What is the going % law firms take if they win a denial on an LTD extension case?

I sent an application for my husband's LTD 5 year extension with Reliance Standard for a claim that ends 10/27/2026. The extension got denied not because of his medical condition but because of the criteria of being in a medical facility, receiving hospice or receiving home health. I used the reasoning that me being his caregiver 24/7 was far better than any of their 3 choices. Also, his medical condition won't get better with rehab which is the category for those 3 options. Anyway, it got denied but because it's ERISA an attorney has to take it without anything up front. I got only one acceptance out of about 6 law firms, all the others said they just couldn't give the time needed. I guess that's all you need is one. The firm that said they'd take it is in TX, that's all the info I'm giving. This is what they offered:

I'm beating myself up for not getting weekly home health. I just felt that someone coming to take his vitals once a week was stupid and abusive of his insurance. Would I be stupid to do this? Does anyone see anything fishy here? It's kinda like this offer or nothing.

How do people deal with a parent, almost 92, who’s always rude to people, especially certain children? This is my mother. She yells and when confronted about it, she claims that we’re not listening. She’s rude to nurses, anyone who in her line of sight.

Does anyone have success stories for MDD/GAD with acupuncture?

FYI-this not for me but for my husband. He’s been in a tailspin for a little over 2 years. Been on just about every psych med since the beginning of this time period. Been inpatient psych ward 5 times since January’25, 2 of those followed with IOP. Been transferred to a facility in Houston for “higher level of care” for ECT. Couldn’t pass the cardiac clearance and actually had OHS in May ‘26 and an ablation in June’26. It seems since that ablation, he’s gotten extremely worse. Screams out daily in frustration. Threatens suicide daily. He sleeps good with the help of meds. He eats and takes a shower because I encourage him. But for the past couple of months, all he does is lay in bed. When a panic attack hits him, that’s when he goes ballistic. Funny thing is, he takes Advil and it mellows him the same as a benzo. Last week he had Saint TMS therapy. Won’t know if that will help for 2-3 weeks. The next med his psychiatrist wants him to try is auvelity. He’s tapered Seroquel off almost all the way, on 10mg liquid. Has about a week left. Only other psych meds he’s taking are Pristiq 100mg & Ativan 1mg three times daily. He’s on metoprolol for his BP and I’ve read it can cause increased anxiety. He’s on his 4th psychiatrist and has a tele health appointment this week. Any advice is appreciated.

Are the patient’s family members supposed to feed paid caregivers along with their loved ones?

Is it possible to deposit a check made out to 2 other people and a company into an individual account without an endorsement?

My husband goes every day with constant ongoing anxiety. He’s at the point where all he does is lay in bed. He’s on Pristiq and tapering seroquel. Any advice?

Anyone taking this, what’s the best time of day to take it? It’s for my husband and this is the 2nd time he’s taking it. First time it really helped.

Anyone take this and have heart problems? My husband was prescribed it but had an open heart surgery on 5/8 and an ablation 2 days ago. He also has high blood pressure and diabetes 2.

My husband has been in a major state of anxiety/depression for about 2 years. Recently, he’s had open heart surgery. He’s presently on amantadine, seroquel, Pristiq and memantine. When he wakes up, he immediately goes into an anxiety mode….every single day. However, when he picks up a book or word search, he’s focused and no longer in that excruciating anxiety state. What is happening??

Anyone have ECT after OHS? If so, how long after surgery did you have to wait before starting treatment?

My hubby’s surgery is scheduled for this Friday, 5/8. If you could please give me recommendations for before, during and after, I’d appreciate it, like: -things to bring to the hospital -tricks that helped with recovery Thanks❤️‍🩹
