1+ Year Post-Orchi: Back to Normal Without TRT?

I'm curious to hear from men who are 1+ year out from an orchiectomy (with or without chemo, although please mention if you had chemo or not since this might have an impact, too) and feel physically completely back to normal without TRT.

I’m not just talking about testosterone being within the “normal” range on paper, because I’m not convinced that tells the whole story. Someone who had much higher levels before, then dropped significantly, or whose estradiol is relatively high compared to his T, may still be impacted by some hormonal changes.

So I’m more interested in the subjective experience: men who are 1+ year post-orchi (and/or chemo) who recovered and feel physically back to pre-surgery levels.

post-chemo RPLND and retrograde ejaculation

My partner had his post-chemo RPLND consultation yesterday in Belgium. We left feeling a lot more reassured, the surgeon is experienced and made a great impression. He’s planning to do it robotically. My main question is about nerve-sparing and retrograde ejaculation: I thought we’d know before surgery whether nerve-sparing was possible, but the surgeon said he won't know until he actually gets in there to see how "sticky" the tissue is from the chemo scar tissue. Regarding post-chemo RPLND, was retrograde ejaculation the case for you or not, and was yours robotic or open? I wonder if the approach makes an impact. I guess either way, there is not much he can influence at this point ..

RPLND questionaire to surgeon, anything important I've missed?

RPLND consultation - questions to ask

  1. How many post-chemotherapy RPLNDs for non-seminoma do you personally perform per year, and how many are complex cases?
  2. What is your personal complication rate for these operations - particularly major bleeding/transfusions, vascular or other major complications, and re-operations?
  3. Would you recommend open or robotic RPLND in my case, and why? If robotic, have you personally performed RPLND with the specific robotic system available at this clinic?
  4. Will you use a nerve-sparing technique, and what is the likelihood of preserving normal ejaculation in my particular case?
  5. What is your personal rate of long-term ejaculation problems after post-chemotherapy RPLND?
  6. Based on my specific case, what do you consider the biggest surgical risk or challenge?
  7. What should I expect regarding recovery - typical hospital stay, when I can walk normally, work, exercise and lift my child, and are there any dietary restrictions?
  8. What are the main complications or warning signs after surgery that would require me to contact the hospital urgently?

RPLND in Belgium

After finally feeling more like himself again, 5 weeks post 3 cycles of BEP, my partner received the news yesterday that he will need an RPLND. This is definitely not what we wanted to hear, though we knew there was a chance.

He did his BEP in Belgium at Jules Bordet, a hospital entirely specialized in cancer. His oncologist is only specialised in germ cell tumor, and we have been really happy with the treatment he received so far.

Months ago, his urologist mentioned that if an RPLND became necessary, he would refer my partner to a specific surgeon at Jules Bordet. His urologist explained he would not perform the surgery himself because it requires strict routine, and said this surgeon is highly experienced and also uses a robotic approach. So that sounded reassuring.
However I really would like to have a 2nd opinion. Not sure if it's possible to find someone here who went to Jules Bordet in Belgium, and where and how I could find more information about this surgeon and make sure he's the best choice in Belgium. There is another, hospital in Leuven, that is larger and was the first to use rotbotic RPLND approach in Belgium.

Stuck on Phone verification Loop

I am a Belgian resident trying to complete Step 2 (AdSense) for the YouTube Partner Program, but I am entirely stuck in a system loophole.

My legal residency is based in Belgium, AdSense locks my verification country prefix to +32. However, my primary Google Account 2-Step Verification is natively tied to my French roaming number (+33), I don't use a Belgian number.
Every time I try to manually clear the pre-filled French number in AdSense to input a valid, standard Belgian number of a friedn , the backend security system immediately flags it and fails with the error "We couldn't send a verification code to this number."

I contacted YouTube support manager, they said they cannot do manual overrides due to regulatory compliance.

Is this system block happening because the phone verification is directly cross-referencing my French 2FA/Google Authenticator setup, and has anyone found a workaround to bypass this cross-border loop without changing their primary account security?

Earn tab blank/greyed out after hitting 4k hours & restoring channel? Is this normal?

One of my videos recently took off, so I hit the required metrics pretty fast. Because the Earn tab always lags behind, it finally updated to over 4,000 watch hours about 2 days ago. But now, the "Apply Now" button is completely greyed out and it just says "We'll notify you when you're eligible."

I don't know if there is any relation, but a few days ago, I accidentally deleted my brand channels under my Google account while trying to clean things up, but I restored my main channel within an hour. I wanted my "try out" channels to not be seen while applying for monetisation, they had either no or almost no views anyway.
The numbers were still running normally after the restoration, so I hoped it didn't affect anything, but now the screen is blank.

Is this just a normal backend delay/sync glitch because of the restoration, or did I mess up my monetization eligibility? How long does it usually take to update? Thanks!

Article: Sperm mitochondria dysfunction in response to testicular cancer

Here is an interesting 2024 study highlighting how mitochondrial dysfunction damages sperm quality before a testicular cancer diagnosis.

A major takeaway from the paper is that sperm quality often dramatically improves 30 days after removing the cancerous testicle.  the authors suggest that if you have enough time before starting chemo or radiation, banking your sperm after surgery (post-orchiectomy) might give you samples with significantly fewer abnormalities.

It also makes me wonder if it is in any way possible to protect mitochondria function through exercise diet or suppelments, and if that could be protective in any way, but here I'm thinking out loud. Maybe there are more studies on that.

How likely is RPLND 2B Non-Seminoma?

I already posted here a similar topic, but I still feel a bit confused about it, so I'm trying to get more feedback : my boyfriend just finished 3x BEP for stage 2B non-seminoma and his blood markers are now clear. We don’t have the post-chemo CT yet.

His pathology was:

  • 70% embryonal carcinoma
  • 30% yolk sac
  • lymphovascular invasion biggest around 2.9 cm.

What confuses me is that his oncologist already said surgery/RPLND is “less likely,” but when I read Reddit it feels like most guys with stage 2B non-seminoma and nodes around 3 cm end up needing post-chemo RPLND.

Anyone here with similar pathology/spread who ended up NOT needing RPLND after BEP?
