So one resident I know(from India and on j1) got this email. It's just a routine email from ecfmg but updating on the j1 situation that matched applicants are facing this year.
Note that this isn't something new. Around 3-4 years back there was a massive visa rejection trend for Pakistan too and at that time organizations like APPNA and so on stepped in to pull diplomatic strings and most residents joined albeit many joined late.

It just means you'll do consular processing and that's all.
After residency/fellowship you'll shift to h1b. The six year h1b can is sort of extended via approved pniw i140.
Instead of adjustment of status you'll do consular processing if that is expanded to h1b(very likely).
So you send your documents to nvc and once your date is current and the consulate has spots they will call you in and you get a medical done plus background checks and an interview.
I had my GC via consular interview during the current administration
Slower process compared to aos and more vetting.
Once your GC is approved you'll get an immigrant visa that acts as your GC once the officer at port of entry stamps it and till your actual gc comes in.
The downside is that it's not convenient as you have to wait for the consular interview( but hey you'll wait while in the US on h1b) and you will have to be in home country for medical and also interview.
It isn't the end of imgs on visa if that is expanded.
