Feedback tips for superficial cervical plexus?

i did this for an awake CEA but it didn’t fully work (got some numbness but he felt pain during incision). did it during pre op with 15 cc 0.5% bupi at C4. I marked the SCM.

i has continued to follow the local pocket until almost half way to the underbelly of the SCM m.

20 min later, loss of sensation to the blunt needle when I tested but couldn’t tolerate incision.

AITA for asking my co worker to pay my nanny’s fee to cover his shift?

My co worker wanted to leave town and in our line of work, we have to find coverage. He tried to send out an email to ask someone to cover his shift. no one replied (either no one available or no one wanted to work an extra shift).

I’m fortunately that I don’t actually “need” to work but I’m happy to help someone.

I texted him I could cover his shift if no one else offers - I said I did have a doctor’s appointment but I can reschedule.

Then I realized I had already sent our nanny her schedule for the week and she was only scheduled to work for half a day (for my doctor’s appointment). Added hours are extra. So within 5 min from the first message, I texted “oh shoot, I don’t have childcare in the afternoon. If you are willing to cover the nanny fee, I can cover your shift”.

Was I an AH for asking him to pay for my nanny fee or should I have just sucked it up? FYI, we are in a very high income job, nanny fee with 1.5x (OT rate) would have been about 200$. He said I was being petty and did pay it - bc it’s “chump change for us” - we make above 200/hr

LMA insertion tips?

Anesthesiologist here but recently switched to ASC. I never loved LMAs but I especially hate the LMA uniques (skin colored). for me, they are harder to place than just tubing someone.

i started at a place where all they have are uniques. first 6 were fine but my last patient looked like he would have a good airway, tall 6+, not obese. did have tonsils but not horribly big.

I deflated the LMA, tried to insert but it wouldn’t make the turn at the posterior pharynx, i tried inflating and inserting. then I tried inserting backwards to hope it get past the turn and turn it back 180 deg but nope. tried to size down to 4. nope.

i just tubed. I would like some tips to improve. overall and wap with unique because that’s all they have.

usually i do better with igel and supreme or air Q.

(intubation was super easy)

i thought i put on a flair but it still warns me

main goal is as a wedding guest. but hopefully can be used on nice dates with dear husband too.

Help me pick a dress - guest, unknown dress code

1 - green w white floral, size small (i’ll def need shapewear)

2 - same as #1 but black. size medium

3 - navy silk halter

4 - navy silk halter shawl

5 - black with pink flower halter

i know people said i need a bra, but i just hate wearing bras with dresses. I prefer halter tops and the straps show. also the band shows because I have back fat. maybe I need boob tape?

dress 1&6 are a bit small on me.

AITA for interrupting conversation for toddler safety

Real arguement tonight, here for judgement.
Husband (37M) and I (37F) and our two young kids were eating in the kitchen island (them on step stools).

my husband had a rough day at work and was telling me about it. our younger 18 month old daughter started to try and climb out, So husband moved toddler to the high chair at the dinning table (which is not visible range of island).

He strapped her seatbelt and put the table on with food then returned to the island to talk with me. I stopped him and asked him to return to our 18mo old to keep an eye on her.

I then gathered the food and brought my 4 year old’s food and my food also over to the dinning room table. After sitting down, I warned my 4 year old “we’re moving here so we can hear (vs yelling around the corner) your dad talk“ (both as a prompt for him to talk, giving him indication we followed for this, and to clue in so my 4 year old doesn’t start rambling nonstop).

at this point, he said he no longer wanted to share because he said it didn’t seem like I cared about his day or cared that much to listen to him and I should have said something like “oh sorry for interrupting earlier, please go on with your story”. he called me cold.

fyi, i have told him multiple (5-10?) times to not leave her unattended in high chair by herself. he says that’s not even an issue. there is “almost no risk”.

so was I too blunt and harsh?

I don’t think I broke rule 8 - as this is more about parenting views/communication. but we shall see

AIW for dinning and dashing because I didn't want to wait for the check?

I 35F am a doctor who takes call at a level 2 trauma hospital. This means I just have to be readily available within 30min, so I don't have to stay inside the hospital if I'm not needed. I went to eat pho that was just a short 5 min drive away.

Half way through my meal, the OR charge called me to let me know they have an emergency case that was just posted. I wolfed down the remaining pho while trying to flag the waitress down. After being unsuccessful twice, I left the card on the table as I finished eating. She still did not come, so I went to the front to pay. The person in front said they will flag my server down (which is weird bc most pho places will let you pay upfront).

She came over and said she will "get to me" after sitting the next guest. But she sat them down and was waiting for their order, then started talking to another server while waiting for them to look at their menu. I walked up to her and asked for the check because I have to go. She turned and stared at me... then said "you aren't special, you can sit back at your table, and I'll get to you when I get to you" (mind you, I did not ask for anything during my stay - was NOT a demanding customer).

So I went to my table, wrote down my number with a short msg "text me to arrange payment" and left.

Manager called me later, I told her the event. She gave me the meal for free. Waitress later texted me saying I was a giant AH because the manager was deducting it from her wages.

FYI:

- I think I spent about 10 min trying to pay
- I'll def pay up front before eating next time
