My father (50 years old, India) has been dealing with chronic lung issues since around July 2025.
Symptoms started with:
- mild cough
- occasional fever
- progressive weight loss
Over months, cough worsened and eventually he started coughing blood (hemoptysis), first mildly and then significantly around Dec 2025.
CT/HRCT around Dec 2025 showed :
Calcification, cavity, consolidation, bronchiectasis and fibrosis seen in right lower lobe, Likely Koch's infection. Scattered ground glass opacity noted in both lung.
Hemoptysis/bleeding timeline:
- First major blood episode: December 2025
- Second major recurrence: February 2026
- Third major recurrence: May 2026 (twice more than dec 2025)
Pattern observed:
- antibiotics temporarily improve symptoms
- bleeding stops for weeks/months
- then cough, weakness and mucus gradually return
- followed again by hemoptysis
Latest CT/HRCT showed (as of May 2026) :
Subpleural interstitial thickening with GGOs & multiple subpleural bullae in both lungs, predominantly in the lower lobes likely UIP pattern of ILD Cavitating consolidation in the lower lobe of right lung with few air space nodules noted in right middle & lower lobes, likely infective etiology.
Doctors initially suspected:
- TB/Koch’s infection
- chronic bacterial infection
- fungal infection
However:
- sputum tests negative
- NTEP tests negative
- bronchoscopy negative so far
- cultures initially negative
- doctor is telling no cancer sign
Bronchoscopy was done recently and we are still waiting for long-duration culture reports (30–50 day reports pending).
Important progression detail:
One thing that changed significantly in May 2026:
Before May 2026:
- my father had recurrent cough and hemoptysis episodes
- but he DID NOT have persistent fever or major fatigue
- even during previous bleeding episodes (Dec 2025 and Feb 2026), fever was not a major symptom
Since the major recurrence in May 2026:
- recurrent fever started appearing (101–102)
- severe weakness/fatigue developed
- dizziness increased
- appetite became poor
- cough became more persistent/dry with occasional thick mucus
Current oxygen saturation is around 97%.
Recent antibiotics:
- injection zostum (did not take due to unavaliability)
- levofloxacin
- doxycycline-type medication
- montek LC
- Econorm (because he is having diarrhea for ~2 weeks)
Doctors currently think there is still an underlying infection/inflammatory process that is not getting detected properly.
My questions:
Has anyone seen cavitary lung disease with repeatedly negative bronchoscopy/TB reports?
Could this still be:
- smear-negative TB,
- aspergillosis/fungal disease,
- post-TB cavitary disease,
- ILD-related inflammation,
- or something autoimmune?
How common is recurrent hemoptysis every few months in chronic cavitary disease?
Is persistent fever despite initial antibiotics concerning if cultures are still pending?
Would you consider:
- repeat HRCT,
- biopsy
What exactly are we treating right now - active infection, old lung damage, fungal disease or something else?
Financially we are exhausted already after bronchoscopy and repeated scans/tests, so we are trying to understand the most rational next step instead of blindly repeating procedures.
Any serious pulmonary insights would genuinely help.
