'm sat there trying to find a break in the patterns to sneak through like I'm in a fucking video game.

I turn around to look for a way around and not five feet from my face is a (completely normal) dude in a balaclava (because it was cold) and a black jacket (because it was cold), with his hands in his pockets (because it was cold).

I literally audibly jumped, and said, way too loud by accident because I was scared, "Ah! Jesus Christ, you scared the shit out of me!". This completely normal regular dude responded with "oh sorry sorry", and passed me to continue walking home. Cause he lives here.

I was so embarrassed! I said "oh my god I'm so sorry I had headphones on" but he was already walking away. I decided that honestly I deserved the sprinklers and just walked through them and back home where I am now writing this because wow! What an amazing night! I had so much fun! And I just had to tell someone about it.
