How can I return an item that I never received?

I was going over my "Returns & Orders" page on the regular Amazon website (upper right corner) to see what the status is for some of the items that I requested from Amazon Vine (ordered, arriving.., delivered..., etc.) and I noticed a status that said "Return received - We will process your refund in 2-3 days. Once your refund is processed, it can take 2-3 business days for the refund appear in your bank account or credit card statement."

I never returned the item since I never received it nor do I believe that they ever shipped it.

I have had items which were shipped and then after the expected delivery date when I didn't receive it, the status would say something like "if you don't receive the item within 5 days, please contact Amazon" and then later change to "returning to sender" but this item apparently never shipped to me, was not delayed in transit, and they labelled it as a return... which I would never do with an Amazon Vine product even if defective. For a defective product, I would just write a negative review on it and that's it.

Has anyone had this happen to them where they didn't submit an Amazon (Vine) return but Amazon claims that the item was returned?

Prescription for glasses

I went to my optometrist and after my usual eye exam, she mentioned that my eyes have changed and my current glasses should be changed (I agreed).

Like most optometrists, I'm sure a large percentage of their income comes from requesting/ordering the prescription glasses themselves.

I mentioned that I would get the glasses from them BUT I also wanted a prescription so that I could get a second pair elsewhere. I went through the standard questions about the frame/options and they put something over my eyes and told me to look forward (I'm assuming to measure the distance between my pupils). Everything was entered for what I requested from them so they put my frame, my requested options (like anti-scratch, anti-reflect, and transition) into a box/container and then the optometrist returned to me and give me my "prescription" for the glasses and told me that my glasses would them would be ready in under 2 weeks and they would call me. I paid for my eye exam and put a deposit for my glasses.

The prescription that they gave me seems like it is lacking a lot of information (since only 2 fields are actually filled in). Is this normal? Would I be able to actually get prescription glasses with only this information (I removed personal identifying information for myself and the optometrist but the optometrist is legitimate/licensed).

I feel that they purposely omitted information so that I wouldn't be able to get my glasses elsewhere.

Anyone who passed their evaluation recently and found that when posting reviews and reviews are more/less similar to how they did it in the past where they would get "excellent" for insightfulness, it is now "poor". My reviews are written in the same manner as prior to my re-evaluation date where they were all "excellent" so none of my reviews even for things as "simple" as a USB charging cables were 1 or 2 sentences like "It works and charges my device".

I reviewed 5 products after my re-revaluation and they are classified as "poor" even though when I click on the "Reviews" and then "Reviewed", it shows "pending".

Also, on a separate note, I'm not sure why my "Review-to-order" ratio is 71% since it should be 100% because I've reviewed all items that I've received but have orders that have either shipped (but not delivered yet) or in the ordered status so the "review-to-order" ratio is not as described by the informational button which is that it should only take into account those items were are delivered.

I discovered a mistake on one of my tax receipts which was generated/submitted to CRA (and RQ) by the financial institution. I can see the receipt on MyAccount on CRA's website (as well as MyAccount on RQ website). It was an RRSP receipt for a contribution that I didn't make.

This RRSP amount in no way causes me to have overcontributed and I tracked it to it being incorrectly logged as a contribution was actually a transfer from one RRSP account in the institution to a different RRSP account within the same institution.

While I did include the contribution on my tax return since CRA/RQ would have caught it as me omitting a tax receipt that they received from the institution, I did not claim the deduction for it. I contacted the institution and informed them of the mistake and the CSR over the phone checked my RRSP accounts with them and as I pointed out to them, there was no new money which went into my RRSP accounts which wasn't a result of a transfer from my other RRSP account.

Other than following up with the institution occasionally, what else can/should I do?

As mentioned, the tax receipt doesn't cause an "overcontribution" nor am I even using the $2k safety net/buffer provided by RRSP contribution overcontribution limit.

The way I see it, since I've early retired, even if the institution doesn't correct this mistake, it doesn't affect me since being retired, I'm not making any additional RRSP contributions so me "losing" the contribution room doesn't affect me and I'm melting down my registered retirement accounts while I have the chance before I start collecting CPP/QPP and OAS.
