⚜️All pending Apo are going out today

New Life has told me that all pending through 5/15 orders for Big Easy will be out today (they actually said even more recent ones, but I’m more concerned about the old ones).

They also said that they don’t anticipate any stock issues and their turn around times should be quick.

⚜️Platform / Issues update

Hey y'all,

I know we're not knocking it out of the park like we usually do, and I want to give you an update on what's impacting us at the margins.

Platform:

  1. Our new portal has a significant issue on two fronts. For whatever reason, the "magic link" isn't being sent when patients request it. We're getting a lot of emails about this and escalating every single one to the platform. We don't have an ETA on a fix, and there's no interim workaround short of requesting a live appointment, which routes you back to our old platform, Healthie.
  2. Certain patients who exit the new signup are getting archived by the platform and have to be reactivated. The platform still can't tell us why and is investigating. They've suggested a temporary workaround of creating an alias email address (the "." or "+" trick), but that's not ideal.
  3. The platform and our pharmacies had communication issues around prescription modifications, which left a number of prescriptions pending. That's been resolved, and those should all push out today or tomorrow.
  4. The platform isn't showing real-time shipping information. Patients are often getting texts from the pharmacy (and sometimes the prescriptions themselves) before anything updates in the new portal. They're working on it.

On the positive side: when the platform works, it works wonderfully. It's making it easy for our staff and physicians to push prescriptions out quickly, and it's been smooth sailing for the overwhelming majority of patients. Still, that's not good enough.

Pharmacies:

  1. Apo/New Life has told us they should be fully caught up and back to a 2-3 day turnaround this week, and potentially back to next-day turnaround next week. I previously outlined their delayed shipping issues here, and those issues impacted every platform, not just us. When we get inquiries through the portal, we do ask for updates, but New Life is buried with patients from us and every other platform they serve. They simply don't have updates to give us, which means we don't have updates to give you beyond the timelines we share publicly. This isn't us or our staff being evasive; and I promise you they aren't trying to be rude. We genuinely don't have more to share, and they are just as frustrated as you are on not being able to help.
  2. As mentioned previously, to help meet demand they prioritized the larger 36mg and 72mg vials, which inadvertently delayed patients prescribed 18mg vials. We told them to go ahead and send the larger 36mg vials instead. Those substitutions started Friday and should wrap up today and tomorrow, with orders going out as they complete.

I cannot stand not being able to get your medication to you faster, and I HATE sharing commitments from others that then don't get fulfilled.

We're working on adding more processes and pharmacy partners to prevent this from happening again, but with how chaotic this space can be, I can't promise it won't come up again. What I can promise is that we'll work nonstop to minimize delays and keep improving our platform and procedures so the impact on you stays minimal.

There's a concept I push with our team: 90% satisfaction isn't good, and 99% isn't even good enough. To put it in perspective, if we had 3,000 patients in a month (not saying that's our number, just roll with me), 90% means 300 are unhappy. 99% still means 30 are unhappy. Even 99.9% means 3 are unhappy. This needs to work reliably, damn near perfectly, every single time. We may never be perfect, but that's always the goal, and if we can't get it right, we'll make it right.

We spent a lot of time walking through this philosophy with our platform and pharmacy partners over the weekend.

We're working on it folks, please bear with us as there is only so much control and information that we have on some of these items.

Apo Shipping update

Hey y’all -

New Life has represented to me that they should be all caught up on orders through today by Monday.

There are a number that were delayed to today that are going out today for Saturday delivery.

They say they should be back to 2 day turnaround times by mid next week.

Our staff is inundated with messages, and some are getting missed, while others are taking longer than we would like to be able to answer. We’re doing all we can, but please understand we cannot control the pharmacy.

Please don’t hesitate to reach out, but this is the most up to date information I have.

⚜️Y'all know "Big Easy" is a reference to New Orleans, right?

I had two separate conversations today with friends out of town that didn't realize that the "Big Easy" in Big Easy Weight Loss is a reference to the most common nickname for our hometown of New Orleans, rather than just a really terrible pun. Besides, New Orleans Weight Loss would have been abbreviated as NO Weight Loss, which, you know, didn't seem all that great of a marketing proposition.

I thought putting the fleur-de-lis would give it away, but i guess not 😂

In other news, we're hiring more support staff to try to keep things moving smoothly. Stay tuned!

⚜️Apo Shipping update! / more updates on our new intake

Hey y'all,

New Life / Apothecary status: They're nearly caught up. A handful of prescriptions got delayed because they required a smaller vial size that Apothecary no longer stocks. Those are shipping today and tomorrow.

They reached out to ask whether we'd prefer to send fewer, larger vials instead. We said no. We're sticking with 3 vials over 3 months to stay within the 28-day BUD guidance, even if that means the vials run larger than necessary. I don't think they're used to platforms prioritizing the patient over operational convenience like that. Unfortunately, this did add a day or two delay to this whole process.

After tomorrow, they should be back to their normal turnaround time.

Other updates:

  • OptioRx: still waiting on integration into the flow, hopefully tonight.
  • BPI / Apothecary: working on adding 5 and 6-month options.
  • Live Care demo tomorrow. If it goes well, we can start migrating everyone over and finally retire Healthie (no more juggling two portals).

⚜️Billboard Question: How Big is your ‘BUT’?

So we got a good one: “How big is your ‘but’?”

But I am curious - what percentage of people you think are going to take this wrong way from the spelling of “But”? 😂

Probably need a tagline. “Don’t let your excuses hold you back” or something similar.

Also - made a couple of enhancements to the the new intake and we are squashing bugs left and right.

BPI is live in the flow, and we are working on getting OptioRx added still. We are also working on a way to sign up for the new portal without having to request a new refill.

⚜️Portal confusion - Links to each portal and when to use them.

We're working on trying to get the portal language cleaner, and get more functionality moved to the new portal so we can quickly retire healthie. I know this is creating some confusion, so i thought I'd post this here. We have added BPI to the new intake, and OptioRx should be added tonight, so we're slowly and surely getting there. We're also demoing the new 'live' appointment functionality.

New Life has informed us they are out of Apothecary vials till 5/11. On 5/11 (and weeks following) we will have dedicated stock for Big Easy so this will not be an issue.

If you are awaiting your Apothecary order we have a few options:

  • You can continue to wait and have your prescription processed 5/11
  • We can substitute your order with another 503(b) with the identical formulation (we won't post the name here)
  • We can change it to one of our other available pharmacies
  • We can refund your prescription

Let us know in the portal what you would like, and we will take care of you.

I know this is not ideal, but it's been a bit of an adjustment for pharmacies to keep up with the changing landscape.

We appreciate your patience, and look back to getting this a bit more smoothly soon!

We're rolling out more of our new async platform (meaning no live interaction with a physician). Please read through the details below before using it.

What's available on the new flow:

  • 12-week prescriptions only (24 week & 4 week are coming soon)
  • Boudreaux's (with Glycine) , NewLife/Apothe, & Hallandale is supported
  • Live updates of your prescription status with tracking reminders
  • Your prescription and dosing instructions right in your portal

What's NOT available on the new flow:

  • BPI, OptioRx (coming soon), Seven Cells, and Miller's
  • NAD, Sermorelin, and other non-GLP treatments

For anything not supported above, use joinbigeasy.com/join or book an appointment through the Healthie portal.

Existing patients: You can now request refills through the new flow. It will ask some of the same intake questions you've answered before.

New to GLPs: You'll be routed to our old platform (Healthie) for a live visit.

Brand new to Big Easy:

  • If you're in DC, GA, LA, MS, NM, ND, TN, VA, or WV, you'll be routed to Healthie for a live phone or video appointment with a physician. (We're still researching whether we can lift this requirement for these states.)
  • If you don't have proof of your existing prescription, you'll be routed to our old platform.
  • Everyone else is welcome to use the new flow!

Already have a live appointment scheduled but want to switch to async? If your appointment is not within the next 24 hours, go ahead and schedule with the new link and message us in the portal and we'll get you sorted.

We're still building out the live flow on the new platform so we can eventually retire Healthie. Not quite there yet, but soon.

Known issues:

  • If you've used other telehealth services, texts may come from a number you recognize from another platform. We're working on it.
  • The new platform and Healthie don't talk to each other.

This is still a new platform, so expect some bugs. We'd love your help finding them, and we appreciate your patience with any quirks along the way.

Someone tagged me elsewhere on this. FDA is proposing to exclude sema, TZ, and liraglutide from the 503B Bulks List. The thing is, these aren't currently on the bulks list, which means 503(b)s aren't supposed to be compounding them from bulk in the first place (outside the separate shortage pathway, which is its own can of worms).

What's notable is FDA walked through and rejected arguments the OFA raised. A formal denial is a stronger posture than the current "hasn't decided" status, even if the practical effect on 503(b)s is similar.

Comments are open through June 30. After that, FDA reviews and issues a final determination. They are supposed to do it within 180 days, but they routinely miss that. Speculating on timing: earliest realistic finalization is early-to-mid 2027, more likely late 2027. If FDA refers it to the Pharmacy Compounding Advisory Committee (the notice flags this as possible), 2028 is in play. And whatever FDA finalizes, OFA will almost certainly challenge in court.

This does not impact 503(a)s

Curious to see how the Bs respond to this one. There are always more ways to approach this.

Hey y'all -

We sent our first 30 orders via our new platform this past week! Thanks to everyone that participated in the beta!

Several bugs to work out, but all and all, it was a pretty good experience from the general feedback we've received.

We're working on making sure some of the bugs get worked out, but are optimistic about launching it live for all Big Easy patients that would like a refill of their prescriptions without a live physician visit shortly.

For new patients to Big Easy, we're working on adding a few features that the physicians have requested. Should be up and running shortly after the existing patient part is complete.
