What is expected of people who express their gratitude?

Sometimes we go out of our way and do something risky, laborious, or costly for someone that allows them to overcome the challenge they face. Acknowledging the rarity of the selflessness of our actions, they tell us that they owe a debt of gratitude to us. In my experience, this situation sets one up for disappointment as situations easily arise where we feel that the other person didn't go out of their way for us. So my question: what is expected of someone who had expressed gratitude for something we extended to them? Are they expected to give something back? To extend to us some latitude patience, tenderness, or preference that hey don't normally extend to others?

I heard this from a relative who obviously thought cleanliness, as a trait, was close to the "Frontline".
