Just this. Ive been a nurse for a little less than a year. Had maybe 6 months on a pcu, then moved across the country and got into a micu.

The other day I got off orientation two days early because of call outs. Cool, fine, whatever, close enough. Also tell me why my unit manager scheduled me 6 in a row as an off orientation gift.

My 6/6 night they already refused to let me have my patients back from the three nights prior, whatever. I dealt with it.

So on Tuesday night this week, by 0200 I hadn’t yet charted a thing other than my med admins and my labs. Because one of my patients was q2 hour labs for critically low sodium. We also spent nearly 2 hours trying to insert a foley in this gentleman. My other patient was vented and pegged.

At 0230 my charge tries telling me im getting tripled. I told her I did not feel safe taking on a third patient when I was already drowning. It turned into a whole blowout between my charge and I, with me threatening to invoke safe harbor bc I was NOT accepting a third patient.

Luckily a seasoned nurse on my unit went to bat for me three different times and on the third they managed to get it so id keep my original patients and not get an admit

I was pissed. I felt like they were setting me up for failure. We are critically understaffed but I don’t see how giving a fresh off orientee an unsafe ratio is going to do anything but create an unsafe environment.

Im now off until 7/23 and really dont want to go back. I love what im learning but im worried about what is going to be expected of me moving forward.

This. Im in Tucson for two more nights and want to see the little gremlins. Any suggestions for times / places??

Graduated last May, worked on a stepdown unit from september to feb, then started on an icu across the country on 4/13.

So I’m still quite green. I get that.

Im still on orientation, to be clear. We had a pt last night, glucose of 48. I called the provider, my preceptor prepped the d50, and provider said no, increase his bicarb drip from 75 to 125. My preceptor was surprised but did what the provider said. Let’s also be clear: I documented this in epic under critical results provider notified blah blah blah, that tab. With the time, what the provider ordered, etc. Also we passed this along to oncoming shift.

Over night, he goes to 26. And the night provider is pissed because he saw the previous reading of 48 and thought nothing was done. So I come in today and had about four different people come up to me asking what happened. When I explained, one specifically said “Oh yeah if they ask say that” as if I just made up some type of tall tale or something.

What im getting at, is how is it that they overlooked both my note and the fact that im still orienting? Nobody said boo to my preceptor about this. Just me.
