Hey y'all,

I spoke at length with the CEO over at New Life (the folks fulfilling both Apothecary and Hyalo) and wanted to pass along what's changing. They took the feedback from this community seriously and moved fast on it.

Quick refresher for anyone new: Apothecary came on board in April, Hyalo joined at the end of July, and in August we moved New Life fulfillment to 100% Hyalo.

Replacements (our policy, not just theirs)

If you ever receive a vial from any of our pharmacies with a crimping or underfill issue, reach out and we'll get it replaced. This is not new. We've always said it publicly, and it's the same position we took back during the ProRx recall (even before we realized we were not impacted). It's not changing.

What New Life is changing

Crimping. The loose metal ring problem has been addressed. New crimping machine installed, and they're now sourcing vials from a different supplier. They've also instructed their staff to keep an eye out and check while fulfilling.

Underfill. Every vial now getting a 10% overfill, so you should be getting full volume with margin to spare. Worth noting this is unusual for a 503(b). You normally only see overfills like this from 503(a)s.

COAs. Every lot is already going to Eagle Analytical (one of the leaders in the space), an FDA-registered lab, on top of their existing internal testing. They're working on making those results more available to patients.

These folks are here for the long haul. They're genuinely receptive to the feedback y'all put out here, and they know they're where they are because of this community - they've been reading since the day they were announced here a few months ago.

Center City is making changes too

After seeing the reaction to their 10mg/1ml concentration, Center City is now testing a 15mg/ml concentration (meaning one injection instead of two at higher doses). Should be available in a few weeks, and I'll announce when it's live for prescriptions.

FWIW, when I asked why any pharmacy would go under 15mg/ml, they pointed to a good amount of microdosing business. I dunno. Seems to me microdosers would still appreciate a smaller 5- or 10-unit injection. But hey, we're getting what we asked for, so I'm good.

For any pharmacies reading this: I still maintain 20mg/ml is the sweet spot, with a .5ml vial as the smallest size.

Boudreaux's is now available in Massachusetts

Massachusetts seems to take a minute whenever a pharmacy has a sterile renewal there. Something to keep an eye on if you're in that state.

As promised, here's the new pricing. If your order was processed starting this morning with New Life, Hallandale, Boudreaux's, or OptioRx, you're getting the new rate. Center City is just about ready to launch, we'll launch it on our old platform if need be, they just asked for a few more days.

Same as every price cut before this one: I intend for it to be permanent. That's always a function of our pharmacy pricing holding steady and everything running the way it should, but the intent is permanent.

This one is a bit of a deviation for us. I've previously held all pharmacies to the same price, and we're going to try it this way instead to see how it goes. My hope is that it keeps driving prices down across the platform.

If it goes well, this probably won't be the last cut. We're also looking at pricing live and async visits differently to better accommodate folks on a budget. We also have a number of pharmacies lined up to add in once we go live on our new platform that will be competitively priced.

We're still working with OptioRx on their pricing. We're also looking at automating the shipment of their third vial, so we can bring them in line as a 12 week program comparable to everyone else.

One thing I want to be upfront about: the new rate applies to new orders going forward, not retroactively. I know that's frustrating if you refilled last week or you're partway through an installment plan, and I wish I could reach backward with it. The honest reason is that we only just secured this pricing, and it wasn't our cost when those orders were processed. Installments are something we offer to make this easier to manage month to month, not a plan that re-prices itself when rates move, so those stay where they were set.

And to repeat what I've said before: price isn't the thing we're competing on. Project zero is still the point. Zero errors, zero lapses in service, zero friction getting your prescription, zero reasons not to get high touch care. A lower price is good, but it only means something if everything around it works, and that's still where most of our attention goes.

Hey y'all,

Still working through some last minute issues with the new platform. We keep squashing bugs and then finding new things that need fixing. Our team still has some scar tissue from our last attempted transition to Bask, so we're making sure they're as comfortable and confident as possible in the new platform before we launch. That may not win me votes on Reddit in the short term, but our team is absolutely phenomenal, and my respect for what they do day in and day out to care for y'all runs deep. If we're going to be here for the long term, I need them along for the ride. I want them happy so they can keep you happy.

The good news is that the tech team we're working with is incredibly responsive, and it's making me excited about what we'll finally be able to build on the technology side for service and care. This has been our biggest pain point since inception, and I truly can't wait for it to stop being an obstacle. I'll have a better idea on the timeline tomorrow, but we're close.

After we launch, one of the first enhancements we'll tackle is pulling lot/batch numbers from the pharmacies into our portal. As long as we can get it through their API, you'll be able to see it right in your portal. There was a comment on another thread where a patient said a platform told them their medication wasn't recalled when it actually was (not us). Our team spent the day contacting patients who received medication in the week following the recall to verify none of them got a recalled lot. But the whole thing made me rethink how dependent we are on what pharmacies tell us, and if there's a way to remove that dependency, all the better.

We're also working on the new Central City integration. We're the first to integrate with them, so it's taking a bit longer to work out the kinks (I need to stop being so optimistic about timelines), but we'll have them up and running shortly as well.

I know there have been a lot of price cuts out there, and we'll be cutting pricing with our pharmacies on Friday. We're never going to be the cheapest option, but our focus will continue to be what I've started calling "project zero": zero errors, zero lapses in service, zero friction in getting your prescription, zero reasons not to get high touch care, zero issues accessing medical care, zero issues finding information relevant to your health on our platform, and longer term, zero reasons to go elsewhere for your primary and functional medicine care.

Reddit tends to focus on price more than most places do, but our focus will always be making this as safe, sustainable, social, and smart as possible.

I truly appreciate y'all, and the trust you place in us for your medical care is never lost on me or our team. On behalf of the whole krewe, thank you.

I just received word that Boudreaux’s initiated a recall for lot 226980 of TZ 20mg/Glycine 5mg/ ml injection solution (3ml) dispensed 7/27/26.

I don’t have any other information at this time, other than it is represented to me that it is a small number of vials and impacted a limited number of patients, none of which were ours. We are currently at a 1.4% usage rate amount patients for Boudreaux’s over the past 3 months, which is likely why we were able to get crossed off their list quickly for impacted patients.

We caught some last-minute bugs on our platform, but we're trying to hammer those out right now. Trying to make this as smooth as possible and we're still working out the integration points between Center City and our new platform. I'm really hopeful we'll open up asynch in the next day or two. Nothing ever moves at the pace I need it to - but we do want to get this right. At worst, we'll open up on Healthie, but I'd really rather not do that.

Center City Update

Center City has told me they have elected to launch with TZ + Glycine, not with B6 as they had previously told me. I have made it known to them that this community would really like them to offer a B6 formulation as well, so we'll see what happens. They are ramping up quickly, and there are a number of platforms onboarding with them, so with the right mix of demand we might be able to make that happen.

Pricing update

As our way to say 'thank you' for your patience while we onboard to our new platform, we're going to launch Central City for 12 weeks - $389 for starter, $439 or all other doses. This may be a limited time pricing, but I don't foresee it going up by much. This will launch on the new platform. It will be 28 day guidance compliant, overnight shipping, up to 90-day BUD (it may double ship depending on BUD). Because tech launches are always a bit of touch and go, I promise that we will launch it regardless next week on Healthie if our new platform isn't ready to go. But I'm feeling good about the new platform. As this is something I actually have control over, I am not going to string y'all along, so I'm setting that drop dead date of when this will launch.

I answered this in a thread, but thought it deserved top billing.

We will be announcing price drops for all (or nearly all) our pharmacies with our new platform launch.

At that time, we’ll also be launching the new pharmacy that was just announced (Central City), Southend, and also new Hyalo/New Life formulations for TZ with Cyanocobalamin (B12) and a TZ with Glycine. Serm aND NAD+ will be added likely in the week following, along with a few other pharmacies.

We are waiting till the new platform is launched for the price cuts and new pharmacies because we want to make sure we can handle the influx. Our processes are incredibly manual right now. A slew of new patients would drop our level of care and service, and that isn’t something we are interested in. I really would sooner we shut our doors than sacrifice on those margins. Our hope is with more automation, we’ll be able to transition to much more of a concierge model of care with more expanded services than where we are now.

This hasn’t been my finest week on Reddit, but I’m committed to making your medical care as safe and sustainable as possible.

In my conversations with these pharmacies to this point, I thought this arrangement would be akin to Empower, Olympia, or New Life's dual 503(a) and 503(b) set up, however that is not the case - this is a separate entity. I felt I was being pretty intentional in the questions I was asking, but perhaps I wasn't. Regardless, this is going to be a brand-new pharmacy integration for us, and I'm sure, due to the backers and pedigree involved, you'll see a number of platforms pick them up. Hence, being precise in how I needed to clean this up. But please, inquire below and I'll answer the questions with the best possible intent.

Y'all, I really can't emphasize how little I like keeping a secret. I literally shop for my wife on Christmas Eve, so I don't botch that surprise every year. But FINALLY I can share this.

The team behind a prominent industry pharmacy team now has two 503(a) pharmacies: Center City Pharmacy out of West Palm Beach, FL, and TruPharma out of Sunrise, FL.

Details are still getting finalized, but we're aiming to launch with them Tuesday on our new platform (more on that later).

What's the same:

• Same active pharmaceutical ingredients sourcing as their former formulation
• Same 20mg/ml concentration with B6 (still nailing down final vial sizes)

Live at launch: 37 states, plus DC, Guam, and Puerto Rico (they are licensed for the U.S. Virgin Islands, but we still don't have a physician). Full list below (California will launch approximately one week after we launch - licensing is finalizing).

In progress, could pop up at any time: Alabama, Arkansas, Connecticut, Kentucky, Louisiana (%#$@&), Massachusetts, Michigan, North Carolina, South Carolina, Vermont, Virginia.

Mississippi and Washington are not in scope right now.

Both of these look to be a much cleaner integration of a 503(a) and 503(b).

Hey y'all,

I owe you all an apology for yesterday.

When the picture went up, I immediately looked into it and spoke to Boudreaux's leadership that evening. What I found was reassuring, but it wasn't mine to share, so I offered to Boudreaux's to share their own statement instead the next morning. My thinking was that this community deserves direct answers from the pharmacies and platforms it relies on, and that pushing for that is how any of this gets better.

The statement wasn't as full as it should have been, and while I told them it wasn't going to make anything better, I did tell them I would post it. I shouldn't have passed along something I couldn't fully stand behind. That left me giving evasive answers to straight questions, which isn't who I am or who I want to be here. I've truly tried to be as transparent and honest with this group over 2 years ago.

The bigger fumble was the packaging question itself. I went to standards, legality, and regulation, in hopes that I could ease some of the concerns about patient safety, but in doing that I buried the standard I'd actually like us to be judged on: higher standards and patient care. Those are the ones that count, and I lost sight of it.

All that is to say, I'll do better, and also in turn, continue to demand better for y'all.

What's paramount is you feel comfortable with your prescription.

If you're a Big Easy patient with an active Boudreaux's prescription and if you'd like packaged syringes, message us in the portal. We'll overnight them to you at no cost from one of our partner pharmacies.

Boudreaux's is moving to prepackaged syringes. Anything through Big Easy from Boudreaux's starting Tuesday should have the new prepackaged syringes. For all the pending orders that may be out for fulfillment or shipment, we'll keep sending packaged syringes to anyone who asks.

Also, as this is a standard the community is overwhelmingly demanding, this will now be a standard that is asked of any pharmacy we onboard moving forward.

I am here to continue to advocate for patients, providers, platforms, and for compounding as a whole. I'll continue to push for higher standards and greater transparency, and finding great pharmacies to introduce to the community.
