Je souffre depuis quelque temps d’une douleur inquiétante derrière le sternum, sous forme de serrement intense et oppressant. Je n’ai pas les brûlures classiques de type “brûlures d’estomac” — la sensation est plutôt une pression, une constriction, un type de douleur très proche de l’angine de poitrine. Par moments, cela ressemble vraiment à un problème cardiaque, ce qui est très anxiogène.

La douleur est très imprévisible : elle peut apparaître au repos (assis ou allongé), mais aussi à l’effort, même lors d’une simple marche. J’ai également parfois une douleur sourde dans la mâchoire et les dents en même temps que l’oppression thoracique, ce qui rend la situation encore plus inquiétante.

En raison de ces symptômes, j’ai effectué un bilan cardiaque complet :

3 ECG de repos : normaux

Échocardiographie : normale

Test d’effort : excellent (12.4 METS, 18% d’inclinaison, fréquence cardiaque max 185 bpm soit 117% du prévu), sans anomalie

Mon cardiologue a donc confirmé que mon cœur est sain, mais mentalement c’est toujours difficile car les sensations restent très convaincantes.

Pour explorer une cause digestive, j’ai réalisé une fibroscopie (FOGD) avec biopsies. Voici le résumé :

Endoscopie (FOGD) :

Œsophage inférieur : très rouge et inflammé, sans ulcère, ligne Z irrégulière

Estomac : hernie hiatale de grade 1 (Hill), pangastrite diffuse

Mes médecins pensent que l’inflammation de l’œsophage et la petite hernie hiatale pourraient provoquer des spasmes œsophagiens ou des douleurs référées plutôt qu’un reflux classique avec brûlures.

Biopsy conclusion:
Mild chronic inactive gastritis associated with H. pylori infection, without malignancy.

My concern is that despite the reassuring cardiac tests, I still struggle mentally with the squeezing “angina-like” sensation because it can feel so intense and convincing, especially when walking or during physical activity. My doctors believe the inflamed esophagus and small hiatal hernia may be causing esophageal spasms or pressure sensations rather than classic burning reflux symptoms.

Has anyone else had H. pylori, gastritis, reflux, or a hiatal hernia cause severe squeezing chest pain without actual heartburn — especially pain triggered by walking or movement?

And for those treated for H. pylori, how long did it take after antibiotics and acid-reducing treatment for the chest tightness/spasm sensations to improve?
