Rules for what doesn't count:
Games: I have only played the games on newgronds and haven't seen anything from Madness Project Nexus.
Lost limbs: if a death separates or eviscerates one of their limbs completely it won't count. Scrapeface is an exception.
In hell: they're already dead, why should it count.
